颅底和脑深部肿瘤的显微手术治疗

目的 :报告 6 0例颅底和脑深部肿瘤的显微手术治疗。方法 :分析 6 0例肿瘤的临床和影像学特征 ,手术入路的选择 ,显微手术方法和技巧 ,手术结果和并发症。结果 :肿瘤全切除 30例 ,近全切除 18例 ,大部分切除 11例 ,部分切除 1例。能重返工作或料理家务 44例 ,能自理生活 11例 ,需人护理 2例 ,死亡 3例。结论 :颅底和脑深部肿瘤手术切除虽然困难 ,危险性大 ,但采用微创的显微外科技术 ,合理选择手术入路 ,积极防治并发症 ,可获得较好予后 ,提高病人术后生存质量。

Microsurgical Treatment of Tumors Involving Skull Base and in Deep Brain

Objective: To Discuss the microsurgical treatment of 60 cases with tumors involving skull base and in deep brain.Metods: the clinical features,preoperative CT and MRI images,operative approaches,micrsurgical techniques,results of operation,and postoperative complications of 60 cases with tumor were discussed.Results:A total removal of the tumor in 30 cases,a subtotal removal in 18 casesa major partial removal in 11 cases,and a partial removal in 1 case,were achieved,respectively,44 of 60 patiens had a good major postoperative recover without nourological deficit or with deficit but able to work or manage household.While operative death occurred in 3 patients.Conclusion: Using the MRI and CT in diagnosis,rational selection of operative approaches,and applying microsurgical techniques can improve operative results with expectation for prolonged good quality survival.
