Objective To clarify mechanism of Roux-en-Y gastric bypass (RYGB) and gastric banding on diabetes induced by STZ injection. Methods 40 rats with STZ induced diabetes were randomly allocated into Roux-en-Y gastric bypass (RYGB) group (group RYGB, n=10), gastric banding group (group GB, n=10 ), diet control group (group F, n=10), control group (group C,n= 10). The fasting blood glucose, the fasting insulin IGF-1, the fasting Plasma leptin, the fasting plasma insulin level, the weight and the food-intake, the operation time, the death rate were measured and recored before and after operation on 1st , 2nd, 3 rd ,4th, 8th and 16 th week postoperatively. Results The fasting blood glucose of the group of gastric banding(GB) descended to (12.6±3.7) mmol/L, the fasting plasma insulin rose to (58.7±9.2) mIU/L, the fasting plasma leptin descended to (14.6±3.3) pg/ml, the weight was (212.6±15.1) g.There were significant differences between before and after operation on 16 th week(P<0.01). The fasting blood glucose of the group of Roux-en-Y (RYBG) descended to 8.8±4.9 mmol/L in the sixteenth week, the fasting insulin IGF-1 rose to (148.6±7.3) ng/L, the fasting plasma insulin rose to (14.1±3.5) pg/ml, the fasting plasma leptin descended to 14.1±3.5 pg/ml, the weight was (200±15.1) g. There were significant differences between before and after operation 16 th week (P<0.01). There were significant differences of the fasting plasma insulin and the the fasting plasma leptin between group F and group C during the 3 rd to 4th week after operation (P<0.05). Compared the weight of the group F and the group C on the third week of operation, there were significant differences (P<0.05), and there were no significant differences in other time. The fasting blood glucose of the group F and the group C had no sig-nificant differences between before and after operation.(P<0.05). Conclusions The fasting blood glucose and the fasting insulin level of the group F improve more than of the group GB at the same time. The plasma insulin and the plasma leptin of the two groups all work in glucose control. The diet control and the modification of the plasma insulin and the plasma leptin all play a major role in the gastric banding mechanism, and the IGF-1 may work in the descending the blood glucose after the operation of Roux-en-Y. In the operation time and die rate, the group of F surpass the group of GB.  相似文献

目的 通过有限元分析探讨有限接触型动力加压钢板(LC-DCP)不同螺钉固定数目及位置对胫骨中段骨折的生物力学影响.方法获取正常胫骨CT薄层扫描图像,利用三维重建软件Mimics10.01及有限元分析软件Ansys 10.0建立正常胫骨有限元模型,并在此基础上制成胫骨中段斜形无缺损骨折模型,以14孔LC-DCP固定,根据螺钉固定数目及位置不同分为5组:A组固定第1、2、3、12、13、14孔,B组固定第1、4、7、8、11、14孔,C组固定第1、3、6、9、12、14孔,D组固定第1、2、7、8、13、14孔,E组第1~14孔全部固定.各模型压缩载荷为1000 N,扭转力矩为5 N·m.结果 A组螺钉受力以第3、12孔为大.B组螺钉压缩时受力均以第1孔为最大,第4孔次之;扭转时以第7、8孔为大.C组螺钉压缩时受力均以第1孔为最大,第3孔次之;扭转时受力以第6、9孔为大.D、E组螺钉压缩时受力均以第1孔为最大,第2孔次之;扭转时受力以第7、8孔为大.结论应用LC-DCP固定治疗胫骨中段骨折时以6枚螺钉固定14孔长钢板为妥,且以固定第1、2、7、8、13、14孔为最佳.
