This article reports the qualitative findings from a focus group interview conducted with six currently registered nurses who participated in a summer nurse externship program. It is Part II of a three-part study that examined the influence of nurse externship programs on the transition process from graduate to professional nurse. The focus group explored the perceptions of former nurses about the impact of the externship program on their transition into the professional nurse role. In the role of nurse externs, individuals were able to view, experience, and, consequently, come to understand the culture of nursing in an acute healthcare system without being full-fledged members of it. Three themes induced from the analysis of the findings were as follows: (1) being a part of the real scene; (2) figuring out the environment; and (3) gaining awareness and becoming frightened.  相似文献

This is the third and final part of a series of articles that report the findings of a study that investigated the effects of a summer externship program on the transition into a professional role among nurses who participated in a summer nurse externship program at an acute care pediatric hospital. Specifically, this article reports the retention and recruitment benefits of a summer nurse externship program for the institution from 1998 through 2003. A total of 153 externs out of the 193 (79.0%) assumed a registered nurse position at the institution, and 77% remained in that role for 12 months. Overall, the nurse extern program for this institution appeared to be an effective recruitment and retention strategy that is on par with the institution and national data for retention and turnover data.  相似文献

Little is known about the perceptions of nursing students externing in newly developed hospital-based programs that focus on socialization and transition to the registered nurse (RN) role rather than on institutional recruitment and retention goals. This qualitative study explored student nurse externs' expectations, experiences, and benefits of participation in a student-focused externship program. Externs wanted to gain experience with skills and learn what it was like to be an RN. Goals were met or exceeded by becoming comfortable in the externship role, growing in skill performance and confidence, and becoming members of the healthcare team. The experience fostered growth from the novice to advanced beginner level of nursing practice. Externs saw the program participation as a valuable way to gain experience and learn what it was like to be an RN.  相似文献

In response to a critical shortage of registered nurses, an accelerated transition program for licensed practical nurses (LPNs) was developed and implemented. A study of the program was undertaken to evaluate students' academic achievement and socialization into the RN role. NCLEX-RN scores and the Professional Nursing Questionnaire (PNQ) of Minnick, Yocum and Scherubel were used for evaluation. Experimental program students were compared with students in the second year of conventional two-year nursing programs. Results of role socialization revealed no significant differences between the groups at either the beginning or end of the program. NCLEX-RN scores were not significantly different. The lack of significant results may be attributable to the inability of the PNQ to detect changes over a short time, or to LPNs' knowledge of the RN role prior to program entry. NCLEX score results indicated that comparable academic achievement was possible in the accelerated program.  相似文献

This study examined the effects of indoor gardening on socialization, activities of daily living (ADLs), and perceptions of loneliness in elderly nursing home residents. A total of 66 residents from two nursing homes participated in this two-phase study. In phase one, experimental group 1 participated once a week for 5 weeks in gardening activities while a control group received a 20-minute visit. While no significant differences were found between groups in socialization or perceptions of loneliness, there were significant pretest-posttest differences within groups on loneliness and guidance, reassurance of worth, social integration, and reliable alliance. The results also demonstrated gardening interventions had a significant effect on three ADLs (transfer, eating, and toileting). Phase two examined differences in the effects of a 5-week versus a 2-week intervention program. Although no significant within-group differences were noted in socialization, loneliness, or ADLs, the 5-week program was more effective in increasing socialization and physical functioning.  相似文献

WITH THE GROWING SHORTAGE of nurses in the workforce, perioperative leaders are facing a significant challenge in recruiting new nurses who are committed to perioperative nursing.ONE MEDICAL CENTER in northern Florida developed a successful summer externship program to introduce nursing students to the perioperative environment. The program incorporates both didactic and clinical experiences.STUDENTS WHO COMPLETE the program are better prepared and motivated to choose a career in perioperative nursing. The retention rate for nurses from the externship program who were hired into the OR in which the program was conducted was 89% after two years. [目的]探讨团体心理辅导对精神专科护士自我接纳与人际关系的影响.[方法]对参加精神专科护士培训的44名护士进行为期10周的成长性团体心理辅导,采用自我接纳量表及人际关系综合诊断量表评价辅导前后精神专科护士的自我接纳状况及人际关系.[结果]团体心理辅导前后自我接纳量表总分、自我接纳因子分、自我评价因子分比较差异有统计学意义(P<0.05);团体心理辅导前后人际关系总分、交友方面、待人接物方面比较差异有统计学意义;交谈方面与接纳总分、评价因子呈负相关,异性交往与接纳因子、评价因子及接纳总分均呈负相关,人际关系总分与评价因子和接纳总分呈负相关.[结论]成长性团体心理辅导有助于提高精神专科护士的自我接纳度,改善人际关系,帮助参加培训的护士更好地适应工作.  相似文献

