目的 探讨小金片联合醋酸泼尼松片治疗浆细胞性乳腺炎的临床疗效。方法 选取2020年2月-2023年2月在皖南医学院第二附属医院就诊的80例浆细胞性乳腺炎患者,依据随机数字表法将所有患者分为对照组和治疗组,每组各40例。对照组口服醋酸泼尼松片,30 mg/次,1次/d,连续治疗7 d,然后剂量改为20 mg/次,1次/d,再连续治疗7 d。治疗组在对照组基础上口服小金片,3片/次,2次/d,连续治疗14 d。观察两组的临床疗效,比较两组的乳房红肿、肿块、乳头溢液、疼痛消失时间,比较病灶最大直径、弹性应变率比值,以及血清中细胞间黏附分子-1(ICAM-1)、Toll样受体4(TLR4)、超敏C反应蛋白(hs-CRP)的水平。结果 治疗后,与对照组总有效率67.50%相比,治疗组患者的总有效率87.50%更高,差异有统计学意义(P<0.05)。治疗后,治疗组患者红肿、肿块、乳头溢液、疼痛消失时间均短于对照组,差异有统计学意义(P<0.05)。治疗后,两组的病灶最大直径、弹性应变率比值低于治疗前(P<0.05);治疗后治疗组的病灶最大直径、弹性应变率比值低于对照组,差异有统计学意义(P<0.05)。治疗后,两组的血清ICAM-1、TLR4、hs-CRP水平低于治疗前(P<0.05),且治疗组血清ICAM-1、TLR4、hs-CRP水平较对照组更低(P<0.05)。结论 小金片联合醋酸泼尼松片治疗浆细胞性乳腺炎的疗效确切,能显著改善临床症状,降低炎症反应,药物安全性良好。  相似文献

目的 观察中风回春片联合硫酸氢氯吡格雷片治疗急性脑梗死恢复期的临床疗效。方法 选取2020年3月—2021年8月南阳市第二人民医院收治的120例急性脑梗死恢复期患者,采用随机数字表法将所有患者分为对照组和治疗组,每组各60例。对照组患者口服硫酸氢氯吡格雷片,75mg/次,1次/d。治疗组患者在对照组基础上口服中风回春片,5片/次,3次/d。两组均连续治疗4周。观察两组临床疗效,比较两组的Barthel指数(BI)评分、美国国立卫生研究院卒中量表(NIHSS)评分、血清炎性因子、神经营养指标、神经损伤指标。结果 治疗后,治疗组的总有效率(91.67%)明显高于对照组的总有效率(75.00%),组间比较差异有统计学意义(P<0.05)。治疗后,两组NIHSS评分下降,BI评分升高(P<0.05),且治疗组BI评分高于对照组,NIHSS评分低于对照组(P<0.05)。治疗后,两组血清白细胞介素-17(IL-17)、白细胞介素-1β(IL-1β)、超敏C反应蛋白(hs-CRP)水平均下降,转化生长因子-β(TGF-β)水平均升高(P<0.05),且治疗组血清IL-17、IL-1β、hs-CRP水平低于对照组,TGF-β水平高于对照组(P<0.05)。治疗后,两组血清脑源性神经营养因子(BDNF)、胰岛素样生长因子-1(IGF-1)、碱性成纤维细胞生长因子(bFGF)水平均升高(P<0.05),且治疗组血清BDNF、IGF-1、bFGF水平高于对照组(P<0.05)。治疗后,两组血清血清淀粉样蛋白A(SAA)、心型脂肪酸结合蛋白(H-FABP)、神经损伤指标神经肽Y(NPY)水平均下降(P<0.05),且治疗组血清SAA、H-FABP、NPY水平低于对照组(P<0.05)。结论 中风回春片联合硫酸氢氯吡格雷片治疗急性脑梗死恢复期的疗效显著,可提高患者的生活自理能力,促进其神经功能恢复,调节血清炎性因子、神经营养指标、神经损伤指标水平。  相似文献

