Objective

To assess the effect of low-osmolar, monomeric contrast media with different iodine concentrations on bolus shape in aortic CT angiography.

Materials and methods

Repeated sequential computed tomography scanning of the descending aorta of eight beagle dogs (5 male, 12.7 ± 3.1 kg) was performed without table movement with a standardized CT scan protocol. Iopromide 300 (300 mg I/mL), iopromide 370 (370 mg I/mL) and iomeprol 400 (400 mg I/mL) were administered via a foreleg vein with an identical iodine delivery rate of 1.2 g I/s and a total iodine dose of 300 mg I/kg body weight. Time-enhancement curves were computed and analyzed.

Results

Iopromide 300 showed the highest peak enhancement (445.2 ± 89.1 HU), steepest up-slope (104.2 ± 17.5 HU/s) and smallest full width at half maximum (FWHM; 5.8 ± 1.0 s). Peak enhancement, duration of FWHM, enhancement at FWHM and up-slope differed significantly between iopromide 300 and iomeprol 400 (p < 0.05). Except for enhancement at FWHM there were no significant differences between iopromide 300 and iopromide 370 and iopromide 370 and iomeprol 400 (p > 0.05).

Conclusions

Low viscous iopromide 300 results in a better defined bolus with a significantly higher peak enhancement, steeper up-slope and smaller FWHM when compared to iomeprol 400. These characteristics potentially affect contrast timing.  相似文献

Aim

To analyze the influence of contrast dose adjusted by weight vs. fixed contrast dose in the attenuation and cost of abdominal computed tomography (CT).

Materials and methods

A randomised, consecutive, parallel group study was conducted in 151 patients (74 men and 77 women, age range 22–67 years), studied with the same CT helical protocol. A dose at 1.75 ml/kg was administered in 101 patients while 50 patients had a fixed dose of 120 ml of same non-ionic contrast material (320 mg/ml). Mean enhancements were measured at right hepatic lobe, superior abdominal aorta and inferior cava vein. Statistical analysis was weight-stratified (<60, 61–70, 71–80 and >81 kg).

Results

Aortic attenuation was significantly superior (p < 0.05) in the dose adjusted by weight group than in the fixed dose group. Patients who weighed >61 kg in dose-adjusted group, presented higher hepatic attenuation, being statistically significant in those >81 kg (p < 0.01). In dose-adjusted group, there was a savings of €4.1 per patient in patients weighing <80 kg. In patients weighing >80 kg, there was an over cost of €10.7 per patient.

Conclusions

An injection volume of 1.75 ml/kg offers an optimal diagnostic quality with a global savings of €1.34 per patient.  相似文献

Purpose

To compare test bolus and bolus tracking for the determination of scan delay of high-pitch dual-source CT pulmonary angiography in patients with suspected pulmonary embolism using 50 ml of contrast material.

Materials and methods

Data of 80 consecutive patients referred for CT pulmonary angiography were evaluated. All scans were performed on a 128-channel dual-source CT scanner with a high-pitch protocol (pitch 3.0, 100 kV, 180 mA s). Contrast enhancement was achieved by injecting 50 ml of iomeprol followed by a saline chaser of 50 ml injected at a rate of 4 ml/s. The scan delay was determined using either the test bolus (n = 40) or bolus tracking (n = 40) technique. Test bolus required another 15 ml CM to determine time to peak enhancement of the contrast bolus within the pulmonary trunk. Attenuation profiles in the pulmonary trunk and on segmental level as well as in the ascending aorta were measured to evaluate the timing techniques. Additionally, overall image quality was evaluated.

Results

In all patients an adequate and homogeneous contrast enhancement of more than 250 HU was achieved in the pulmonary arteries. No statistically significant difference between test bolus and bolus tracking was found regarding attenuation of the pulmonary arteries or overall image quality. However, using bolus tracking 15 ml CM less was injected.

Conclusion

A homogeneous opacification of the pulmonary arteries and sufficient image quality can be achieved with both the bolus tracking and test bolus techniques with significant lower contrast doses compared to conventional contrast material injection protocols.  相似文献

Purpose

To evaluate the accuracy in distinguishing adrenal adenomas from nonadenomas by means of quadriphasic CT exam, including unenhanced (UE), arterial enhanced (AE), portal enhanced (PE) and 5-min delayed enhanced (DE) CT scans.

