Professional psychologists are increasingly likely to encounter opportunities to work with patients and families facing end-of-life issues. Psychologists can provide psychological assessment, intervention for patients and families, consultation with and support of health care team members, grief therapy, and program development and evaluation. Psychological services are useful for healthy individuals who wish to make thoughtful plans about their own future care, patients with life-limiting illnesses, families stressed by providing end-of-life care, bereaved individuals, and health care providers who face issues of burnout and strain. Several challenges for psychologists working in end-of-life care are noted, including training, development of clear roles, and reimbursement. This article reports the development and validation of the Suicide Intervention Response Inventory, a self-administered questionnaire designed to assess the paraprofessional's ability to select an appropriate response to the self-destructive client. The Suicide Intervention Response Inventory demonstrated the ability to discriminate among three groups of 28 respondents known to differ in suicide counseling skills, and it detected enhancement of such skills in 127 volunteers over the course of crisis intervention training. As predicted, the Suicide Intervention Response Inventory also showed moderate convergence with the Counseling Skills Evaluation technique, an instrument devised to tap more general capacities for facilitative responding. These preliminary indications of the validity of the Suicide Intervention Response Inventory, coupled with its high internal consistency and test-retest reliability, suggest that it may be a useful adjunct to more general indices of counseling effectiveness in screening, evaluating, and researching paraprofessionals who deal with potentially suicidal clients. His current research interests include supply and demand issues in professional psychology.] Academic training programs in clinical psychology vary in the emphasis that they place on science and practice, and this paper examines whether these differential emphases are linked to distinctive internship outcomes. In a study of 2,130 internship applicants from clinical psychology programs, differences were noted among practice-oriented programs, balanced science-practice programs, and science-oriented programs. Against a backdrop of some similarities, a differential emphasis on science and practice within academic training programs was related to significantly different internship match rates, as well as to successful matching in qualitatively different internship settings. Results provided qualified support for future work that might further address the relationship between academic training programs and outcomes in the field of clinical psychology. Forty clinically depressed women were randomized to a running, weight lifting, or delayed treatment condition. Self-concept was assessed at baseline, pre-, mid-, and posttreatment for all subjects and at 1, 7, and 12 months for exercise groups. Significant improvements in self-concept were found for exercise groups relative to control groups. No significant differences between exercise groups were found, and improvements were reasonably well-maintained over time. Differential changes on dimensions of self-concept were not demonstrated. Previous quantitative reviews of research on the efficacy of psychotherapy for depression have included only a subset of the available research or limited their focus to a single outcome measure. The present review offers a more comprehensive quantitative integration of this literature. Using studies that compared psychotherapy with either no treatment or another form of treatment, this article assesses (1) the overall effectiveness of psychotherapy for depressed clients, (2) its effectiveness relative to pharmacotherapy, and (3) the clinical significance of treatment outcomes. Findings from the review confirm that depressed clients benefit substantially from psychotherapy, and these gains appear comparable to those observed with pharmacotherapy. Initial analysis suggested some differences in the efficacy of various types of treatment; however, once the influence of investigator allegiance was removed, there remained no evidence for the relative superiority of any 1 approach. Recent studies of vocational structure have demonstrated that experimentally provided vocational constructs are used in less complex, less differentiated ways than are Ss' personally elicited construct dimensions. The possible reasons underlying these differences are addressed in this 2-part study. Results of Study 1 supported significant differences between the use of elicited and provided constructs and ruled out 1 methodological artifact that may have accounted for these differences. Study 2 helped to isolate the personal meaningfulness of the elicited construct as an active agent that accounts for differences in the use of these vocational constructs. Findings of both studies document, for the 1st time, a means of increasing differentiation and converge to suggest the important role of personal meaning in vocational structure. Predicated on a postmodern epistemology, constructivist theories emphasize the proactive, self-organizing features of human knowing and their implications for psychotherapy. This article outlines the potential contributions of 4 clinical lineages sharing a constructivist philosophy (personal construct theory, structural-developmental cognitive therapy, narrative reconstruction, and constructivist family therapy) and briefly reviews emerging trends in psychotherapy research compatible with this perspective. Although current constructivist approaches confront problems as well as prospects, they represent a growing force within psychotherapy, contributing to the sophistication of psychological theory, the diversification of clinical research, and the extension of therapeutic technique. The relationship between cultural dissimilarity and the duration, satisfaction, and perceived effectiveness of counseling was studied in the context of 70 actual treatment dyads that varied as to the race of the counselor and the client (White or non-White). White counselors provided fewer sessions than non-White counselors, and non-White clients expressed lower levels of overall satisfaction with counseling, regardless of counselor race. No differences in counseling effectiveness were observed as measured by counselor and client ratings of perceived relief, understanding, and coping. Relative to other treatment dyads, however, White clients seeing White counselors attributed their change more to counseling than to other, outside factors. In their recent longitudinal study of youth victimization and consequent traumatization, S. Boney-McCoy and D. Finkelhor (1996) contrast their position with previous retrospective research (S. Harter, R. Alexander, & R. A. Neimeyer, 1988; M. R. Nash, T. C. Hulsey, M. C. Sexton, T. L. Harralson, & W. Lambert, 1993a), arguing that their data support the impact of victimization per se, independent of the moderating effect of family environment. Because Boney-McCoy and Finkelhor's argument may misrepresent the results of such studies, this article (a) clarifies the actual findings of previous retrospective studies of abuse, (b) suggests methodological limitations both in Boney-McCoy and Finkelhor's research and in that of S. Harter et al., 1998, and M. R. Nash et al., 1993a, that should be remedied by future investigators, and (c) argues that both retrospective clinical research and prospective community surveys converge on a common ground, namely, that specific abuse experiences can best be understood and investigated in the context of the prior, contemporaneous, and subsequent family environments in which they occur.