目的 探讨基于角色转变冲击模型制订的ICU新护士培训方案的应用效果。方法 2018年7月选取6所三级甲等医院入职的100名ICU新护士作为研究对象,随机分为试验组和对照组。试验组按照基于角色转变冲击模型制订的培训方案给予培训,对照组给予常规培训。采用新护士转型冲击评价量表和护理行为六维度量表评价培训效果。 结果 培训后,试验组转型冲击量表总得分及各维度得分均低于对照组,差异有统计学意义(P<0.05);试验组护理行为六维度量表总分及各维度得分均高于对照组,差异有统计学意义(P<0.05)。结论 ICU新护士培训方案可以减轻ICU新护士转型冲击,提升ICU新护士的岗位胜任力。  相似文献

The purpose of this quasi-experimental study was to determine the effect of preceptorship on the socialization of the baccalaureate graduate nurse into roles of professional nurses. Two groups, one having a preceptorship experience in the final quarter of their baccalaureate program (n = 33) and one having the traditional course (n = 33), participated in the study. Both groups completed Schwerian's Six-Dimension Scale of Nursing Performance on three testing occasions: prior to the course, immediately following the course, and the six months after graduation. There was a significant interaction effect between group and time. The preceptor group at the 6-month follow-up scored significantly higher on four of the six subscales as well as on the overall socialization instrument.  相似文献

Research indicates significant differences between nurse cohorts in many work‐related factors. This study compared nurse competence between three generational cohorts comprising the current nursing workforce. The Nurse Competence Scale was used to collect data for this cross‐sectional study from 2052 nurses in a university hospital in Finland. Data were analysed statistically. Significant differences were found between nurse cohorts in their competence. The length of work experience had a significant impact on the development of competence. The oldest cohort, with the longest work experience, had the highest competence scores (70.1 on a visual analogue scale), and the youngest had the lowest (59.0). All cohorts were most competent in patient‐related nursing tasks, in maintenance of professional competence and in ethical care. Nurses were weakest in the development of nursing practice and the use of evidence‐based knowledge. Targeted interventions in teaching–coaching for different nurse generations are needed to ensure the maintenance of nurse competence and high‐quality patient care.  相似文献

PROBLEM: New nurse graduates experience a stressful role transition into healthcare organizations, with 30% leaving their first job within 1 year and 57% leaving by 2 years of employment. AIM: This study tests whether an internship program, Social and Professional Reality Integration for Nurse Graduates (SPRING), improves new nurse graduate retention, sense of belonging, organizational commitment, and anticipated turnover. DESIGN: Quasi-experimental, posttest only, control group design. METHOD: New nurse graduates hired by an academic institution that completed SPRING were compared with baseline nurse graduates who did not complete SPRING, using the Organizational Commitment Questionnaire, Modified Hagerty-Patusky Sense of Belonging Instrument, and Anticipated Turnover Scale. Retention was also examined. RESULTS: Anticipated Turnover was higher for baseline nurses than for 6-month SPRING nurses. Six-month SPRING nurses have lower antecedent sense of belonging than baseline or 12-month SPRING nurses. One-year retention is higher for SPRING new graduates than for non-SPRING new graduates. CONCLUSION: Internship programs for nurse graduates must support the socialization of nurses and their transition into the professional role as well as teach the skills and knowledge needed for competence. This study supports the value of a comprehensive program for new nurse graduates in improving nurse retention and decreasing new nurse graduate intent to leave the organization at 6 months.  相似文献

The aim of this research is to find the effect of mindfulness-based education given to ındividuals with substance-use disorder according to self-efficacy theory on self-efficacy perception. The research was planned as a test model with pretest-posttest control groups. Individuals who have been diagnosed with substance abuse in the substance abuse clinic of Turgut Ozal Medical Center, and at the Substance Abuse Treatment and Education Centre of Gaziantep 25 Aralık State Hospital formed the core of this study. The research was conducted between January 2018–May 2019. 112 patients, 56 in the treatment and 56 in the control group, participated in the study. Scientific research started after ethical approval. Mindfulness therapy was applied to experimental group patients. For the data collection, the Socio-Demographic Characteristics Questionnaire and Self-Efficacy-Sufficiency scale were used. t-Test for dependent variables to compare the mean scores of experimental and control groups and t-test for independent variables were used. It was seen that the average total Self-Efficacy-Sufficiency score of the patients in the treatment group was 76.50 ± 12.62 before the intervention and increased to 85.50 ± 14.95 after the intervention. There is a significant difference between the average scores. Even when the treatment and control groups are compared, a significant difference was still detected between the groups. There was no significant difference between the groups in two of the Self-Efficacy-Sufficiency steps after the intervention in the experimental group, but there were significant differences between the other two factors and total score averages.  相似文献

