主动免疫治疗复发性自然流产的免疫功能监测

目的 :观察复发性自然流产 (recurrentspontaneousabortion ,RSA)患者主动免疫治疗前后细胞免疫功能的变化情况 ,探讨主动免疫对RSA的疗效。方法 :采用小剂量丈夫淋巴细胞皮内注射治疗RSA32例 ,观察其局部皮肤反应及治疗前后外周血CD4、CD8细胞和CD4/CD8的变化。结果 :局部皮肤反应随免疫次数的增加而逐渐减轻 ,首次免疫与末次免疫后皮肤反应面积比较 ,差异有显著性 (P <0 .0 0 1) ,妊娠结局良好。主动免疫治疗前后CD4细胞百分比无明显改变 (P >0 .0 5 ) ,而CD8细胞百分比治疗后有明显升高 (P <0 .0 5 ) ,CD4/CD8比率明显下降 (P <0 .0 0 1)。结论 :局部皮肤反应联合细胞免疫功能测定可以有效地监测主动免疫治疗效果 ,提高免疫治疗的成功率。

The observation of treatment of recurrent spontaneous abortion by active immunotherapy

Objective:To observe the changes of cellular immunity after the therapy and investigate the effect of the treatment of recurrent spontaneous abortion by active immunotherapy.Methods:32 cases with RSA were treated by their husbands' lymphocyte and were observed the local skin reaction.The changes of the serum CD 4,CD 8 cell and the ratio of CD 4/CD 8 before and after the active immunotherapy were observed simultaneously.Results:The local skin reactions were gradually alleviated by the active immunotherapy.The skin reaction area of the last therapy were remarkablely different from that of the first therpy time ( P <0.001),and the pregnant results after therpy are satisfactable.The percentage of CD 4 cell had no significant change after the treatment( P >0.05),but the percentage of CD 8 cell was higher significantly ( P <0.05) and the ratio of CD 4/CD 8 was decreased remarkably after theactive immunotherapy ( P <0.001).Conclusion:Application of local skin reaction combined with cellular immunity could effectively inspect the effect of active immunotherapy,and raise the success rate of treatment.
