目的了解常见食物过敏原特应性斑贴试验(atopy patch test,APT)结果与特应性皮炎发病及临床表型的关系。方法对79例特应性皮炎患者进行常见食物过敏原斑贴试验,并与皮肤点刺试验和特异性IgE(sIgE)检测结果以及临床表型相关性进行分析。结果 5种食物过敏原中,牛奶APT阳性率最高(24.3%),小麦阳性率最低(16.0%),各种食物APT阳性率均高于相应的皮肤点刺试验(SPT)及sIgE阳性率。各种食物过敏原APT、SPT和sIgE 3种诊断试验结果一致性比较差(Kappa值均0.75)。儿童(≤14岁)患者各种食物过敏原APT阳性率均高于青少年成人(14岁),但是仅有牛奶(P0.01)和大豆(P0.05)APT阳性率的差异有统计学意义。鸡蛋白、牛奶和大豆APT阳性率与特应性皮炎病情严重度评分呈一致的升高趋势,但是只有鸡蛋白阳性率升高趋势有统计学意义(P0.05),小麦和海虾APT阳性率与病情严重度评分无相关性。结论在特应性皮炎患者特别是儿童患者中,食物过敏原特应性斑贴试验可作为辅助诊断食物过敏的方法之一。  相似文献

目的:分析儿童荨麻疹患者过敏原血清特异性IgG和IgE检测结果。方法:对63例儿童荨麻疹患者采用酶联免疫法检测食物过敏原特异性IgG,采用免疫印迹法检测食物及吸入性过敏原特异性IgE。结果: 食物过敏原IgG的总阳性率为76.19%,其中主要过敏原是鸡蛋、牛奶;食物过敏原IgE的总阳性率为41.27%,其中主要过敏原是鸡蛋、淡水鱼;食物过敏原IgG和IgE的检测结果不一致(P=0.000)。吸入性过敏原IgE的总阳性率为57.14%,其中主要过敏原是尘螨和蟑螂。不同年龄组患儿过敏原阳性检出率无明显差异。结论:针对儿童荨麻疹患者进行血清特异性IgG和IgE抗体进行联合检测,有助于明确过敏性皮肤病的过敏原,对临床过敏性疾病的预防和治疗具有重要意义。  相似文献

目的探讨血清中14种食物过敏原特异性IgG与皮肤过敏性疾病的关系。方法应用酶联免疫法检测128例皮肤过敏性疾病患者血清中食物特异性IgG水平。结果128例患者食物过敏原特异性IgG有1~7种不等,总阳性率为89.1%,2种以上升高者为69.3%。食物特异性lgG升高以鸡蛋(50.0%)和蟹(47.8%)最多见,其次为虾和大豆(二者均为27.3%)。根据实验结果调整患者饮食,症状在2月内有明显改善者为72.2%。结论测定食物过敏原特异性IgG在皮肤过敏性疾病的诊治中有重要意义。  相似文献

目的分析温州地区过敏性疾病患者的主要过敏原类型和分布特征,为临床过敏性疾病的诊治和预防提供可靠的实验依据。方法采用德国Mediwiss Analytic Gmb H生产的Allergy Screen过敏原定量检测系统检测402例过敏性疾病患者血清中18种过敏原特异性IgE抗体(sIgE)含量,采用Roche E601分析仪检测患者血清中总IgE含量。结果温州地区402例过敏性疾病患者总Ig E阳性率为57.46%,s Ig E阳性率为78.86%;吸入性过敏原以户尘螨为主,阳性率45.02%,食入性过敏原以牛奶为主,阳性率26.12%;吸入性组过敏原阳性率64.18%高于食入性组53.23%,2组率比较差异有统计学意义(χ~2=9.933,P=0.010);不同性别间过敏原检出率为男165例(83.33%),明显高于女152例(74.51%),检出率比较差异有统计学意义(χ~2=4.692,P=0.030);402例患者以多种过敏原呈阳性反应为主,其中21.39%的患者仅对单一过敏原有阳性反应,17.91%的患者对2种过敏原有阳性反应,而3种及以上过敏原呈阳性反应的占39.56%。结论温州地区过敏性疾病患者血清中吸入性过敏原以户尘螨为主,食入性过敏原以牛奶为主,大多数患者易被多种过敏原致敏。通过对其过敏原进行筛查可为疾病的预防和治疗提供重要理论依据。  相似文献

