老年非甲状腺疾病患者甲状腺功能检测及评价

本文对老年非甲状腺疾病患者107例、健康老年人30例的血清T_3、T_4、FT_3、FT_4、γT_3、TSH水平进行测定。结果表明,老年心血管疾病、恶性肿瘤、肺部疾病、糖尿病、肝病患者T_3明显降低(P<0.05),FT_3显著降低(P<0.01),γT_3显著增高(P<0.01)。老年其他疾病(神经系疾病、胆囊炎等)T_3虽无明显变化,但FT_3仍明显降低(P<0.05)。T_4仅在肺部疾病患者明显降低(P<0.05),其余各组T_4、FT_4、TSH均无显著改变,健康老年T_3较健康成年人明显降低(P<0.05),其变化程度与疾病严重程度有一定关系,其中以FT_3更为敏感。故认为,测定甲状腺激素水平对判断老年患者病情、估计预后有重要参考价值。

Thyroid Function in Elderly Patients with Non-Thyroid Disease

Serum total thyroxine (TT4), triidothyronine (T3) Free T3 and Free T4, reverse triidothyronine (rT4) andthyroid stimulating hormone (TSH) were determined in 107 elderly patients with non-Thyroid disease, with 30 healthyelderly and 52 normal adults as the control. Elderly patients with cardiovascular diseases, malignant tumors, pul-monary diseases, dlabetis mellitus, liver diseases and so on, their serum T3 were significantly decreased (p<0.05),FT3 were considerably lower and rT3 significantly increased (p<0.05), Elderly patients with nerve system diseases,chelocystitis and so on, their serum T3 were not changed significantly but FT3 were significantly lower (p<0.05).Serum T4, FT4, TSH were not significantly changed in other groups, T4 was considerably low in pulmonary diseases,and serum T3 in healthy old people was remarkably lower than the healthy adults. Thyroid function measurement ofthe elderly patients can be used to assess the severity of the disease and to predict their prognosis.
