彩色多普勒超声对急性阑尾炎的诊断价值

摘    要:目的:探讨彩色多普勒超声对急性阑尾炎的诊断价值。方法:联合应用低频和高频探头,采用前腹壁加压法、顺逆时针旋转法对86例急性阑尾炎患者进行彩色多普勒超声检查,与术后病理结果对照,分析其声像图特征。结果:超声诊断急性阑尾炎的直接征象为阑尾肿大、壁增厚,阑尾腔内积液和(或)阑尾周围脓肿等;间接征象包括阑尾周围高回声、肠间隙积液及周围肠管激惹征等。超声诊断急性阑尾炎的总符合率为87.2%,单纯性阑尾炎符合率83.3%,化脓性阑尾炎符合率93.1%,坏疽性阑尾炎符合率88.9%,阑尾周围脓肿符合率100.0%。结论:各型急性阑尾炎具有较典型声像图特征,超声诊断准确率较高,在临床应用与治疗方面有重要作用。

Abstract:Objective: To explore the diagnosis value of color Doppler ultrasound. Methods: A total of 86 cases with acute appendicitis underwent color Doppler ultrasound scanning by compression meth od of anterior abdominal well and rotation method in anticlockwise and clockwise with probes of high and low frequency. The images were analyzed and compared with pathological test after surgery. Results. The direct characters of ultrasonic image of acute appendicitis were swelling appendix with thickening well, effusion in cavity and/or periappendicular abscess. And indirect characters were high echo area around ap pendix, interintestinal effusion and peripheral irritable bowel syndrome etc. The total diagnosis accuracy of acute appendicitis by ultrasonic scanning was 87.2%, and the accuracy was 83.3% for simple appendicitis, 93.1% for suppurative appendicitis, 88.9 % for gangrenous appendicitis, and 100% for periappendicular abscess. Conclusions: The ultrasonic images of all acute appendicitis possess typical characters of ultrasonogram, and show the high diagnosis accuracy. It plays an important role in clinical application.