采用酶联免疫吸附法(ELISA)检测94例过敏性疾病患者及33例健康对照者血清14种日常食物过敏原特异性IgG抗体,以探讨过敏性疾病与食物不耐受的关系,结果显示94例过敏性疾病患者食物过敏原IgG抗体阳性率100%,较对照组明显增高,具有临床意义。通过调整饮食对患者治疗,成人组有效率68.1%,儿童组有效率78.6%,可使多数患者病情好转。  相似文献

目的了解食物和吸入过敏原在婴幼儿特应性皮炎中所占的比例以及主要的食物过敏原。方法对95例患儿进行婴幼儿过敏原筛查(phadiatop infant),并同85例健康儿童进行比较。同时对患儿血清进行吸入过敏原筛查(phadiatop)及多价食物过敏原筛查(fx5E),并从95例中随机抽取60例进行几种常见食物的特异性IgE(sIgE)检测。结果95例患儿组婴幼儿过敏原筛查阳性55例(57.89%),85例正常儿童中阳性16例(18.82%)两者比较差异有显著性意义(P<0.01)。患儿组多价食物过敏原阳性54例(56.8%),吸入过敏原阳性19例(20%),两者比较差异有显著性意义(P<0.01)。食物特异性IgE检测中血清浓度在3级以上的,鸡蛋白为23.3%,牛奶为11.7%,小麦为10%,蛋黄为6.7%,花生为6.7%,黄豆为1.67%。结论婴幼儿特应性皮炎患儿中过敏原检测阳性率高于正常儿童,其中食物IgE的阳性率比吸入IgE的阳性率更高,食物过敏原在婴幼儿特应性皮炎中占有重要地位。  相似文献

目的:探寻荨麻疹患儿常见的过敏原种类,为其治疗及预防提供依据.方法:采用Mediwiss 公司AllergyScreenAnalytic GmbH 过敏原检测系统,对257 例荨麻疹患儿进行血清特异性IgE(specific IgE,sIgE)和总IgE 检测,并在不同年龄组间进行比较.结果:257例荨麻疹患儿sIgE阳性142例(55.27%),总IgE阳性率69.65%.其中吸入性过敏原最常见的依次为户尘螨18.68%、霉菌组合17.51%、猫毛皮屑4.67%、屋尘4.28%和狗毛皮屑3.89%;食物性过敏原中,最常见的依次为牛奶28.79%、羊肉16.34%、牛肉7.00%、鸡蛋白5.45%和虾3.11%.比较<3 岁组、3~6 岁组和>6 岁组,户尘螨、猫毛皮屑、牛奶和羊肉sIgE阳性率的差异有统计学意义(P < 0.05).结论:尘螨和霉菌是荨麻疹患儿最主要的吸入性过敏原,牛奶、牛羊肉、鸡蛋白是荨麻疹患儿最常见的食物性过敏原.血清sIgE检测为荨麻疹患儿提示了相关过敏原,可以针对性地回避相关过敏原.  相似文献

目的:检测湿疹患儿常见的过敏原,寻找儿童湿疹可能的过敏促发因素,为其治疗及预防提供依据。方法:采用Mediwiss公司AllergyScreen Analytic GmbH过敏原检测系统,对872例湿疹患儿进行血清特异性IgE(specific IgE,sIgE)和总IgE检测,并在不同年龄组间进行比较。结果:872例湿疹患儿slgE阳性690例(79.13%),总IgE阳性709例(81.30%)。其中吸入性过敏原最常见的依次为户尘螨(34.60%)、点青霉(22.71%)、屋尘(16.28%)和狗毛皮屑(10.89%);食物性过敏原中,最常见的依次为牛奶(36.12%)、羊肉(22.94%)、鸡蛋白(20.07%)和牛肉(13.88%)。比较1岁组,1~3岁组和3岁组,户尘螨、屋尘、狗毛皮屑、点青霉、鸡蛋白、牛奶、羊肉和蟹的sIgE阳性率的差异有统计学意义(P0.05)。结论:户尘螨、点青霉和屋尘是湿疹患儿最主要的吸入性过敏原,牛奶、羊肉、鸡蛋白是湿疹患儿最常见的食物性过敏原。随年龄增大,吸入性过敏原逐步取代食物成为主要过敏原。血清sIgE检测为湿疹患儿提示了相关过敏原,可以针对性地回避相关过敏原。  相似文献