目的 分析活血解毒丸联合阿莫西林分散片对急性乳腺炎患者淋巴细胞、炎症因子水平及疗效影响。方法 回顾性选取2014年1月—2017年2月河南中医药大学第一附属医院收治的急性乳腺炎患者298例,根据治疗方法不同分为对照组和观察组。对照组患者采用阿莫西林分散片治疗,观察组患者采用活血解毒丸联合阿莫西林分散片治疗。观察两组患者治疗后主要症状积分情况、症状消失时间、复发情况及淋巴细胞、炎症因子水平变化情况。结果 两组患者治疗后乳房胀痛、局部皮肤灼热、乳房结块及恶寒发热等主要症状积分均较治疗前改善,同组治疗前后比较差异有统计学意义(P<0.05);观察组患者治疗后主要症状积分均优于对照组,差异有统计学意义(P<0.05)。观察组患者退热时间、红肿消退时间、肿痛消失时间及通乳时间均较对照组短,差异有统计学意义(P<0.05)。两组患者治疗后淋巴细胞、CRP及IL-6水平均较治疗前下降,同组治疗前后比较差异有统计学意义(P<0.05);观察组患者治疗后淋巴细胞、CRP及IL-6水平均低于对照组,差异有统计学意义(P<0.05)。观察组患者1年内乳房肿块、乳房疼痛及乳房溢液的总复发率低于对照组,差异有统计学意义(P<0.05)。结论 活血解毒丸联合阿莫西林分散片治疗急性乳腺炎可快速改善患者症状,降低患者淋巴细胞、CRP、IL-6水平并减少复发。  相似文献

目的 探讨紫丹活血片联合硝苯地平治疗血管痉挛性心绞痛的临床疗效。方法 选择2019年6月—2021年6月开封市中心医院收治的180例血管痉挛性心绞痛患者,随机分为对照组和治疗组,每组各90例。对照组口服硝苯地平片,10 mg/次,3次/d。治疗组在对照组的基础上口服紫丹活血片,100 mg/次,3次/d。两组连续治疗15 d。观察两组患者临床疗效,比较治疗前后两组患者症状缓解时间,心绞痛发作次数和持续时间,血清因子白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、基质金属蛋白酶-9(MMP-9)和C反应蛋白(CRP)水平,及不良反应情况。结果 治疗后,治疗组临床有效率为98.89%,明显高于对照组的87.78%(P<0.05)。治疗后,治疗组症状缓解时间均明显早于对照组(P<0.05)。治疗后,两组心绞痛发作次数、持续时间均明显降低(P<0.05),且治疗组明显低于对照组(P<0.05)。治疗后,两组血清因子IL-6、TNF-α和CRP水平明显下降,而MMP-9水平明显升高(P<0.05),且治疗后治疗组血清因子水平明显好于对照组(P<0.05)。治疗组不良反应发生率明显低于对照组(5.56%vs 12.22%,P<0.05)。结论 紫丹活血片与硝苯地平联合治疗血管痉挛性心绞痛,明显提高临床疗效,症状得以有效改善,对心绞痛发作频次及血管炎性反应都有效降低。  相似文献

