血清TPSA、FPSA和F/TPSA检测在BPH与PCa诊断中的意义

摘    要:目的:探讨血清总前列腺特异性抗原(TPSA)和游离前列腺特异性抗原(FPSA)与F/TPSA比值在诊断和鉴别诊断前列腺增生(BPH)与前列腺癌(PCa)中的作用及其疗效和愈后观察。方法:采用化学发光法测定26例BPH和29例PCa患者及86例其它疾病患者血清的TPSA和FPSA水平并计算F/TPSA比值。结果:非PCa病人血清的TPSA和FPSA水平与年龄有显著相关性(P<0.01),而F/TPSA与年龄无明显相关(P>0.05);随着年龄增加,PCa的发病率也增加(P<0.025);3组中PCa组的TPSA与FPSA水平明显高于其它两组(P<0.001),F/TPSA则低于其它两组(P<0.002)。结论:联合检测TPSA、FPSA和F/TPSA可明显提高BPH与PCa诊断的准确性,有助于肿瘤疾病筛选,对PCa疗效和预后观察有一定的意义。

Abstract:Objective:To investigate the serum levels of total prostatic specific antigen(TPSA),free prostatic specific antigen(FPSA) and T/FPSA ratio and the significance to diagnosis and differential diagnosis of benign prostatic hyperplasia(BPH) and prostatic cancer(PCa),and to evaluate the curative effect and the prognosis of these diseases.Methods:Serum TPSA and FPSA levels were measured with chemiluminescence immunoassay in 26 patients with BPH,29 patients with PCa and 86 controls.F/TPSA ratio was calculated from the data.Results: Serum TPSA and FPSA levels of 26 patients with BPH and 86 controls increased with age and there were positive correlations(P<0.01),but F/TPSA ratio didn't correlate with age(P>0.05).The diseased ratio of PCa increased with age and there were positive correlations (P<0.025).Serum TPSA and FPSA levels were significantly higher in patients with PCa than those in BPH and the controls(P<0.01).F/TPSA ratio in patients with PCa was lower than that in BPH and the controls(P<0.002).Conclusion:Combined determination of TPSA,FPSA and F/TPSA can enhance the sensitivity and specificity for diagnosis of BPH and PCa.That determination aids in screening the tumor disease and there is clinical significance in curative effect and prognosis of PCa.