Methods

This retrospective study had institutional review board approval; the need for informed consent was waived. From September 2007 to September 2009, 104 adrenal masses were evaluated in 87 patients (49 M, 38 F, mean age 58.4 years) undergoing UE, AE (35-s delay), PE (80-s delay) and DE (5-min delay) CT scans. The mean adrenal attenuation during all imaging phases was measured by two readers. The accuracy values of absolute unenhanced attenuation (UE), absolute wash-out (AWO), relative percentage wash-out (RPWO) and percentage enhancement wash-out (PEW) were assessed by using receiver operator curves (ROC) analysis. The overall accuracy of the quadriphasic protocol and other triphasic protocols were evaluated. A value of p ≤ 0.05 was considered significant.

Results

The accuracy in characterizing adrenal lesions was 86.5% (90/104) for UE attenuation (≤10 HU threshold), 90.1% (82/91) for RPWO (≥30% threshold), 85.7% (78/91) for AWO (≥12 HU threshold) and 83.5% (76/91) for PEW (≥30% threshold), respectively. Quadriphasic CT (accuracy 97.1%, 101/104) performed better than triphasic CT including only AE scan (efficiency 90.0%, 94/104; p = 0.011) and triphasic CT including only PE scan (efficiency 96.1%, 100/104; p = 0.025).

Conclusion

Quadriphasic CT protocol including 5-min DE scan may be used to characterize incidentally detected adrenal masses. RPWO represented the best wash-out parameter for characterizing adrenal lesions.  相似文献

Objective

In medical imaging, lowering radiation dose from computed tomography scanning, without reducing diagnostic performance is a desired achievement. Iterative image reconstruction may be one tool to achieve dose reduction. This study reports the diagnostic performance using a blending of 50% statistical iterative reconstruction (ASIR) and filtered back projection reconstruction (FBP) compared to standard FBP image reconstruction at different dose levels for liver phantom examinations.

Methods

An anthropomorphic liver phantom was scanned at 250, 185, 155, 140, 120 and 100 mA s, on a 64-slice GE Lightspeed VCT scanner. All scans were reconstructed with ASIR and FBP. Four readers evaluated independently on a 5-point scale 21 images, each containing 32 test sectors. In total 672 areas were assessed. ROC analysis was used to evaluate the differences.

Results

There was a difference in AUC between the 250 mA s FBP images and the 120 and 100 mA s FBP images. ASIR reconstruction gave a significantly higher diagnostic performance compared to standard reconstruction at 100 mA s.

Conclusion

A blending of 50–90% ASIR and FBP may improve image quality of low dose CT examinations of the liver, and thus give a potential for reducing radiation dose.  相似文献

Objectives

To investigate the relationship between cardiac output, end diastolic volume and the contrast enhancement in coronary CT angiography using 320-detector CT.

Materials and methods

A total of 38 patients underwent coronary CT angiography by using a 320-detector CT scanner (detector configuration, 320 × 0.5 mm). The attenuation value of the ascending aorta at the level of the orifice of the left main trunk was measured. The cardiac output (CO), end diastolic volume (EDV) and stroke volume (SV) were measured by echocardiography. The EDV was normalized to the body surface area (BSA). The total blood volume injected from the left ventricle from the beginning of the contrast agent injection to the time of image acquisition was determined to be the total injected blood volume (TIV), which is a product of SV and the number of heart beats from the initiation of contrast agent injection to the scan.

Results

There was a negative correlation between the attenuation of the ascending aorta and CO (r = −0.44, P = 0.0053). However, the negative correlation between the attenuation of the ascending aorta and TIV was stronger (r = −0.52, P = 0.0007). There was a negative correlation between the attenuation of the ascending aorta and EDV/BSA (r = −0.45, P = 0.0039).

