针刺治疗功能性胃肠疾病疗效显著,但其中枢机制不明确,目前研究主要聚焦在某些中枢核团,如迷走背核复合体(DVC)、中缝大核(NRM)、蓝斑(LC)、背侧网状亚核(SRD)、下丘脑室旁核(PVN)、小脑顶核(FN)、中央杏仁核(CeA)等。核团之间是否通过某种相互联系参与针刺调节胃功能尚不清楚。进一步整理相关文献发现中枢神经系统许多脑区或核团都与胃功能密切相关,如DVC、NRM、臂旁核(PBN)、LC、中脑导水管周围灰质(PAG)、小脑、PVN、弓状核(Arc)、海马、CeA等,这些核团相互之间大多数都有一定的纤维联系,其中以DVC为基本中枢,其他核团都直接或间接地通过DVC参与对胃功能的调控。针刺调节胃功能是否也以DVC为关键靶点,其他核团直接或间接与DVC形成某种神经环路参与针刺对胃功能的调控?如探讨CeA-DVC神经环路参与针刺调节胃功能的可能性。因此引入更先进的光遗传、化学遗传等技术,从神经环路角度以DVC为中心开展针刺调节胃功能的中枢机制的研究将成为下一步的重点,以实现运用现代神经科学技术阐释腧穴配伍的中枢整合机制。 相似文献

孤束核在针刺“内关”调节胃运动中的作用研究 总被引:1,自引:0,他引:1
目的:探讨迷走神经中枢孤束核在针刺"内关"对胃运动异常大鼠胃内压影响中的作用机制。方法:采用神经电生理学方法,用微电极细胞外记录大鼠孤束核单个神经元的活动,在正常生理状态下及经血管注射抑制胃运动的药物阿托品或促进胃运动的药物胃复安后,观察针刺"内关"对孤束核神经元放电及胃内压的影响。结果:针刺各组大鼠"内关"30 s,NTS神经元单细胞活动均比针刺前有明显增加,且这种神经元放电的激活均具有统计学的极显著性差异(P0.01)。无论是在正常大鼠、阿托品后胃运动低下大鼠,还是胃复安后胃运动亢进大鼠,针刺"内关"均能引起大鼠胃内压的显著升高,与针刺前相比,这种胃内压水平的增加具有统计学的显著性差异(P0.01)。结论:针刺"内关"对不同胃运动状态的模型大鼠NTS神经元活动均有明显的激活作用,针刺"内关"对胃功能的调节作用以促进胃运动为主。针刺"内关"对胃运动的这种调节作用可能是通过激活延髓内与内脏传入信息相关的中枢核团神经元而实现的。 相似文献

本文总结了针灸治疗胃镜检查副反应的临床应用和针灸调节胃肠活动基础研究两个方面的资料。临床应用部分包括针灸治疗胃镜检查副反应的常用穴位、常用方法,针刺减轻胃镜检查心率异常,针灸配合胃镜检查提高胃镜诊断准确率;相关的针灸调节胃肠功能活动的实验研究包括针灸对胃运动及胃电的影响、对胃酸及胃肠激素的调整作用、对胃粘膜的保护作用以及经络及穴位的特异性。认为针灸治疗胃镜检查副反应疗效肯定,无副作用,比药物治疗更具优越性。 相似文献

Objective: This study focused on the effects of laser acupuncture(LA) and manual acupuncture(MA) at different acupoints on gastric motility and heart rate variability(HRV) simultaneously to elucidate the site-specific effects of acupoints and the correlation between changes of gastric motility and low frequency/high frequency(LF/HF) ratio.Methods: Ten male healthy Sprague-Dawley rats(weight 190-250 g) were anesthetized with an intraperitoneal injection of 10% urethane(1.2 g/kg).Gastric motility and HRV were recorded before and during MA or LA.Results: Stimulating on PC6 or ST36 significantly enhanced gastric motility, while stimulating on BL21 caused no change.In contrast, MA or LA at CV12 significantly suppressed gastric motility.Stimulating PC6 or ST36 significantly increased heart rate(HR), while stimulating on CV12 or BL21 induced no significant changes of HR.Stimulating PC6 significantly increased LF/HF, while stimulating on ST36, CV12 or BL21 induced no significant effects.Conclusion: These results indicated that there was acupoint specificity in the effects of acupuncture on gastric motility and HRV.The stimulatory effects of MA and LA at PC6 and ST36 on HR were associated with sympathetic activity.The stimulatory effect of MA or LA at PC6 or ST36 on gastric motility was associated with vagal activity.Laser needle can be used as an alternative stimulation therapy. 相似文献

