急诊内镜对上消化道非静脉曲张性出血的临床疗效分析

目的探讨急诊内镜对上消化道非静脉曲张性出血的临床疗效。方法选取2008年9月至2011年9月荆州市第一人民医院确诊的118例上消化道非静脉曲张性出血患者作为研究对象,根据诊疗方法分为急诊内镜组和内科常规治疗组,应用SPSS 14.0统计学软件对两组的病因检出率、有效止血率、再出血率和手术率等数据进行χ2检验分析,比较两组的疗效情况。结果急诊内镜组的病因检出率、有效止血率显著高于内科常规治疗组,分别为93.0%对60.7%,93.0%对70.5%;再出血率和手术率显著低于内科常规治疗组,分别为3.5%对16.4%以及3.5%对11.5%,比较差异均有统计学意义(P<0.05)。结论急诊内镜下治疗上消化道非静脉曲张性出血患者具有诊断率高、并发症少、疗效可靠等优点,值得在临床上进一步研究和推广。

Value of Emergent Endoscopy in Diagnosis and Treatment of Acute Nonvariceal Upper Gastrointestinal Bleeding

Objective To investigate the value of emergent endoscopy in diagnosis and treatment of acute nonvariceal upper gastrointestinal bleeding(ANVUGIB).Methods 118 ANVUGIB patients in the First People′s Hospital of Jingzhou from September 2008 to September 2011 were collected as study objects,which were divided into emergent endoscopy group and conventional therapy group according to the diagnosis and treatment method.Application SPSS 14.0 statistical software was used to carry out χ2 test to analyze the diagnosis rates,hemostasis success rates,rebleeding rates and surgery rates of the groups were compared.Results The diagnosis rate and success rate of hemostasis of emergent endoscopy were significantly higher than that of conventional therapy group,with the corresponding data of 93.0% vs 60.7%,93.0% vs 70.5%,but the rates of rebleeding and surgery were much lower than conventional therapy group,with the corresponding data of 3.5% vs 16.4% and 3.5% vs 11.5%(P<0.05).Conclusion Emergent endoscopy treatment of ANVUGIB is with the advantages of high diagnosis rate,less complication and reliable efficacy,and it is worthy of further study and application in clinical practice.