Conclusion

In 320-detector CT, contrast enhancement in CCTA with a lesser amount of contrast medium decreases when cardiac output is high. Patients with larger EDV/BSA may also show decreased attenuation.  相似文献

Purpose

To assess the value of the delayed phase (DP) in pancreatic carcinomas which appear iso-attenuating in the pancreatic parenchymal phase (PPP).

Materials and methods

Fifty-seven preoperative MDCT studies of pancreatic carcinomas were retrospectively reviewed. The size of the tumors, and the Hounsfield unit (HU) of the tumors and pancreatic parenchyma were measured. The tumor-to-pancreas contrast (TPC: |HU [tumor] − HU [normal pancreas]|) was calculated.

Results

Eight cases (14.0%) showed iso-attenuation and 49 showed hypo-attenuation in the PPP. The DP images revealed seven of eight (87.5%) iso-attenuating tumors to be hyper-attenuating. The size of iso-attenuating tumors was smaller than that of hypo-attenuating tumors (mean ± S.D.: 12.4 ± 4.8 mm vs. 30.3 ± 9.0 mm, p < 0.0001). In hypo-attenuating tumors, TPC in the PPP (60.2 ± 24.6 HU) was higher than those in the portal venous phase (PVP, 40.5 ± 23.0 HU, p < 0.0001) and DP (18.3 ± 11.8 HU, p < 0.0001). In contrast, in iso-attenuating tumors, TPC in the DP (26.0 ± 4.9 HU) was higher than those in the PPP (9.2 ± 3.7 HU, p = 0.0003) and PVP (7.1 ± 4.7 HU, p = 0.001) phases.

Conclusion

The DP image is helpful in depicting small iso-attenuating pancreatic carcinomas as slightly hyper-attenuating tumors.  相似文献

Aim

To evaluate the accuracy of depth measurements on supine chest computed tomography (CT) for transthoracic needle biopsy (TNB).

Materials and methods

We measured skin-lesion depths from the skin surface to nodules on both prebiopsy supine CT scans and CT scans obtained during cone beam CT-guided TNB in the supine (n = 29) or prone (n = 40) position in 69 patients, and analyzed the differences between the two measurements, based on patient position for the biopsy and lesion location.

Results

Skin-lesion depths measured on prebiopsy supine CT scans were significantly larger than those measured on CT scans obtained during TNB in the prone position (p < 0.001; mean difference ± standard deviation (SD), 6.2 ± 5.7 mm; range, 0–18 mm), but the differences showed marginal significance in the supine position (p = 0.051; 3.5 ± 3.9 mm; 0–13 mm). Additionally, the differences were significantly larger for the upper (mean ± SD, 7.8 ± 5.7 mm) and middle (10.1 ± 6.5 mm) lung zones than for the lower lung zones (3.1 ± 3.3 mm) in the prone position (p = 0.011), and were larger for the upper lung zone (4.6 ± 5.0 mm) than for the middle (2.4 ± 2.0 mm) and lower (2.3 ± 2.3 mm) lung zones in the supine position (p = 0.004).

Conclusions

Skin-lesion depths measured on prebiopsy supine chest CT scans were inaccurate for TNB in the prone position, particularly for nodules in the upper and middle lung zones.  相似文献

Objectives

To compare vascular and parenchymal contrast enhancement in multidetector computed tomography of the liver using two contrast media with different iodine concentration (Iodixanol 320 mgI/mL and Iomeprol 400 mgI/mL) and similar viscosity, using fixed total iodine volume (40 gI) and iodine delivery rate (1.6 gI/s).

Methods

110 patients were prospectively randomized into two groups. Group A received 125 mL of Iodixanol 320 and group B 100 mL of Iomeprol 400.Attenuation values were measured at the level of the aorta, portal vein and liver parenchyma on unenhanced, arterial, portal and equilibrium phases. A non inferiority test was performed on the differences between the two groups. An independent reader evaluated image quality.

Results

The equivalence of the two CM was demonstrated in all measurements. Higher, but not statistically significant, attenuation values were obtained with Iomeprol 400 in the aorta during the arterial phase (305.3 HU versus 288.4 HU; P = 0.32) and with Iodixanol 320 in the liver parenchyma, during both portal (59.8 HU versus 65.5 HU; P = 0.78) and equilibrium (40.4 HU versus 41.8 HU; P = 0.55) phases.