Objective: To investigate the effects of paired ST36 and RN12 acupuncture stimulation on gastric motility and the firing rate of responsible neurons in anesthetized rats.Methods: Using electrophysiological methods, we measured the effects of acupuncture at RN12, ST36, or RN12+ST36 on gastric motility as well as nerve discharges from the gastric sympathetic, vagal nerve, and wide dynamic range(WDR) neurons in the spinal dorsal horn T7-9 in anesthetized rats.Results: 1) Acupuncture at RN12 inhibited gastric motility(83.84±4.49)% and vagal nerve discharge(65.64±5.67)%, but promoted sympathetic nerve activity(243.70±40.67)%; acupuncture at ST36 induced opposing effects on gastric motility(111.31±2.01)% and vagal nerve discharge(166.98±15.92)%. Interestingly, acupuncture at RN12+ST36 together yielded effects similar to that of acupuncture at RN12 alone(93.59±2.05)%,(54.52±7.12)%. 2) WDR neuron discharges induced by gastric distension were inhibited by acupuncture at RN12(194.56 ±18.79)%. However, acupuncture at ST36 induced a more significant inhibitory effect.Conclusion: Acupuncture at RN12 inhibits gastric motility by activation of the sympathetic reflex at the spinal level; acupuncture at ST36 promotes gastric motility by activation of the parasympathetic reflex at the supraspinal level, and acupuncture at RN12+ST36 produces a similar decrease in gastric motility as RN12 stimulation alone. Both acupoints decrease the effect of noxious gastric distention on WDR neuron activity, but paired RN12+ST36 stimulation does not significantly affect WDR neuron discharge. 相似文献

OBJECTIVE: To observe the effect of gastric distention(GD) and acupuncture at three different acupoints on the spontaneous discharge of neurons in the medial vestibular nucleus(MVN), and to clarify the specific function of the MVN in the central integration mechanism underlying acupuncture regulation.METHODS: GD was conducted using a balloon inserted in the stomach cavity, and acupuncture was performed separately at each of three acupoints:Zusanli(ST 36), Quchi(LI 11), and Weishu(BL 21).The effect of acupuncture and GD on the spontaneous discharge of MVN neurons was assessed using a glass microelectrode filled with a sodium acetate electrolyte solution containing 1% pontamine sky blue; the discharge signals from the neurons were amplified by the microelectrode amplifier and recorded in the Spike2 system.RESULTS: GD and acupuncture significantly affected the spontaneous discharge of MVN neurons. Furthermore, acupuncture at Zusanli(ST 36) and Weishu(BL 21) was significantly more effective at altering the discharge of GD-responsive MVN neurons compared with GD-nonresponsive neurons.CONCLUSION: GD and acupuncture at three different acupoints affected the electrical activity of MVN neurons. The MVN is involved in the central integration mechanism underlying acupuncture regulation of gastric functions. The effects of acupuncture on gastric function may therefore be mediated via these particular MVN neurons. 相似文献

目的:探讨不同穴位对胃运动异常大鼠的双相调节效应以及产生这种效应的神经机制。方法:采用神经电生理学方法,用微电极细胞外记录大鼠孤束核单个神经元的活动,观察在正常生理状态及胃运动增强和胃运动抑制的病理状态下,分别针刺"足三里""内关""中脘""气海"对孤束核神经元放电及胃内压的影响。结果:在正常生理状态及胃运动增强和胃运动抑制的病理状态下,针刺"足三里""内关"均以促进胃运动为主,而针刺"中脘""气海"均以抑制胃运动为主,且针刺以上不同穴位均有效激活了孤束核神经元放电。结论:穴位的双相调节作用并不一定是同一穴位对不同病理状态下的内脏器官有双相调节效应,可能表现为不同穴位的综合作用是纠正机体机能活动向正常平衡与稳态状态方向的良性调节。 相似文献

目的:运用数据挖掘技术探求古代针灸治疗胃脘痛特定穴运用规律。方法:通过制定数据的来源和采集标准、规范病症腧穴名称,将数据挖掘技术运用于古代针灸治疗胃脘痛的特定穴规律研究。结果:针灸治疗胃脘痛选取特定穴88个,占总穴位90.74%,以胃经募、合、经、络穴为主,其中中脘运用频率最高;特定穴配伍分析显示,脾背俞配伍交会穴支持度最高,而脾胃经的募、背俞穴与交会穴相配伍最多。结论:特定穴的选取及特定穴间的配伍是古代针灸治疗胃脘痛的主要处方原则,体现了循经取穴的特点,且操作方法以艾灸为主,针刺为辅,不重视针刺手法及灸法。 相似文献
