食管鳞癌中Survivin和Smac/Diablo的表达及意义

摘    要:目的:探讨Survivin和Smac/Diablo在食管鳞癌中的表达及其临床意义。方法:应用免疫组化SP法对80例经病理确诊为食管鳞癌的病例和相对应的30例病理确定为正常组织及30例癌旁组织进行Survivin蛋白和Smac/Diablo蛋白半定量免疫组化分析。结果:Survivin在食管鳞癌组织,癌旁组织,正常组织的阳性表达率分别为81.25%,10.0%,3.33%,有显著性差异(χ2=72.983,P<0.01)。Smac/Diablo在食管鳞癌组织,癌旁组织,正常组织的阳性表达率分别为61.25%,76.67%,93.33%,有显著性差异(χ2=11.518,P<0.01)。Survivin和Smac/Diablo表达与患者的年龄、性别、分化程度、浸润深度等均无明显关系(P>0.05)。但与TNM分期、淋巴结转移相关(P<0.05)。在Smac阳性组中的表达率为23.21%(13/56),在Smac/Diablo阴性组中的表达率为16.67%(9/54),两者间有统计学差异(P<0.05),两者关联性较强,呈负相关。Survivin阳性率与淋巴结转移呈低度正相关(r=0.329,P<0.05),Smac/Diablo阳性率与淋巴结转移呈负相关(r=-0.733,P<0.05)。结论:Survivin蛋白和Smac/Diablo蛋白表达与食管癌的病理分期,淋巴结转移都有着密切的关系,Smac/Diablo在食管鳞癌组织中低表达,其表达和Survivin呈明显的负相关,两者可能在食管鳞癌细胞凋亡信号传导网络中发挥重要作用。

The Expression and significance of Survivin and Smac / Diablo in esophageal squamous cell carcinoma

Abstract:Objective:To investgate the expression and clinical significance of Survivin and Smac / Diablo in esophageal squamous cell carcinoma , thereby further to explore its clinical value. Methods :Smac and Survivin protein expression was detected using immunohistochemistry respectively in 80 esophageal squamous cell carcinoma ,30 para-carcinoma tissues and 30 normal esophageal tissues. Results:Survivin proteins positive expression rate respectively was 81.25% in ESCC , 10.0%in adjacent tissues and 3.33% in normal tissues , with significant difference (χ^2= 72. 983, P〈0. 01). Smac proteins positive expression rate respectively was 61. 25% in ESCC , 76.67% in adjacent tissues and 93.33% in normal tissues , with significant difference (χ^2= 11. 518, P〈0. 01). Survivin and Smac proteins expression had significant difference in TNM staging and LNM respectively. (P〈0.05). Survivin and Smac expression had no significant relationship with patient's age, sex, differentiation, invasion depth respectively (P〉 0. 05). Survivin in the Smac-positive group and negative expression group were 23.21% (13/56) and 16.67% (9 / 54), there is significant difference between the two groups (χ^2= 8. 528, P 〈0. 05; r -0. 324, P 〈0. 05), with a strong correlation, and was a negative correlation. Survivin proteins positive expression rate had positive correlation with the LNM (r = 0. 329, P〈0. 05),Smac protein expression had negative correlation with the LNM (r =- 0. 733, P 〈 0. 05)o Conclusion: Survivin protein and Smac / Diablo protein expression were closely related with the pathological stage of esophageal cancer, lymph node metastasis. Smac was a negative correlation with Survivin and the two genes might be an important link in the signal pathway of ESCC cells.