Abstract:
Objectives To determine biomechanical performances of limited contact-dynamic compression plate (LC-DCP) fixation with screws in different number and position for treatment of fractures of tibial shaft. Methods CT scan was performed for a fresh sample of adult tibia to construct a 3D finite element model of normal tibia using Mimics 10. 01 and Ansys 10. 0. Finite element models of oblique fractures of middle tibia were made on the basis of the normal model to be all fixed with LC-DCP of 14 holes but with screws in 5 different patterns. Model A was fixed with screws in the 1st, 2nd, 3rd, 12th, 13th and 14th holes; model B with screws in the 1st, 4th, 7th, 8th, 11th and 14th holes; model C with screws in the 1st,3rd, 6th, 9th, 12th and 14th holes; model D with screws in the 1st, 2nd, 7th, 8th, 13th and 14th holes;and model E with screws in all 14 holes. Finite element software Ansys 10. 0 was used to establish 3-D finite element models of 5 fixation models for tibial shaft fracture. Five kinds of digital loadings were simulated in computer, with 1000 N compressive load and 5 N · m torque moment for all models. Results In model A, the highest stresses were on the 3rd and 12th holes. In model B, the highest compressive stress was on the 1st hole, next by the 4th hole; the highest torsion stresses were on the 7th and 8th holes. In model C, the highest compressive stress was on the 1st hole, next by the 3rd hole; the highest torsion stresses were on the 6th and 9th holes. In models D and E, the highest compressive stress was on the 1st hole, next by the 2nd hole; the highest torsion stresses were on the 7th and 8th holes. Conclusions In treatment of tibia shaft fractures with minimally invasive percutaneous plate osteosynthesis, fixation with a 14-hole LC-DCP and 6 screws is appropriate. The appropriate fixation holes are the 1st, 2nd, 7th, 8th, 13 th and 14th holes.  相似文献

自体骨髓干细胞移植治疗失代偿期肝硬化   总被引:2,自引:2,他引:0  
Objective To investigate the therapeutic effects of bone marrow stem cells for the treatment of decompensated liver cirrhosis. Methods The clinical data of 28 patients with decompensated liver cirrhosis who were admitted to the No. 105 Hospital of PLA from July 2009 to March 2010 were retrospectively analyzed. Bone mesenchymal stem cells (BMSCs) were separated and purified from the bone marrow cells of the patients in vitro,and then they were transplanted into the liver of the patients through proper hepatic artery. The clinical symptoms and physical signs of the patients were monitored, and the biochemical indexes of the liver were measured at the 2nd, 4th, 8th and 12th weeks after operation. All data were analyzed using the analysis of variance, LSD test or Dunnett's test. Results The appetite and physical strength of 26 patients were apparently improved one week after operation. The ascites of 23 patients were reduced at the end of the 4th week. At the end of the 12th week,the level of alanine aminotransferase was decreased from (99 ± 36)U/L to (48 ± 26 )U/L, aspartate aminotransferase was decreased from (86 ±36)U/L to (46 ±22)U/L, total bilirubin was decreased from (38 ± 16)μmol/L to (18 ± 13)μmol/L, albumin was increased from (29 ± 4)g/L to (35 ± 5 )g/L, and the prothrombin time was decreased from ( 19 ±4)s to ( 13±4) s. There were significant differences in the biochemical indexes of the liver after operation when compared to those before operation ( F =267. 35, 143. 52, 218. 74, 125. 57, 331.25, P <0.05). No fever, rash or allergic reaction was detected. Hematoma was detected in the incision site of the posterior superior iliac spine in one patient, and it disappeared one week later; one patient had discomfort in the liver area during the process of the BMSCs transplantation, and the discomfort disappeared at the end of the process; one patient died of hemorrhage of the upper gastrointestinal tract at the end of the 3rd month after operation. Conclusion The short term outcome of the autologous BMSCs transplantation for the treatment of decompensated liver cirrhosis is satisfactory, while its long term effect needs to be further investigated.  相似文献