In this study, the professional and bureaucratic role conceptions and role deprivation of students participating in a preceptorship program were compared to those in a traditional faculty-supervised clinical group. The role conceptions and role deprivation of nursing faculty and preceptors were also examined. One hundred eighteen students in a two-year upper division baccalaureate program participated in this study. The Corwin Nursing Role Conception tool was administered. Results indicated no differences in role conceptions or role deprivation in students participating in the preceptorship program and those who did not. There were no differences in the developmental pattern of role conception and role deprivation during two years of nursing education. Also, nursing faculty had a significantly higher professional role conception and a significantly lower bureaucratic role conception than preceptors but there were no differences in role deprivation between the two groups.  相似文献

This population-based study compared nurses who had participated in a sexual assault nurse examiner training program (SANEs) to nurses who had not participated in the program (nonSANEs) on their opinions and practices in relation to the collection of forensic evidence. Self-administered surveys were distributed to all SANEs and non-SANEs employed in sexual assault care centres in Ontario (N = 317). We found that SANEs were more likely to indicate that certain samples, items, or questions should not be taken and/or asked as a regular part of the forensic examination. They were less likely to perceive the presence of physical injuries and semen and/or sperm as being "extremely important" to a positive legal outcome. Finally, more SANEs reported experiencing dilemmas with respect to their dual roles as caregivers and evidence collectors. These findings are discussed in relation to the more expansive and comprehensive experience and education of SANEs versus nonSANEs. Implications for care offered to victims of sexual assault are discussed.  相似文献

OBJECTIVE: To determine the effects of community based nurses specialising in Parkinson's disease on health outcomes and healthcare costs. DESIGN: Two year randomised controlled trial in 438 general practices in nine randomly selected health authority areas of England. PARTICIPANTS: 1859 patients with Parkinson's disease identified by the participating general practices. MAIN OUTCOME MEASURES: Survival, stand-up test, dot in square test, bone fracture, global health question, PDQ-39, Euroqol, and healthcare costs. RESULTS: After two years 315 (17.3%) patients had died, although mortality did not differ between those who were attended by nurse specialists and those receiving standard care from their general practitioner (hazard ratio for nurse group v control group 0.91, 95% confidence interval 0.73 to 1.13). No significant differences were found between the two groups for the stand-up test (odds ratio 1.15, 0.93 to 1.42) and dot in square score (difference -0.7, -3.25 to 1.84). Scores on the global health question were significantly better in patients attended by nurse specialists than in controls (difference -0.23, -0.4 to -0.06), but no difference was observed in the results of the PDQ-39 or Euroqol questionnaires. Direct costs for patient health care increased by an average of pound2658 during the study, although not differentially between groups: the average increase was pound266 lower among patients attended by a nurse specialist (- pound981 to pound449). CONCLUSIONS: Nurse specialists in Parkinson's disease had little effect on the clinical condition of patients, but they did improve their patients' sense of wellbeing, with no increase in patients' healthcare costs.  相似文献

目的了解不同编制和不同年龄护士对非经济薪酬的认识和需求。方法采用不同编制和不同年龄护士非经济薪酬需求量表对718名护士进行调查。结果护士对非经济薪酬的需求迫切,在编护士对非经济薪酬的需求平均得分为(4.01±0.39)分,合同制护士为(4.23±0.47)分,两者比较,t=-6.82,差异具有统计学意义(P<0.01)。不同年龄段护士对非经济薪酬需求比较,差异具有统计学意义(均P<0.01),25岁以下护士在有趣性活动、获得社会尊重、友谊关怀中得分较高;26-35岁护士则在获得社会尊重、职业发展机会、进修培训中得分较高;36-55岁护士倾向于获得社会尊重与友谊关怀。结论管理者应从组织层面、护士个体发展层面以及护理管理和制度上对护士非经济薪酬需求给予重视、关注和支持,使非经济薪酬发挥其独特的激励作用,以提高护士工作的积极性和主动性。  相似文献

目的:探讨不同应对方式对急诊护士心理健康状况的影响。描述不同应对方式与急诊护士心理健康状况之间的关系。方法:应用简易应对方式量表和康奈尔健康问卷对5家综合医院急诊科121名护士进行调查。结果:①护士积极应对方式得分高于常模,消极应对方式得分低于常模,差异均有统计学意义。②急诊护士最多的心理健康问题是不适应。③急诊科护士的心理健康状况评分与消极应对方式得分呈负相关,与积极应对方式得分呈正相关。结论:应对方式对急诊护士的心理健康有重要的调节作用。积极应对方式为保护因素,消极应对方式为危险因素。  相似文献
