显微手术治疗大型听神经瘤(附19例报告)

目的：探讨显微手术治疗大型听神经瘤操作技巧及临床疗效。方法：对19例巨大听神经瘤显微外科手术的病入作回顾性分析，17例采用单侧枕下乙状窦后入路，2例采用乙关窦前入路。结果：本组肿瘤全切15例；次全切除4例，无手术残废病例及其它并发症；镜下肿瘤全切率78.9%，术后面瘫6例；肿瘤复发3例；其余病入恢复良好；结论：对大型听神经瘤采用不同的手术入路，应用显微外科技术，可提高肿瘤的全切率，隆低残废率和病残率，有效地保留面神经的功能。

Microsurgical treatment of large acoustic neuroma

s Objective:To explore the skills and effect of microsurgery in treatment of large acoustic neuroma.Methods:A retrospective analysis of 19 patients with large acoustic neuroma was performed.17 patients were treated via unilateral retrosigmoid approach and 2 via pre-sigmoid sinus approach. Results:Total resection of the tumor was achieved in 15 of the 19 cases (78.9%) and subtotal in 4 cases; good outcome was achieved in 10 cases; facioplegia was observed in 6 cases and recurrent acoustic neuroma in 3 cases. Conclusion:Selection of different surgical approaches and the techniques of microsurgery can improve the rate of total tumor resection and decrease the morbidity and mortamity of patients with large acoustic neuroma.