目的 检测细胞内外ifnγ和il-4水平,评估慢性乙型肝炎(chb)和hbv携带患儿th1/th2应答状态.方法 收集2007年5月-2009年2月54例住院患儿,将其分为chb组(23例)和hbv携带组(31例),另外选取34名健康儿童作为对照.采用elisa法检测研究对象全血细胞外ifnγ和il-4,采用流式细胞术检测细胞内ifnγ和il-4.方差齐者行方差分析,不齐者行非参数kruskal-wallis检验分析.结果 elisa法检测发现,ifnγ、il-4水平和th1/th2比值在chb组、hbv携带组和健康对照组间差异无统计学意义(f=0.342、0.020和0.507,p值均>0.05).流式细胞术检测发现细胞内ifnγ水平在chb组、hbv携带组和健康对照组分别为(7.68±4.62)、(11.71±4.36)和(13.61±6.71)μg/ml,th1/th2比值分别为0.96±0.30、1.67±0.76和2.11±1.12,差异有统计学意义(f=0.255和0.140,p<0.05或<0.01);il-4水平在三组间的差异无统计学意义(f=0.425,p>0.05).结论 chb和hbv携带组患儿细胞内因子表达失衡,细胞免疫功能受到抑制.与elisa法相比,流式细胞技术能够更加真实、准确地反映th1和th2的应答状态. abstract: objective to evaluate extracellular and intracellular ifnγ and il-4 levels in assessing th1/th2 balance in children with chronic hepatitis b (chb) and asymptomatic carriers.methods fiftyfour hospitalized children including 23 chb patients and 31 asymptomatic carriers were collected from may 2007 to february 2009.thirty-four healthy children were served as control.serum ifnγ and il-4 levels were measured by enzyme-linked immunosorbent assay (elisa) and intracellular ifnγand il-4 levels were detected by flow cytometer.analysis of variance (for homogenous variance) and kruskal-wallis test (for non-homogenous variance) were performed.results the differences on extracellular ifnγ, il-4 and th1/th2 among chb, asymptomatic carriers and control groups were not statistically significant (f=0.342, 0.020 and 0.507, p > 0.05); while the intracellular ifnγlevels were (7.68 ± 4.62), (11.71 ±4.36) and (13.61 ±6.71) μg/ml, and th1/th2 ratios were 0.96 ±0.30, 1.67 ±0.76 and 2.11 ± 1.12in three groups respectively (f=0.255 and 0.140, p < 0.05 or < 0.01).the differences in intracellular il-4 levels among three groups were not significant (f=0.425, p > 0.05).conclusions cytokine balance is affected in chb children and asymptomatic carriers, and flow cytometry analysis is considered as a better method in evaluating the status of th1/th2 balance.  相似文献

Objective To evaluate the effect of Wnt/β-catenin signals on the fracture healing in transgenic mice. Methods Col2al-ICAT transgenic mice were generated by the gene targeting technology with the ICAT transgene specifically expressed in chondrocytes as a competitive inhibitor to block Wnt/ β-catenin signals. The 8-week-old Col2al-ICAT mice were used in the experimental group and the wild type (WT) littermates with the same age served as the control group. A transverse osteotomy was performed at the middle of the tibia and the fracture healing was evaluated on the 7th, 9th, 14th, 21th and 28th days re-spectively after fracture. Roentgenogyaphy and histology observations were performed to evaluate the fracture healing pattern and the histomorphometric analysis was used to quantitate the cartilage callus volume / total callus volume (CV/TV) or bony callus volume / total callus volume (BV/TV). Results X-ray exam-ination revealed that on the 21st day after fracture, callus appeared at the fracture gap to form a bony bridge in WT mice while a radiolucent zone was apparent in the fracture gap in the Col2al-ICAT transgenic mice. Histology observation revealed that compared with WT mice, the formation of cartilage callus and endochondral ossification were delayed in Col2al-ICAT transgenic mice. Histomorphometric analysis indicated that the peak value of CV/TV arrived later in Col2al-ICAT transgenie mice than in WT mice. The BV/TV in Col2al-ICAT transgenic mice was significantly less than that in WT mice on 14th and 21st days after fracture (P<0.05). Conclusion The Wnt/β-catenin signals cause delayed fracture healing in Col2al-ICAT transgenic mice by affecting the cartilage callus formation and endochondral ossification.  相似文献