目的 探讨鱼腥草滴眼液联合夫西地酸滴眼液治疗急性细菌性结膜炎的临床效果。方法 选取2019年1月—2020年12月河南科技大学第一附属医院收治的160例急性细菌性结膜炎患者,随机分成对照组和治疗组,每组各80例。对照组患眼眼睑内滴入夫西地酸滴眼液,1滴/次,2次/d。在对照组基础上,治疗组给予鱼腥草滴眼液,1滴/次,6次/d。两组均连续治疗7 d。观察两组患者临床疗效,比较治疗前后两组患者典型表现缓解时间,Schirmer I试验(SIt)值和泪膜破裂时间(BUT),及血清肿瘤坏死因子-α(TNF-α)、白细胞介素-18(IL-18)和超敏C反应蛋白(hs-CRP)水平。结果 治疗后,治疗组临床有效率为97.50%,较对照组的88.75%显著提高(P<0.05)。与对照组相比,治疗组灼热感、异物感、结膜分泌物和结膜充血的缓解时间均显著缩短(P<0.05)。治疗后,治疗组SIt值显著增加,BUT显著延长(P<0.05);而对照组SIt值显著减小,BUT显著缩短(P<0.05)。治疗后,治疗组SIt值和BUT均显著优于对照组(P<0.05)。治疗后,两组患者血清TNF-α、IL-18和hs-CRP浓度较治疗前均显著下降(P<0.05),且治疗组较对照组更低(P<0.05)。结论 鱼腥草滴眼液联合夫西地酸滴眼液治疗急性细菌性结膜炎的整体疗效确切,可安全有效,且迅速地改善患者眼部表现。  相似文献

目的 探讨消糖灵胶囊联合利拉鲁肽治疗肥胖2型糖尿病的临床疗效。方法 选择2019年12月—2021年6月在河北中石油中心医院治疗的120例肥胖2型糖尿病患者,随机分为对照组和治疗组,每组各60例。对照组早餐前皮下注射利拉鲁肽注射液,0.6 mg/次,1次/d。治疗组在对照组的基础上口服消糖灵胶囊,3片/次,2次/d。两组患者连续用药15周。观察两组患者临床疗效,比较治疗前后两组患者症状缓解时间,血糖指标空腹血糖(FPG)、餐后2 h血糖(2 h PBG)、糖化血红蛋白(HbA1c)和空腹血清胰岛素(FINS)水平,血清因子肿瘤坏死因子-α(TNF-α)、可溶性血管细胞黏附因子-1(Svcam-1)、白细胞介素-6(IL-6)和超敏C反应蛋白(hs-CRP)水平,及不良反应。结果 治疗后,治疗组总有效率为98.33%,明显高于对照组的83.33%(P<0.05)。治疗后,治疗组多饮、多食、多尿、口干缓解时间均明显早于对照组(P<0.05)。治疗后,两组患者FPG、2 h FPG、HbA1c、FINS指标和血清因子TNF-α、Svcam-1、IL-6、hs-CRP水平均明显下降(P<0.05),且治疗后治疗组这些指标明显低于对照组(P<0.05)。治疗期间,治疗组不良反应发生率为6.67%,明显低于对照组的13.33%(P<0.05)。结论 消糖灵胶囊联合利拉鲁肽治疗肥胖2型糖尿病效果确切,症状缓解明显,并能有效控制患者血糖,降低机体炎症反应。  相似文献

目的 探讨奥美沙坦酯与氨氯地平联合氢氯噻嗪治疗高血压的疗效对比。方法 选取2017年2月—2019年2月新疆喀什地区第二人民医院治疗的100例高血压患者为研究对象,根据随机数字表法将患者分为对照组和观察组,每组各50例。对照组患者口服苯磺酸氨氯地平片,5 mg/次,1次/d;氢氯噻嗪片,25 mg/次,1次/d。观察组患者口服奥美沙坦酯片,20 mg/次,1次/d。氢氯噻嗪片用法同对照组。两组均连续治疗2个月。观察两组患者的临床疗效,同时比较两组患者治疗前后的24 h动态血压情况和血清因子水平。结果 治疗后,观察组患者的降压总有效率为92.00%,显著高于对照组的74.00%(P<0.05)。治疗后,两组患者24 h动态血压均明显降低(P<0.05),且观察组显著低于对照组(P<0.05)。治疗后,两组患者同型半胱氨酸(Hcy)、超敏C反应蛋白(hs-CRP)水平均明显降低(P<0.05),且观察组各血清因子水平均显著低于对照组(P<0.05)。结论 奥美沙坦酯联合氢氯噻嗪可有效控制血压水平,降低血清因子水平,对促进高血压患者病情康复具有积极意义。  相似文献

