高血糖管理路径在皮肤科住院患者血糖管理中的应用

目的 探讨高血糖管理路径在皮肤科住院患者血糖管理中的应用效果。 方法 将2017年4-9月收治的245例皮肤科住院患者设为对照组,采用常规血糖管理方案,将2017年10月-2018年3月收治的240例皮肤科住院患者设为观察组,实施高血糖管理路径。比较2组糖化血红蛋白检测人数、内分泌科会诊次数、最大血糖波动幅度、平均住院天数以及住院费用。 结果 观察组糖化血红蛋白检测率、内分泌科会诊次数均高于对照组(χ²=54.258,P<0.001;χ²=14.091,P＜0.001),住院费用低于对照组(t=-4.552,P＜0.001),观察组出院前1天最大血糖波动幅度低于对照组(t=4.561,P＜0.001)。 结论 在皮肤科患者治疗中实施高血糖管理路径,有助于筛查出异常血糖,提高糖化血红蛋白检测率,及时采取干预,为合理用药提供依据,降低患者血糖波动幅度,减少患者住院费用。

Application of hyperglycemia management pathway in blood sugar management of inpatients in the dermatology department

Objective To explore the application of the hyperglycemia management pathway in the blood sugar management of inpatients in the dermatology department.Methods Totally 245 dermatological inpatients admitted between April and September 2017 were selected into a control group and given the routine blood sugar management,while 240 admitted between October 2017 and March 2018 were chosen into an observation group and implemented the hyperglycemia management pathway.The positive rate of hemoglobin A1c(HbA1c),the number of endocrinology consultation,the biggest glucose fluctuation,the average length of stay and hospitalization cost were compared between the two groups.Results The positive rate of hemoglobin A1c(HbA1c)and the number of endocrinology consultation of the observation group were significantly higher than that of the control group(χ²=54.258,P<0.001;χ2=14.091,P<0.001),while the range of glucose fluctuation and hospitalization cost were significantly lower than the latter(t=4.561,P<0.001;t=-4.552,P<0.001).Conclusion The implementation of the hyperglycemia management pathway can help to find out abnormal blood sugar and HbA1c in dermatology patients,so that timely intervention and proper medications can be provided to lower the fluctuation of glucose level and cost of hospitalization.