Conclusions

Iodixanol 320 and Iomeprol 400 injected at the same iodine delivery rate (1.6 gI/s) and total iodine load (40 gI) did not provide statistically significant differences in liver parenchymal and vascular contrast enhancement.  相似文献

Purpose

To explore the feasibility of multiphase contrast–saline mixture with dual-flow injection technique for visualization of right ventricular (RV) cavity and interventricular septum (IVS) in 64-row multidetector computed tomography (MDCT) coronary angiography.

Materials and methods

Twenty-four patients underwent coronary CT angiography (CTA) imaging with 64-row MDCT. In twelve patients (group A), 60 ml contrast medium (CM) bolus was followed by 40 ml saline, and in the other twelve patients (group B), 50 ml CM bolus was followed by 50 ml contrast–saline mixture at 60:40 ratio. The CM, saline and contrast–saline mixture flow rate were all 5.0 ml/s. Two experienced radiologists measured the CT values of ascending aorta, descending aorta, pulmonary artery and RV, rated the uniformity of RV cavity, the visualization of coronary arteries and IVS independently.

Results

By Kappa test, agreement between the two radiologists was 0.93 and 0.86 concerning the CT value measurements and the grades of the three indexes, respectively. By t-test, the mean CT values of ascending aorta and descending aorta of the two groups had no statistical difference (t = 1.459, P > 0.05; t = 1.619, P > 0.05); while the mean CT values of pulmonary artery and RV cavity had statistical differences (t = 8.316, P < 0.05; t = 10.372, P < 0.05). By two-related rank sum test, according to the visualization of coronary arteries and the uniformity of RV cavity, there were no statistical differences (U = 66.00, P > 0.05; U = 54.00, P > 0.05); while according to the visualization of IVS, group B was better than group A (U = 8.00, P < 0.05).

Conclusion

In coronary CTA, a contrast–saline mixture after CM bolus can provide clear visualization of RV and IVS and LV without impairing coronary CTA image.  相似文献

Objective

The purpose of this study was to evaluate the correlation between bolus-tracking parameters and heart rate (HR) with attenuation of the ascending aorta and create a linear regression model for predicting coronary attenuation in coronary computed tomography angiography (CCTA).

Methods

A total of 50 patients (31 men, 19 women; mean age, 67.2 ± 10.8 y) underwent CCTA using a 320-detector CT scanner. A bolus-tracking scan was performed to optimize the scan timing. The average HR under normal breathing for 10 s was recorded just before the bolus-tracking scan started. Attenuation values of the pulmonary artery at 7 s (PA7) and 10 s (PA10) after the beginning of the injection were recorded during the bolus-tracking scan and the ascending aortic attenuation (CEAAo) was measured during the diagnostic scan.

Results

A positive correlation was observed between PA7 and CEAAo (r = 0.41, P = 0.003) and PA10 and CEAAo (r = 0.66, P < 0.0001), and weak negative correlation was observed between HR and CEAAo (r = −0.46, P = 0.15). A multivariable linear regression model for predicting CEAAo was evaluated, and the residual error between the predicted and the measured CEAAo was within approximately ±100 HU.

Conclusions

Coronary attenuation could be predicted using HR and pulmonary artery attenuation during the bolus-tracking method.  相似文献

Purpose

To investigate the dose reduction potential of low kV triple-rule-out dual-source CT angiography (TRO-CTA) in non-obese (BMI ≤ 25 kg/m2) patients with acute chest pain.

Materials and methods

Sixty consecutive patients were randomly assigned to two different retrospectively ECG-gated TRO-CTA protocols in this prospective trial: Thirty patients were examined with a 120-kV standard protocol (320 reference mAs with automatic tube current modulation, automatically adapted pitch and ECG-pulsing) and served as the control group (group 1), an otherwise identical 100 kV protocol was used in the other thirty patients (group 2) for a radiation dose reduction. Subjective image quality was assessed on a 5 point scale (1: excellent, 5: non-diagnostic) by two blinded observers. Quantitative image analysis assessed vascular attenuation, signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) in different vascular segments. The effective dose was calculated from the dose length product (DLP) using a conversion coefficient of 0.017 mSv mGy−1 cm−1.

