Neuronal mitochondrial dysfunction increases inflammatory mediators and leads to free radical generation and anti-oxidant enzymatic alterations,which are major neuropathological hallmarks responsible for autism.Mitochondrial dysfunction in autism is associated with decreased ATP levels due to reduced levels of cyclic adenosine monophosphate.Rat models of autism were established by intracerebroventricular injection of propionic acid.These rat models had memory dysfunction,decreased muscle coordination and gait imbalance.Biochemical estimation of propionic acid-treated rats showed changes in enzyme activity in neuronal mitochondrial electron transport chain complexes and increases in pro-inflammatory cytokines,oxidative stress and lipid biomarkers.Oral administration of 10,20 and 30 mg/kg adenylate cyclase activator forskolin for 15 days reversed these changes in a dose-dependent manner.These findings suggest that forskolin can alleviate neuronal mitochondrial dysfunction and improve neurological symptoms of rats with autism.This study was approved by the RITS/IAEC,SIRSA,HARYANA on March 3,2014(approval No.RITS/IAEC/2014/03/03). 相似文献

To determine percent of patients without malignancy and ≤ 40 years of age with high cumulative radiation doses through recurrent CT exams and assess imaging appropriateness. From the cohort of patients who received cumulative effective dose (CED) of ≥ 100 mSv over a 5-year period, a sub-set was identified with non-malignant disease. The top 50 clinical indications leading to multiple CTs were determined. Clinical decision support (CDS) system scores were analyzed using a widely adopted standard of 1–3 (red) as “not usually appropriate,” 4–6 (yellow) “may or may not be appropriate,” and 7–9 (green) “usually appropriate.” Clinicians reviewed patient records to assess compliance with appropriate use criteria (AUC). 9.6% of patients in our series were with non-malignant conditions and 1.4% with age ≤ 40 years. CDS scores (rounded) were 2% red, 38% yellow, 27% green, and 33% unscored CTs. Clinical society guidelines for CT exams, wherever available, were followed in 87.5 to 100% of cases. AUCs were not available for several clinical indications as also referral guidelines for serial CT imaging. More than half of CT exams were unrelated to follow-up of a primary chronic disease. We are faced with a situation wherein patients in age ≤ 40 years require or are thought to require many CT exams over the course of a few years but the radiation risk creates concern. There is a fair number of conditions for which AUC are not available. Suggested solutions include development of CT scanners with lesser radiation dose and further development of appropriateness criteria. 相似文献

Understanding the socioeconomic impact of chronic graft-versus-host disease (GVHD) on affected patients is essential to help improve their overall well-being. Using data from the Chronic GVHD Consortium, we describe the insurance, employment, and financial challenges faced by these patients and the factors associated with the ability to work/attend school and associated financial burdens. A 15-item cross-sectional questionnaire designed to measure financial concerns, income, employment, and insurance was completed by 190 patients (response rate, 68%; 10 centers) enrolled on a multicenter Chronic GVHD Consortium Response Measures Validation Study. Multivariable logistic regression models examined the factors associated with financial burden and ability to work/attend school. The median age of respondents was 56years, and 87% of the patients were white. A higher proportion of nonrespondents had lower income before hematopoietic cell transplantation and less than a college degree. All but 1 patient had insurance, 34% had faced delayed/denied insurance coverage for chronic GVHD treatments, and 66% reported a financial burden. Patients with a financial burden had greater depression/anxiety and difficulty sleeping. Nonwhite race, lower mental functioning, and lower activity score were associated with a greater likelihood of financial burden. Younger age, early risk disease, and higher mental functioning were associated with a greater likelihood of being able to work/attend school. In this multicenter cohort of patients with chronic GVHD, significant negative effects on finances were observed even with health insurance coverage. Future research should investigate potential interventions to provide optimal and affordable care to at-risk patients and prevent long-term adverse financial outcomes in this vulnerable group. 相似文献