目的 探讨猴耳环消炎片联合法莫替丁片治疗急性胃肠炎的临床疗效。方法 选择2019年4月—2020年4月在郑州大学附属郑州中心医院治疗的急性肠胃炎患者84例,采用双色球随机分组法分成对照组与治疗组,各42例。对照组口服法莫替丁片,20 mg/次,2次/d;治疗组在对照组基础上口服猴耳环消炎片,0.96 g/次,3次/d。两组治疗7 d。比较两组临床疗效,观察两组临床症状改善时间和血清学指标。结果 经治疗,对照组患者有效率为80.95%,显著低于治疗组(97.62%,P<0.05)。经治疗,治疗组患者在腹痛、腹泻、呕吐等症状改善时间上均短于对照组(P<0.05)。经治疗,两组血清超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、降钙素原(PCT)、肿瘤坏死因子-α(TNF-α)水平均显著下降(P<0.05),且治疗后治疗组血清学指标低于对照组(P<0.05)。结论 急性胃肠炎患者给予猴耳环消炎片和法莫替丁片共同治疗不仅促进患者临床症状改善,还有利于机体炎症反应降低,有着良好的临床应用价值。  相似文献

目的 探究吗替麦考酚酯分散片联合醋酸泼尼松片治疗儿童肾病综合征的效果。方法 选择2016年1月—2018年1月于常州市儿童医院接受治疗的50例肾病综合征患儿作为研究对象,采用奇偶数法将患儿均分为两组,每组各25例。对照组接受环孢素软胶囊联合醋酸泼尼松片治疗,实验组接受替麦考酚酯分散片联合醋酸泼尼松片治疗,两组均持续治疗6个月。对比两组临床治疗总有效率、治疗中不良反应发生率及治疗前后IgA、IgG、IgM、蛋白排泄率(UAER)、血浆白蛋白(Alb)、尿素氮(BUN)、血肌酐(Scr)等指标变化。结果 治疗后,实验组治疗总有效率显著高于对照组(P<0.05)。治疗后,两组IgA、IgG、IgM水平均高于治疗前,且实验组高于对照组(P<0.05),但对照组治疗后IgM水平与治疗前差异无统计学意义(P>0.05)。治疗后,两组UAER、BUN及Scr水平均显著降低,而Alb显著升高,且实验组改善更显著(P<0.05)。两组治疗期间药物不良反应发生率无显著差异(P>0.05)。结论 吗替麦考酚酯分散片联合醋酸泼尼松片治疗儿童肾病综合征的效果良好,能够显著改善患儿免疫功能和肾功能,提高临床疗效,且治疗安全性较高。  相似文献

目的 探讨消糜栓与氟康唑片联合治疗念珠菌性阴道炎的临床疗效。方法 选择2021年2月—2021年8月在濮阳市油田总医院治疗的97例念珠菌性阴道炎患者,随机分为对照组(49组)和治疗组(48组)。对照组患者口服氟康唑片,150 mg/次,1次/d。在对照组的基础上,治疗组阴道给予消糜栓,1粒/次,1次/d。两组用药7 d观察治疗效果。观察两组患者临床疗效,比较治疗前后两组患者症状改善时间,血清白细胞介素-6(IL-6)、γ干扰素(IFN-γ)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)水平,及不良反应情况。结果 治疗后,治疗组临床有效率为97.91%,明显高于对照组的81.63%(P<0.05)。经治疗,治疗组症状改善时间均明显早于对照组(P<0.05)。治疗后,两组患者血清IL-6、IFN-γ、TNF-α、CRP水平均明显降低(P<0.05);且治疗组IL-6、IFN-γ、TNF-α、CRP水平均明显低于对照组(P<0.05)。治疗组不良反应发生率为6.25%,明显低于对照组的14.29%(P<0.05)。结论 对于念珠菌性阴道炎治疗,采用氟康唑片与消糜栓治疗效果确切,并可以调节血清炎性因子水平,能有效降低复发率,且安全有效。  相似文献

