巨大儿体格发育和气质特点研究

目的:探讨巨大儿体格发育规律和气质特点,为科学育儿提供客观依据。方法:采用1∶1配对条件选择巨大儿和正常出生体重儿各50名,对8个婴幼儿期生长发育监测点的体重和身长进行比较分析,根据CPTS对两组儿童进行气质测评。结果:巨大儿组体重在36个月以内均高于正常出生体重儿组(P<0.01);身长在36个月时已不占优势,与正常出生体重儿组身长无明显差异(P>0.05)。巨大儿在各个生长发育监测点有肥胖倾向(BMI>18)的人数比例均超过正常出生体重儿组,差异有统计学意义。巨大儿组与正常出生体重儿组气质构成无差异;巨大儿组适应性和心境得分高于正常出生体重儿组,其余气质维度得分两组无差异(P>0.05)。结论:宫内过度生长与儿童婴幼儿期体格发育有密切关系,巨大儿在保证母乳喂养和按时添加辅食的情况下,仍易发生脂肪的过度积累。在巨大儿的气质特点中,适应性较慢、心境相对消极。应根据巨大儿的体格发育和气质采取正确干预指导措施,促使巨大儿获得身心最佳发展。

Study on the characteristic patterns of physical development and temperament status of macrosomia

Objective:To investigate the physical development and temperament status of macrosomia and provide guide lines for child health care.Methods:Weight and length of 8 growth development monitoring time for 50 macrosomia and 50 normal birth weigh infants selected by 1∶ 1 case-control study were analyzed .Temperamental characteristics were studied by using CPTS.Results:The weight of macrosomia group was higher than that of control group in 36 months(P<0.01).However the length of macrosomia group was not significantly different with that of control group when they were 36 months(P>0.05).The rate of BMI>18 in macrosomia group was relatively higher than that in control group within every growth development monitoring time. No significant difference was found in temperament between macrosomia group and control group,but the adaptability and quality of mood scores of macrosomia group were higher than control group(P<0.05).Conclusion:The intervention and instruction must be reinforced according to the feature of physical development and temperament of macrosomia.