Results

There was no significant difference of age, BMI, heart rate, pitch or scan length between both patient groups. Subjective image quality was rated similar in both groups (group 1: 1.2 ± 0.4, group 2: average score = 1.3 ± 0.5). Vessel attenuation was significantly higher in group 2 than in group 1 (ascending aorta: 456 ± 83 HU vs. 370 ± 78 HU, p < 0.001; pulmonary artery: 468 ± 118 HU vs. 411 ± 91 HU, p = 0.03; left coronary artery: 437 ± 110 HU vs. 348 ± 89 HU, p < 0.001), however, there was no significant difference in SNR (13.2 ± 7.6 vs. 14.5 ± 7.5, p = 0.49) or CNR (13.8 ± 6.6 vs. 15.9 ± 7.7, p = 0.25). The effective radiation dose of the 100 kV protocol was significantly lower (9.6 ± 3.2 mSv vs. 18.1 ± 9.4 mSv, p < 0.0001).

Conclusion

TRO-CTA with 100 kV is feasible in non-obese patients and results in diagnostic image quality and significantly reduced radiation dose.  相似文献

Purpose

To evaluate the reliability of attenuation values in virtual non-contrast images (VNC) reconstructed from contrast-enhanced, dual-energy scans performed on a second-generation dual-energy CT scanner, compared to single-energy, non-contrast images (TNC).

Materials and methods

Sixteen phantoms containing a mixture of contrast agent and water at different attenuations (0–1400 HU) were investigated on a Definition Flash-CT scanner using a single-energy scan at 120 kV and a DE-CT protocol (100 kV/SN140 kV). For clinical assessment, 86 patients who received a dual-phase CT, containing an unenhanced single-energy scan at 120 kV and a contrast enhanced (110 ml Iomeron 400 mg/ml; 4 ml/s) DE-CT (100 kV/SN140 kV) in an arterial (n = 43) or a venous phase, were retrospectively analyzed. Mean attenuation was measured within regions of interest of the phantoms and in different tissue types of the patients within the corresponding VNC and TNC images. Paired t-tests and Pearson correlation were used for statistical analysis.

Results

For all phantoms, mean attenuation in VNC was 5.3 ± 18.4 HU, with respect to water. In 86 patients overall, 2637 regions were measured in TNC and VNC images, with a mean difference between TNC and VNC of −3.6 ± 8.3 HU. In 91.5% (n = 2412) of all cases, absolute differences between TNC and VNC were under 15 HU, and, in 75.3% (n = 1986), differences were under 10 HU.

Conclusions

Second-generation dual-energy CT based VNC images provide attenuation values close to those of TNC. To avoid possible outliers multiple measurements are recommended especially for measurements in the spleen, the mesenteric fat, and the aorta.  相似文献

Objective

To evaluate diagnostic image quality of high-pitch dual source comprehensive cardiothoracic CT protocol in patients presenting with acute undifferentiated chest pain.

Materials and methods

Consecutive symptomatic subjects (n = 51) with undifferentiated acute chest pain underwent ECG-synchronized high-pitch dual-spiral chest CT angiography (Definition Flash, Siemens Medical Solutions, 2 × 100 kVp or 2 × 120 kV if BMI > 30, collimation: 128 × 0.6 mm, pitch: 3.2). Independent investigators determined the image quality of each cardiac and pulmonary vessel segment, measured contrast-to-noise-ratio (CNR), and determined radiation exposure. In addition, the prevalence of CT findings (pulmonary embolism (PE), aortic dissection (AD) and significant coronary stenosis (≥50%)) was determined. Univariate and multivariate analysis were performed to determine the subpopulation with highest diagnostic quality.

