恶性淋巴瘤中细胞凋亡与增殖的特征及意义

观察恶性淋巴瘤中细胞凋亡与细胞增殖的特征，探讨其发生机制。用TUNEL法检测调亡细胞，观察其特征，以每平方毫米面积中的凋亡细胞数作为凋亡指数（apoptoticindex，AI）；用枸椽酸-微波-ABC法检测PCNA表达并以PCNA阳性细胞百分率作为增殖指数（PCNAindex，PI）。结果：与反应性增生相比，在大多数淋巴瘤类型中AI显著减小，PI显著增大，而AI／PI在所有淋巴瘤类型中均显著减小。结论：所用检测方法客观而敏感；细胞凋亡减少是恶性淋巴瘤发生的重要原因，特别是小淋巴细胞性、滤泡性、弥漫小裂细胞性淋巴瘤发生的主要原因；AI／PI可能是诊断恶性淋巴瘤、评价其生物学行为的可靠参数。

CHARACTERISTICS AND SIGNIFICANCE OF APOPTOSIS AND PROLIFERATION IN MALIGNANT LYMPHOMAS

PURPOSE To observe the characteristics of apoptosis and proliferation in malignant lymphotna and toexplore the mechanism of development of lymphomas.METHODS Apoptotic cells were identified by the terminal deoxynucleotidyl transferase-mediated dUTPnick end labeling (TUNEL) method and the number of apoptotic cells in 1 mm2 of neoplasm was counted asapoptotic index (Al); Using citric acid-microwave-ABC immunohistochemical method, PCNA expressionwas detected and the percentage of positive cells was used as proliferative index (PI).RESULTS Compared with reactive hyperplasia, AI was significantly reduced (P<0. 05 ) and PI was increased (P< 0. 05) in most lymphoma subtypes. The Al/PI ratios in all types was remarkably reduced (P<0. 01).CONCLUSIONS The detective methods are objective and sensitive. Reduced apoptosis is an importantreason in the development of lymphomas and specially is a main mechanism in small lymphocytic, follicular, diffuse small cleaved cell lymphomas. The research provides reliable data for the study of apoptosis induction andgenes regulation in treatment of malignant lyrnphomas.