We report herein the condensation of 4,7-dichloroquinoline (1) with tryptamine (2) and D-tryptophan methyl ester (3) . Hydrolysis of the methyl ester adduct (5) yielded the free acid (6) . The compounds were evaluated in vitro for activity against four different species of Leishmania promastigote forms and for cytotoxic activity against Kb and Vero cells. Compound (5) showed good activity against the Leishmania species tested, while all three compounds displayed moderate activity in both Kb and Vero cells.  相似文献

Clinical and in vitro investigations were carried out to test the efficacy of gut lavage, hemodialysis, and hemoperfusion in the treatment of poisoning with paraquat or diquat. In a patient suffering from diquat intoxication 130 times more diquat was removed by gut lavage 30 h after ingestion than was removed by complete aspiration of the gastric contents.Determination of in vitro clearances for paraquat and diquat by hemodialysis showed that, at serum concentrations of 1–2 ppm, such as are frequently encountered in poisoning in man, toxicologically relevant quantities of herbicide cannot be removed from the body. At a concentration of 20 ppm, on the other hand, hemodialysis proved to be effective, the clearance being 70 ml/min at a blood flow rate of 100 ml/min. The efficacy of hemoperfusion with coated activated charcoal was on the whole better. Especially at concentrations around 1–2 ppm, the clearance values for hemoperfusion were some 5–7 times higher than those for hemodialysis.In a patient suffering from paraquat poisoning, both hemodialysis as well as hemoperfusion were carried out. The in vitro results could be confirmed: At serum concentrations of paraquat less than 1 ppm no clearance could be obtained by hemodialysis while by hemoperfusion with activated charcoal quite high clearance values were measured and the serum level dropped down to zero.
Zusammenfassung Klinische Untersuchungen und Laboratoriumsversuche wurden durchgeführt, um die Wirksamkeit von Darmspülung, Hämodialyse und Hämoperfusion bei Paraquat- und Deiquat-Vergiftungen zu prüfen.Bei einem Patienten wurde 30 Std nach Deiquat-Aufnahme durch Darmspülung 130mal mehr Deiquat entfernt als durch vollständige Aspiration des Mageninhaltes. In vitro-Versuche ergaben, daß bei Blutserumkonzentrationen von 1–2 ppm, die bei Vergiftungen oft gemessen werden, durch Hämodialyse keine toxikologisch relevanten Paraquat- oder Deiquat-Mengen entfernt werden können. Dagegen erwies sich die Hämodialyse bei 20 ppm und einer Blutumlaufgeschwindigkeit von 100 ml/min mit einer Clearance von 70 ml/min als wirksam. Die Hämoperfusion mit beschicheter Aktivkohle war in diesen Versuchen aber eindeutig überlegen, denn insbesondere bei Konzentrationen um 1–2 ppm waren die Clearance-Werte 5–7mal höher als bei der Hämodialyse.Die in vitro-Ergebnisse wurden bei einem Patienten mit einer Paraquat-Vergiftung bestätigt: Bei Konzentrationen unter 1 ppm war die Hämodialyse wirkungslos, während durch Hämoperfusion relativ hohe Clearance-Werte erreicht wurden, so daß der Serumspiegel rasch unter die Nachweisgrenze abfiel.

