How to Connect the Gap between Clinical Trials and Clinical Practice

Clinical research methods have been rapidly developing, and the design of clinical trials including traditional Chinese medicine is advancing. To a certain extent, all of these ensure that the results of clinical research are objective and scientific, but whether these results and the resulting guidelines or consensus have much practical significance on clinical practice is still controversial. The authors engage in both clinical practice and clinical research; they strongly feel that it is necessary to discuss the relationship between clinical trials and clinical practice. This essay discusses this relationship in four parts.     

Review on emergency medical response against terrorist attack

Terrorism is a global issue and a constant international threat. As a result, anti-terrorism and emergency response strategies are tasks of critical importance that have a direct impact on the national security of every country in the world. This paper reviews new characteristics of international anti-terrorism measures and offers an in-depth reflection on emergency medical response countermeasures; additionally, this paper presents the goals of related research, which include: 1) to present a model of a highly efficient medical response command; 2) to introduce the pre-planning phases of the emergency medical response; 3) to establish a response system capable of handling various types of terror attacks; 4) to promote anti-terrorism awareness to the general public and emphasize its prevention; and 5) to continue basic investigations into emergency medical responses for various types of terrorist attacks(for example, the classifications and characteristics of new injuries, pathophysiology, prevention and treatment of the resultant stress disorders, improved high-efficiency medical response measures and equipment, etc.).     

Enlightenment of Prevention Experience of Scientific Misconduct in Extraterritorial Medical Field to China

Scientific misconduct of researchers causes great waste of academic resources and academic life, which is not only related to the vigorous development of a country’s scientific undertaking, but also directly affects the reputation of the country in the field of scientific research. Based on the analysis of the causes of scientific misconduct of medical researchers in China, this paper comprehensively summarized a series of mature experiences of extraterritorial medical field in preventing scientific misconduct, and put forward corresponding and reasonable countermeasures and suggestions for China’s scientific research governance from three aspects: Establishing an effective governance structure based on the normative framework, clarifying the definition standards of scientific research dishonesty in laws and regulations, and formulating good prevention procedures n a healthy academic environment, in order to promote the healthy development of scientific research in China’s medical field and further promote the innovation and progress of medical science and technology. © 2022, Editorial department of Chinese Medical Ethics. Nonpeptide somatostatin analogs： recent advances in its application and research

Along with its wide anatomical distribution, somatostatin （SST） acts on multiple targets via a family of 5 receptors to produce a broad spectrum of biological effects. Therefore, a variety of peptide analogs have been produced and are widely used in clinical treatment. However, because of their flaws in the structure of peptide, the clinical efficacy is limited. In this review, we summarize the structure, pharmacological effects and the potential clinical value of non-peptide SST analogs. We focus on the research and development of non-peptide SST analogs since 1998, and discuss the problems and potential prospects for non-peptide SST analogs. We believe that as more non-peptide somatostatin analogs are successfully developed, the extensive clinical application of SSTs will contribute a great deal to medical science.     

MEDICAL EDUCATION

Among the objects of our Association, to further medical know- ledge and to uphold the standards of medical education are the most pressing. How far we have succeeded may be gauged from the Report on the Council of Medical Education appearing elsewhere in this issue. It seems that having fought advance guard action against inertia, we are now in a position to take part in a general forward movement under the leadship of the Ministry of Education. Realizing tbe im- portance of medical education the Minister has recently created the National Commiasion on Medical Education which has now brought out a monthly publication¨医育 (Medical Culture)" under its aegis.     

Surgical therapy in vitiligo—A review

Background Despite modern advances in medical science and in the social attitude of people,vitiligo on exposed areas still is a stigmatizing condition leading to serious psychological problems in daily lives of the victim.Treatment of vitiligo is difficult and prolonged and despite the advancement made in the field of vitiligo surgery over the past four decades,cutaneous surgeons still struggle to select among various surgical modalities which have a promising outcome.The basic principle of all the available surgical techniques as well as their advantages and disadvantages have been reviewed.Objective The purpose of this review is to find out if we can select a proper method of surgical modalities available to treat a vitiliginous area depending on the size and distribution of the lesions in order to achieve the best possible cosmetic results.Methods This article is based on a review of various articles regarding vitiligo and its emerging surgical treatments.Conclusion Although the treatment of vitiligo keeps on changing with new developments available,discussing the present surgical modalities available for treating vitiligo along with their limitations and advantages provides with great medical knowledge about the disease and its treatment.     

A Study on the Classification and the‘Catching’of the‘Arrived Qi’in Acupuncture

This essay studies the phenomenon of‘obtaining of qi’in acupuncture.Combined with clinical practice,the authors think that it was a habitual misunderstanding in the past to equate‘needling sensation’to ‘obtaining of qi’in acupuncture practice.The two concepts are different and are related with each other.The authors have also classified the phenomenon of‘obtaining of qi’according to its depth in the skin layer. Disease-syndrome combination clinical study of psoriasis: Present status, advantages, and prospects

Psoriasis is an immune-abnormal,chronic,proliferative skin disease determined by polygenic inheritance and induced by a number of environmental factors.It causes worldwide concern because of its high-prevalence,harmful and incurable characteristics.Over the years,Chinese medicine(CM) treatment of psoriasis has accumulated a wealth of clinical experience.Disease-syndrome combination, which achieves more satisfactory clinical effect,is the basis to highlight the special CM advantages in treating psoriasis.In this paper,we review the advantages of treating psoriasis with the combination of disease and syndrome,analyze the prospects of research on treating psoriasis combining disease with syndrome.We also make a point that there are several key points for the clinical research of combination of disease and syndrome.It can be expected that carrying out clinical research on the combination of disease and syndrome will help improve the clinical efficacy of medical treatment of psoriasis,which will be the main direction of research in the future.     

Q方法在专家经验分类及理解中应用的可行性分析

The Q method, combining qualitative and quantitative methods, refers to the qualitative analysis of Q-sorts based on quantitative techniques. It is used to research individual subjective experience, analyzing consensus and divergence to identify and categorize subjects＇ viewpoints. The sorting process is completely performed by the subjects, independent of study researchers. The Q method in medical research has been applied in many fields, including nursing care, clinical studies, doctor and patient＇s perceptions, health evaluation and decision making. The authors used the Q method to research Chinese medicine （CM） group decision making, exploring its practical feasibility in this important field. Four primary domains are addressed： （1） integration of expert opinion; （2） expert classification; （3） ascertaining the entire viewpoint orientation of a certain type of expert; and （4） comparison of expert opinion using an additional perspective. The essence of the Q method caters to the CM thinking model and should be introduced into CM and explored more deeply.

The physician charter on medical professionalism: a Jewish ethical perspective

The physician charter on medical professionalism creates standards of ethical behaviour for physicians and has been endorsed by professional organisations worldwide. It is based on the cardinal principles of the primacy of patient welfare, patient autonomy, and social welfare. There has been little discussion in the bioethics community of the doctrine of the charter and none from a Jewish ethical perspective. In this essay the authors discuss the obligations of the charter from a Jewish ethical viewpoint and call on other cultures to develop their own unique perspectives on this important document.

Towards local participation in the creation of ethical research guidelines

Research ethics committees are entrusted with implementing guidelines to protect both scientists and human subjects of research from harm. These guidelines are often based on western contexts and may not resonate with the local moral traditions of the communities that they seek to protect. In this essay, we discuss how using principles of deliberative democracy with a "local derivation" approach may help in the drafting and implementation of ethical guidelines for research that better serve society.
