中西医结合防治髋关节置换术后下肢深静脉血栓形成的临床观察

目的：观察中西医结合防治髋关节置换术后下肢深静脉血栓形成的疗效。方法：68例纳入观察的患者随机分为观察组和对照组各34例，两组患者均接受髋关节置换、术后功能锻炼和肝素抗凝等常规治疗，观察组术后1d起同时给予中药舒筋活血汤加减治疗，3周后观察两组患者治疗前后的凝血功能和DVT发生率。结果：观察组发生DVT1例（2．9％），对照组发生DVT4例（11．8％），观察组术后DVT发生率明显低于对照组，但经，检验，差异无统计学意义（P〉0．05）；两组患者治疗后APTT和PT均有不同程度的升高，与治疗前对比差异均具有统计学意义（P〈0．05）；但治疗后观察组的APTT和PT均高于对照组，差异均具有统计学意义（P〈0．05）。结论：中西医结合在对髋关节置换术患者的抗凝方面起到了协同作用，更有效的降低了髋关节置换后DVT的发生率，值得临床运用。

Methods： 68 cases were randomly divided into observation group and control group with 34 cases in each, Two groups of patients undergoing hip replacement, postoperative functional exercise and heparin and other conventional treatment, the observation group was given Shujin Huoxue decoction 1 days after surgery, after 1 days, 3 weeks after the two groups were observed before and after treatment in patients with coagulation function and the incidence rate of DVT. Results： The observation group occurred in DVT1 cases （2.9%）, control group DVT4 patients （11.8%）, DVT occurred in the patients in the observation group was significantly lower than that of the control group, but by x2 test, no significant difference （P〉0.05）; Elevated APTT and PT in varying degrees after treatment in two groups, compared with before treatment, the differences were statistically significant （AP〈0.05）; But after treatment in observation group, APTT and PT were higher than those in the control group, the differences were statistically significant （＊P〈0.05）. Conclusion： In anticoagulation for patients with hip replacement surgery has played a synergistic effect of combination of the integrative medicine, more effective to reduce the incidence of DVT after hip replacement, it is worthy of clinical application.