目的:明确急性荨麻疹患者食入性过敏原特点。方法:对2018年6月至2019年10月我科确诊为急性荨麻疹的患者采用 ELISA 酶联免疫捕获法定量检测食入性过敏原特异性IgE(specific IgE,sIgE),对鸡蛋、牛奶sIgE阳性的部分患者行食物经口负荷实验(oral food challenge,OFC)验证。结果:416例患者中161例sIgE定量检测阳性,其中鸡蛋和牛奶sIgE阳性例数最多,分别为56例和29例。鸡蛋sIgE浓度为0.35~0.70 IU/mL和0.7~2.0 IU/mL患者中OFC阳性率分别为2.8%(1/36)和81.8%(9/11)。牛奶sIgE为0.35~0.70 IU/mL和0.7~2.0 IU/mL患者中OFC阳性率分别为0%(0/16)和37.5%(3/8)。结论:急性荨麻疹患者常见食入性过敏原为鸡蛋、牛奶,患者sIgE浓度<0.7 IU/mL 时可不规避过敏原食物。  相似文献

目的应用特异性Ig E的检测方法,找出泉州地区过敏性疾病患者常见的过敏原及其分布情况,为临床治疗提供帮助。方法采用特异性Ig E方法对1 182例过敏性疾病患者进行过敏原检测。结果 1 182例患者总阳性率为60.24%,吸入组主要过敏原为尘螨、蟑螂,食物组主要过敏原为螃蟹、海洋鱼;不同性别总阳性率、吸入组阳性率及两者均阳性的率差异无统计学意义,食物组阳性率存在差异,男性比较高为49.03%(χ2=6.391,P0.05);(4~16)岁总阳性率、吸入组阳性率及两者均阳性率都比其他年龄组高,(0~3)岁食物组阳性率最高。结论通过筛查得出泉州地区过敏性疾病的主要变应原是尘螨、蟑螂、螃蟹、海洋鱼;不同性别食物组阳性率男性较女性高;不同年龄过敏原阳性率情况存在差异。  相似文献

Background

It is well known that atopic dermatitis (AD) is related to food hypersensitivity, although its prevalence varies among several studies according to age group, severity, country, survey time, and test method.

Objective

To examine the prevalence and status of food hypersensitivity among childhood AD patients in Korea.

Methods

A total of 95 patients were enrolled in the study. The history of food hypersensitivity was collected by interviews. The severity of AD was evaluated by eczema area and severity index (EASI). We took blood samples to measure serum total and food-specific immunoglobulin E (IgE) levels. Based on the histories and serum IgE levels, open oral food challenge (OFC) testing was performed to confirm food hypersensitivity.

Results

Forty-two (44.2%) of the 95 AD patients had histories of food hypersensitivity. They reported that the most common suspicious foods were egg (n=13, 13.7%), pork (n=9, 9.5%) and cow milk (n=8, 8.4%). The mean EASI score was 16.05±9.76. Thirty-nine (41.1%) of the 95 patients showed elevated serum food-specific IgE levels. The specific IgE levels were elevated for egg (n=17, 17.9%), milk (n=12, 12.6%), peanut (n=10, 10.5%) and wheat (n=8, 8.4%). Fifty-one (53.8%) of 95 patients underwent open OFC, and only 7 (13.7%) of these patients showed positive reactions.