Family caregivers are essential partners for patients undergoing hematopoietic cell transplantation (HCT). The caregiving role is emotionally, physically, and financially demanding. Intervention efforts to provide relief for caregiver stress during HCT are highly warranted. Storytelling interventions are accruing evidence for efficacy in therapeutic contexts. The purpose of this study was to conduct a 3-full consecutive day digital storytelling (DST) workshop to build knowledge on caregivers’ lived experiences during HCT, to pilot test DST with a small group of HCT caregivers, and to demonstrate feasibility and acceptability using qualitative and quantitative measures. Six adult caregivers of allogeneic HCT recipients (mean age, 60.2 years) attended a 3-day DST program (66% female, 83% white). All successfully created their personal audiovisual digital story (2 to 3 minutes long) and completed a survey. All participants rated the DST workshop as highly acceptable and therapeutic (mean score 5, on a scale of 1 to 5). Group discussions and interviews with participants further demonstrated high satisfaction and acceptability of the workshop format, setting, process, and structure. The survey results showed decreases in anxiety and depression from before to after the DST workshop with all participants showing change in the expected direction. This study demonstrates the feasibility and acceptability of a 3-day DST workshop as a distress-relieving tool for HCT caregivers. Future research is needed to test the efficacy of DST relative to a control condition. 相似文献

Cavity preparations that are finished with hand instruments allow significantly less marginal leakage than those which are completed only with rotary instruments. This does not mean that hand-instruments walls are perfectly smooth; it only means that these walls allow less marginal leakage. Cavity preparations restored with composite resin showed more marginal leakage than those restored with amalgam. Also, the degree of marginal leakage was greater in cavity preparations restored with composite resin than in those restored with amalgam. Cavity preparations are more prone to marginal leakage at sharply defined cavosurface acute angles than at the smooth or straight surfaces of the cavity. This pattern of marginal leakage was true for both amalgam restorations and composite restorations, but the degree of marginal leakage was greater when composite resin was used as a restorative material. Since less marginal leakage was found in the finished cavity preparations, it is obvious that all cavity preparations must be finished with hand instruments. This may not completely eliminate the problem of marginal leakage, but it will reduce the potential secondary caries both in degree and frequency. Also, since sharply defined acute angles did exhibit more marginal leakage, this part of the cavity preparation needs special attention. With amalgam and composite restorations at least, these angles should not be sharply defined but should be slightly round, so that better condensation of restorative materials can be obtained. The success of restorative procedures is dependent on comprehension of the responses of enamel and dentin, including responses to masticatory forces. The regional variation resulting from masticatory forces is critical because clinically it relates to the thickness of enamel and dentin occlusogingivally. Three-dimensional finite element models of an intact mandibular molar were developed to analyze stresses in enamel and dentin occlusogingivally, buccolingually, and mesiodistally. There were dramatic regional variations in the magnitude and character of different stresses caused by masticatory forces, and despite being organically "bonded," enamel and dentin responded independently. A three-dimensional, linear, elastic finite element model of a maxillary first premolar from longitudinal ground sections was developed to investigate stress variation in the enamel and dentin adjacent to the dentinoenamel junction (DEJ). The effect of regional variation in the contour of the DEJ on the stress patterns for enamel and dentin was also analyzed. The normal (compressive or tensile) and shear stresses in the dentin and enamel surfaces of the DEJ were computed for a vertical load of 170 N acting on the entire occlusal surface of the model. The normal stresses in dentin and enamel were maximum on the occlusal surface of the model and diminished along the buccal and lingual surfaces of the DEJ. However, the magnitude of the normal stresses increased at the cervical enamel, which also showed increased values for shear stress distribution. The normal and shear stresses were markedly affected by the contour of the DEJ and the thickness of enamel in the occlusal third on the buccal and lingual surfaces. The results suggested that because the mechanical interlocking between enamel and dentin in the cervical region is weaker than in other regions of the DEJ, enamel in this region may be susceptible to belated cracking that could eventually contribute to the development of cervical caries. 相似文献