Results

Among 51 subjects (66% male, average age: 63 ± 15.8), the prevalence of positive CT findings was moderate (overall: 11.7%). Overall, image quality of the pulmonary, aortic and coronary vasculature was good (1.26 ± 0.43 and CNR: 2.52) with an average radiation dose of 3.82 mSv and 3.2% of segments rated non-evaluable. The image quality was lowest in the coronary arteries (p = 0.02), depending on the heart rate (r = 0.52, p < 0.001). In subjects with a heart rate of ≤65 bpm (n = 30) subjective image quality and CNR of the coronary arteries were higher (1.6 ± 0.5 vs. 2.1 ± 0.5, p = 0.03 and 1.21 ± 0.3 vs. 1.02 ± 0.3, p = 0.05) with only 1.5% segments classified as non-evaluable.

Conclusion

High-pitch dual-spiral comprehensive cardiothoracic CT provides low radiation exposure with excellent image quality at heart rates ≤65 bpm. In subjects with higher heart rates, image quality of the aortic and pulmonary vasculature remains excellent, while the assessment of the coronary arteries degrades substantially.  相似文献

Objectives

To determine whether texture analysis of non-contrast enhanced computed tomography (CT) images in apparently disease-free areas of the liver is altered by the presence of extra- and intra-hepatic malignancy in colorectal cancer patients.

Materials and methods

Hepatic attenuation and texture were assessed from non-contrast enhanced CT in three groups of colorectal cancer patients: (A) 15 controls with no malignancy; (B) nine patients with extra-hepatic malignancy but no liver involvement; (C) eight patients with hepatic metastases. Regions of interest were manually constructed only over apparently normal areas of liver tissue excluding major blood vessels and areas of intra-hepatic fat, which may otherwise alter CT texture irrespective of the presence of malignancy. Texture was analysed on unfiltered images and following band-pass image filtration to highlight image features at different spatial frequencies (fine: 2 pixels/1.68 mm in width, medium: 6 pixels/5.04 mm and coarse: 12 pixels/10.08 mm). The relative contributions made to the image by features at two different spatial frequencies were expressed as filter ratios (fine/medium, fine/coarse and medium/coarse). Texture was quantified as mean grey-level intensity, entropy and uniformity.

Results

Texture was not altered on unfiltered images whereas relative texture analysis following image filtration identified differences in fine to medium texture ratios in apparently disease-free areas of the liver in patients with hepatic metastases as compared to patients with no tumour (entropy, p = 0.0257) and patients with extra-hepatic disease (uniformity, p = 0.0143).

Conclusions

Relative texture analysis of unenhanced hepatic CT can reveal changes in apparently disease-free areas of the liver that have previously required more complex perfusion measurements for detection.  相似文献

Objective

To compare on-site and blinded off-site reading of baseline ultrasound (US) and contrast enhanced ultrasound (CEUS) for classification and characterisation of focal liver lesions.

Materials and methods

99 patients (57 women and 42 men, age range 18–89 years, mean age: 59 years) with 53 malignant and 46 benign liver lesions were studied with unenhanced US followed by contrast enhanced US after injection of 2.4 ml SonoVue® (Bracco, Milano, Italy). Image interpretation was performed on-site with clinical information available by consensus of two readers and off-site by two independent blinded readers at two different centers. Comparison of pre and post contrast scans and of the different readers was performed. Reference examinations were histology, intraoperative US, MRI or CT.

Results

Sensitivity for malignancy improved from 81/89/66% (on-site/off-site reader 1/2) before to 100/96/96% post contrast administration (p < 0.05, except for reader 1). Specificity improved from 48/48/54% on baseline US to 89/80/76% on CEUS (p < 0.05). Accuracy for specific lesion diagnosis was 62/59/50% pre and 90/77/72% post contrast (p < 0.05). Classification and characterisation post contrast were mildly inferior for off-site reading. Agreement between on-site and off-site readers of unenhanced scans was fair (κ = 0.29–0.39) while it was good for CEUS (κ = 0.63–0.79).

Conclusions

CEUS improves classification and characterisation of focal liver lesions and interobserver agreement compared to conventional US. Classification and characterisation post contrast were mildly but statistically significantly better for on-site than for off-site reading.  相似文献

Objective

To compare the incidence of pulmonary embolism (PE) and additional pathologic findings (APF) detected by computed tomography pulmonary angiography (CTPA) according to different age-groups.