Conclusion

The overall prevalence of food hypersensitivity in patients with childhood AD in Korea was 8.3% (7/84). The most common foods causing food hypersensitivity were egg and milk. Among the foods causing hypersensitivity, AD patients in Korea often underestimated peanut, while they overestimated pork.  相似文献

Background

Atopic dermatitis (AD) is characterized by itching and eczema-like skin lesions, and its symptoms alleviate with age. Recently, the prevalence of AD has increased among adolescents and adults. The increasing prevalence of AD seems to be related to westernized lifestyles and dietary patterns.

Objective

The aim of this study was to evaluate the dietary patterns and nutrient intake of patients with AD.

Methods

The study population consisted of 50 children with AD who visited the Department of Dermatology at Kangnam Sacred Heart Hospital, Seoul, Korea from May 2008 to May 2009. Physical condition and calorie intake were evaluated using the Eczema Area and Severity Index score and Food Record Questionnaire completed by the subjects, and the data were analyzed using the Nutritional Assessment Program Can-pro 3.0 (The Korean Nutrition Society, 2005) program to determine the gap between the actual ingestion and average requirements of 3 major nutrients (i.e. carbohydrates, proteins, and lipids), vitamins (i.e. A, B, C, and E), niacin, folic acid, calcium, iron, phosphorus, and zinc in all subjects.

Results

The intake rate of proteins was 18.02% (recommended dietary allowance [RDA], 7%~20%), of carbohydrates was 67.7% (RDA, 55%~70%), and of lipids was 14.24% (RDA, 15%~30%). Thirty-one subjects (62%) showed deficiency of folic acid, and 21 subjects (42%), of iron supplements.

Conclusion

Essential nutrient intake tends to be lower in AD patients than in healthy subjects, and this low intake is closely related to the severity of AD.  相似文献

BackgroundThe etiology of chronic urticaria (CU) remains unknown in most patients. Possible causes in some cases include food, but the role of allergy to food antigens in patients with CU remains controversial.ObjectiveThe aim of this study was to evaluate the association between food allergy and CU.MethodsKorean patients with CU were assessed for a previous history of food allergy that caused symptoms of CU. Blood samples were taken from 350 patients to measure food allergen-specific IgE. Based on history and laboratory results, open oral food challenge (OFC) tests were performed.ResultsOf 350 participants, 46 (13.1%) claimed to have experienced previous food hypersensitivity. Pork (n=16) was the main food mentioned, followed by beef (n=7), shrimp (n=6), and mackerel (n=6). We found that 73 participants (20.9%) had elevated levels of food-specific IgE, with pork (n=30), wheat (n=25), and beef (n=23) being the most common. However, when the open OFC tests were conducted in 102 participants with self-reported food hypersensitivity or raised levels of food-specific IgE, only four participants showed a positive reaction to pork (n=3) or crab (n=1).ConclusionAlthough some participants claimed to have a history of CU related to food intake, when an open OFC test was conducted, few of them had positive results. We therefore conclude that food allergy is an uncommon cause of chronic CU.  相似文献

目的 通过对银屑病患者食物不耐受情况的调查分析,研究银屑病是否存在食物不耐受的可能及特点。方法 采用ELISA法检测银屑病患者及健康对照血清中食物不耐受相关性IgG抗体。结果 194例银屑病患者发生食物不耐受现象者共136例(男82例,女54例),食物不耐受发生率70.12%。50例正常对照组发生食物不耐受现象共10例(男2例,女8例),食物不耐受发生率20.00%。两组比较差异有统计学意义(P〈0.01)。在日常食用的14种食物中,银屑病患者发生食物不耐受的阳性率依次为大豆(64.22%)、蟹(27.13%)、虾(23.35%)、牛奶(22.00%)、牛肉(20.59%)、蘑菇(6.62%)、鳕鱼(5.88%)、鸡蛋(5.88%)、玉米(3.52%)、猪肉(1.47%)、鸡肉(1.47%)、西红柿(1.47%)、大米(0.78%),小麦未见阳性病例。正常对照发生食物不耐受的阳性率依次为蟹(70.00%)、鸡蛋(40.00%)、虾(30.00%)、玉米(20.00%)、大豆(20.00%)、大米(20.00%)、蘑菇(10.00%)、牛奶(10.00%)、牛肉(10.00%)、鸡肉(10.00%),鳕鱼、猪肉、西红柿未见阳性病例。两组比较食物不耐受种类有明显不同。结论 银屑病患者可能存在食物不耐受情况,进行食物特异性IgG检测可能对银屑病患者科学饮食有一定的指导意义。  相似文献

