超声造影在原发性与转移性小肝癌诊断中的应用

目的:分析原发性与转移性小肝癌(≤3cm)的造影剂灌注特征并探讨其临床价值。方法:经病理证实的67例小肝癌患者行超声造影(contrast-enhanced ultrasonography,CEUS)检查,并分析其造影特征。结果:超声造影诊断小肝癌的准确性为95.5%(64/67)。原发性肝癌中93.5%(43/46)病灶为多血供,造影表现为动脉相整体均匀增强;转移性肝癌中76.2%(16/21)病灶为乏血供,造影表现为动脉相周边呈环状或内部呈不规则散在点状增强。结论:超声造影对小肝癌诊断具有重要临床价值,但对部分小肝癌的不典型造影表现有待进一步研究。

Application of contrast-enhanced ultrasonography on the diagnosis of small primary and metastatic hepatic carcinoma

Objective:To identify the contrast medium perfusion characteristic of small primary and metastatic hepatic carcinoma(≤3cm) and study the value of contrast-enhanced ultrasonography.Methods: Sixty-seven patients of small hepatic carcinoma confirmed by pathology were examined and analyzed by contrast-enhanced ultrasonography.Results: The accuracy of small hepatic carcinoma diagnosed by contrast-enhanced ultrasonography was 95.5%(64/67).Contrast-enhanced ultrasonography showed 93.5%(43/46) of primary hepatic carcinoma were hypervascular and total homogeneous enhancement in arterial phase,76.2%(16/21) of metastatic hepatic carcinoma were hypovascular and ring-like around or irregularly dispersive punctual enhancement in arterial phase.Conclusion: Contrast-enhanced ultrasonography has an important clinical value for the small hepatic carcinoma.But atypical contrast-enhanced ultrasonography of part hepatic carcinoma need further research.
