深部热疗对放射性肺炎预防作用的临床研究

摘    要:目的:观察深部热疗能否降低肺癌胸部放射治疗患者放射性肺炎的发生及其影响因素。方法:选取80例行胸部放射治疗的肺癌患者,分为热疗组和非热疗组。热疗设备使用HG-2000体外局部高频热疗机,加温温度42 ℃~43 ℃,时间为40 min,每周2次,共8~12次。结果:热疗组与非热疗组的放射性肺炎发生率分别为12.5%和35.0%,热疗组明显低于非热疗组(P<0.05),差异具有统计学意义;单因素分析显示热疗、放射治疗总剂量及V20与放射性肺炎的发生有直接关系;二分类Logistic 多因素回归分析显示V20(P=0.004)、热疗(P=0.011)为放射性肺炎发生与否的独立影响因素。结论:深部热疗可以降低肺癌患者胸部放射治疗时放射性肺炎的发生率,且放射性肺炎的发生与放射治疗总剂量及V20相关。多因素分析结果示V20、热疗为放射性肺炎发生的独立影响因素。

Abstract:Objective:To observe whether the deep hyperthermia can reduce the incidence of radioactive pneumonia on lung cancer patients and research its influence factors.Methods:A total of 80 patients with lung cancer who received radiotherapy were classified according to whether received deep hyperthermia.Heat treatment equipment use the HG-2000 local high-frequency hyperthermia.Heating temperature was 42 ℃~43 ℃ for 40 minutes,2 times a week,a total of 8~12 times.Results:The incidence rate of radioactive pneumonia in deep hyperthermia group was 12.5% compared to 35.0% in another group,deep hyperthermia group was significantly lower than another group (P<0.05).Single factor analysis showed that the total dose of hyperthermia and radiation therapy and V20 were directly related to the occurrence of radiation pneumonitis.Binary Logistic regression showed V20(P=0.004),hyperthermia(P=0.011) were independent factors for radioactive pneumonia.Conclusion:Deep hyperthermia can reduce the incidence of radiation pneumonitis in patients with lung cancer,and the incidence of radiation pneumonitis was related to the total dose and V20 of radiotherapy.Multivariate analysis showed that V20 and hyperthermia were independent influencing factors of radiation pneumonitis.