BackgroundPsoriasis is a chronic inflammatory disease with systemic repercussions and an association with comorbidities such as metabolic syndrome, cardiovascular diseases, and obesity. Psoriasis patients have a higher prevalence of obesity compared to the general population. Diet is a relevant environmental factor, since malnutrition, inadequate body weight, and metabolic diseases, in addition to the direct health risk, impair the treatment of psoriasis.ObjectivesTo evaluate food intake patterns, anthropometric, and metabolic syndrome-related aspects in psoriasis patients.MethodsCross-sectional study through anthropometric assessment and food frequency questionnaire. Food frequency questionnaire items were evaluated by exploratory factor analysis and identified dietary patterns were analyzed by multivariate methods.ResultsThis study evaluated 94 patients, 57% female, with a mean age of 54.9 years; the prevalence of obesity was 48% and of metabolic syndrome, 50%. Factor analysis of the food frequency questionnaire identified two dietary patterns: Pattern 1 – predominance of processed foods; Pattern 2 – predominance of fresh foods. Multivariate analysis revealed that Patterns 1 and 2 showed inverse behaviors, and greater adherence to Pattern 2 was associated with females, eutrophic individuals, absence of lipid and blood pressure alterations, and lower waist-to-hip ratio and skin disease activity.Study limitationsMonocentric study conducted at a public institution, dependent on dietary memory.ConclusionTwo dietary patterns were identified in a Brazilian sample of psoriasis patients. The prevalence of obesity and metabolic syndrome were greater than in the adult Brazilian population. The fresh diet was associated with lower indicators of metabolic syndrome in this sample.  相似文献

大约1/3中到重度特应性皮炎患儿存在食物过敏,但食物过敏是否会诱发或加重特应性皮炎患儿的湿疹样皮损仍存在争议.食物过敏可以诱发特应性皮炎患儿的两种反应,非湿疹样反应和湿疹样反应.前者为IgE介导,诊断相对比较简单,多数情况下借助病史、皮肤点刺试验及特异性IgE抗体水平等即可明确诊断.后者为细胞介导,诊断必须借助口服食物激发试验.如果没有条件做食物激发试验时,也可以借助诊断性的饮食回避来明确可疑过敏食物.  相似文献

目的检测银屑病患者14种血清食物特异性IgG抗体水平。方法采用酶联免疫吸附法(ELISA)检测63例银屑病患者及60例健康对照者14种血清食物特异性IgG抗体水平。将银屑病组中58例IgG抗体检测阳性者,随机分成观察组和对照组(每组29例),观察组进行不耐受食物禁食/轮替调整6个月,然后再次检测两组IgG抗体水平。结果银屑病组食物不耐受检测阳性率为92.06%,健康对照组食物不耐受检测阳性率为16.67%,两组相比差异有统计学意义(P<0.05);食物特异性IgG阳性数与银屑病严重程度指标PASI评分呈正相关,rs=0.886,P<0.05,差异有统计学意义;观察组禁食/轮替调整6个月后IgG抗体水平与对照组相比明显下降,差异有统计学意义(P<0.05)。结论食物不耐受与银屑病之间具有相关性,饮食调整对本病的防治具有重要意义。  相似文献

食物依赖一运动诱发的过敏反应是食物过敏导致严重过敏反应的一种独特类型,临床较少见,单纯进食致敏食物或运动本身均不会导致其发生,但两者联合则会诱发不同程度的过敏反应,严重时可危及生命。发病机制尚不完全明了,各种假设包括:胃肠道通透性增加、血液的重新分布、阿司匹林的摄入、血液pH值的改变等。治疗上以抗组胺药物、肾上腺素和糖皮质激素等对症治疗为主。  相似文献