The lung offers a rich opportunity for development of therapeutic strategies focused on isozymes of protein kinase C (PKCs). PKCs are important in many cellular responses in the lung, and existing therapies for pulmonary disorders are inadequate. The lung poses unique challenges as it interfaces with air and blood, contains a pulmonary and systemic circulation, and consists of many cell types. Key structures are bronchial and pulmonary vessels, branching airways, and distal air sacs defined by alveolar walls containing capillaries and interstitial space. The cellular composition of each vessel, airway, and alveolar wall is heterogeneous. Injurious environmental stimuli signal through PKCs and cause a variety of disorders. Edema formation and pulmonary hypertension (PHTN) result from derangements in endothelial, smooth muscle (SM), and/or adventitial fibroblast cell phenotype. Asthma, chronic obstructive pulmonary disease (COPD), and lung cancer are characterized by distinctive pathological changes in airway epithelial, SM, and mucous-generating cells. Acute and chronic pneumonitis and fibrosis occur in the alveolar space and interstitium with type 2 pneumocytes and interstitial fibroblasts/myofibroblasts playing a prominent role. At each site, inflammatory, immune, and vascular progenitor cells contribute to the injury and repair process. Many strategies have been used to investigate PKCs in lung injury. Isolated organ preparations and whole animal studies are powerful approaches especially when genetically engineered mice are used. More analysis of PKC isozymes in normal and diseased human lung tissue and cells is needed to complement this work. Since opposing or counter-regulatory effects of selected PKCs in the same cell or tissue have been found, it may be desirable to target more than one PKC isozyme and potentially in different directions. Because multiple signaling pathways contribute to the key cellular responses important in lung biology, therapeutic strategies targeting PKCs may be more effective if combined with inhibitors of other pathways for additive or synergistic effect. Mechanisms that regulate PKC activity, including phosphorylation and interaction with isozyme-specific binding proteins, are also potential therapeutic targets. Key isotypes of PKC involved in lung pathophysiology are summarized and current and evolving therapeutic approaches to target them are identified.  相似文献

This study explored gender-related symptoms and correlates of alcohol dependence in a crosssectional study of 150 men and 150 women with a lifetime diagnosis of alcohol use disorders (AUD). Participants were recruited in equal numbers from treatment settings, correctional centres and the general community. Standardized measures were used to determine participants' use of substances, history of psychiatric disorders and psychosocial stress, their sensation seeking and family history of substance use and mental health disorders. Multivariate analyses were used to detect patterns of variables associated with gender and the lifetime severity of AUD. Men had a longer history of severe AUD than women. Women had similar levels of alcohol dependence and medical and psychological sequelae as men, despite 6 fewer years of AUD. More women than men had a history of severe psychosocial stress, severe dependence on other substances and antecedent mental health problems, especially mood and anxiety disorders. There were differences in family history of alcohol-related problems approximating same-gender aggregation. The severity of a lifetime AUD was predicted by its earlier age at onset and the occurrence of other disorders, especially anxiety, among both men and women. The limitations in the generalizability of these findings due to sample idiosyncrasies are discussed.  相似文献

Abstract

The uptake of metals from food and water sources by insects is thought to be additive. For a given metal, the proportions taken up from water and food will depend both on the bioavailable concentration of the metal associated with each source and the mechanism and rate by which the metal enters the insect. Attempts to correlate insect trace metal concentrations with the trophic level of insects should be made with a knowledge of the feeding relationships of the individual taxa concerned. Pathways for the uptake of essential metals, such as copper and zinc, exist at the cellular level, and other nonessential metals, such as cadmium, also appear to enter via these routes. Within cells, trace metals can be bound to proteins or stored in granules. The internal distribution of metals among body tissues is very heterogeneous, and distribution patterns tend to be both metal and taxon specific. Trace metals associated with insects can be both bound on the surface of their chitinous exoskeleton and incorporated into body tissues. The quantities of trace meals accumulated by an individual reflect the net balance between the rate of metal influx from both dissolved and particulate sources and the rate of metal efflux from the organism. The toxicity of metals has been demonstrated at all levels of biological organization: cell, tissue, individual, population, and community. Much of the literature pertaining to the toxic effects of metals on aquatic insects is based on laboratory observations and, as such, it is difficult to extrapolate the data to insects in nature. The few experimental studies in nature suggest that trace metal contaminants can affect both the distribution and the abundance of aquatic insects. Insects have a largely unexploited potential as biomonitors of metal contamination in nature. A better understanding of the physico-chemical and biological mechanisms mediating trace metal bioavailability and exchange will facilitate the development of general predictive models relating trace metal concentrations in insects to those in their environment. Such models will facilitate the use of insects as contaminant biomonitors.  相似文献