Materials and methods

1353 consecutive CTPA cases for suspected PE were retrospectively reviewed. Patients were divided into seven age groups: ≤29, 30–39, 40–49, 50–59, 60–69, 70–79 and ≥80 years. Differences between the groups were tested using Fisher's exact or chi-square test. A p-value < 0.0024 indicated statistical significance when Bonferroni correction was used.

Results

Incidence rates of PE ranged from 11.4% to 25.4% in different age groups. The three main APF were pleural effusion, pneumonia and pulmonary nodules. No significant difference was found between the incidences of PE in different age groups. Furthermore, APF in different age groups revealed no significant differences (all p-values > 0.0024).

Conclusion

Purpose

To assess thin-section chest CT findings in systemic lupus erythematosus (SLE) with antiphospholipid syndrome (APS), in comparison with SLE without APS.

Materials and methods

We retrospectively reviewed the medical records and thin-section CT findings of 17 consecutive patients with an established diagnosis of SLE with APS, comparing with 37 consecutive SLE patients without APS, between 2004 and 2008, and patients who had other autoimmune disease, such as Sjögren syndrome, were excluded. No significant differences were seen between the two groups in age, gender, smoking habits, or history of steroid pulse and biological therapy. CT images of 2 mm thickness obtained with a 16- or 64-detector row CT were retrospectively evaluated by two radiologists in consensus on ultra high-resolution gray-scale monitors.

Results

The frequency of thin-section CT abnormalities was higher in SLE with APS group (82%) than in SLE without APS group (43%). Ground-glass opacity (59%), architectural distortion (47%), reticulation (41%), enlarged peripheral pulmonary artery (29%), and mosaic attenuation (29%) were significantly more common in the SLE with APS group than in the SLE without APS group (Fisher's exact test, p < 0.01).

Conclusion

SLE patients with APS have increased prevalence of thin-section chest CT abnormalities than those without APS.  相似文献

Objectives

To evaluate intraobserver and interobserver agreement of manually encompassed lung lesions for perfusion measurements using volume-perfusion computed tomography (VPCT).

Materials and methods

Institutional review board approval and informed consent were obtained. HIPAA guidelines were followed. A 65-s dynamic study was acquired with scan parameters 80 kV, 60 mA s (80 mA s for patients ≥70 kg), 128 × 0.6 mm collimation. Blood flow (BF), blood volume (BV) and Ktrans parameters were determined by syngo volume perfusion CT body with 88 lesions analyzed retrospectively.

Results

Within-subject coefficients of variation for intraobserver agreement (range 6.59–12.82%) were superior to those for interobserver agreement (range 21.75–38.30%). Size-dependent analysis revealed lower agreements for lesions <4 cm as compared to larger lesions. Additionally, agreements of the upper, middle and lower lung zones were different.

Conclusions

Intraobserver agreement was substantial for VPCT lung cancer perfusion measurements encouraging the use for tumor characterization and therapy response monitoring. Interobserver agreement is limited and unexperienced readers should be trained before using this new method.  相似文献

Objective

To determine if sputum colonization with Aspergillus species in patients with cystic fibrosis (PWCF) correlates with radiological abnormalities and/or a reduction in pulmonary function (FEV1).

Methods

We prospectively evaluated 32 PWCF utilizing high resolution computed tomography (HRCT) of the thorax and pulmonary function testing (PFT). The cohort was assessed as two groups: Aspergillus positive (n = 16) and Aspergillus negative (n = 16) based on sputum culture for Aspergillus species. A modified Bhalla scoring system was applied to each HRCT scan by two blinded radiologists.

Results

Aspergillus positive patients had more severe and significant bronchiectasis compared to those Aspergillus negative (p < 0.05). This was most marked in the right upper and lower lobes (RUL, RLL). Total Bhalla score was clinically significant in both groups and approached statistical significance between groups (p = 0.063). No difference in pulmonary function between the groups was detected.

Conclusion

PWCF colonized by Aspergillus species have greater radiological abnormalities undetectable by PFTs. Early radiological evaluation of Aspergillus colonized PWCF is therefore warranted.  相似文献
