18F-FDG PET/CT联合DWMRI影像特点与NSCLC患者瘤细胞功能状态及总生存期的关系研究

摘    要:目的:探讨非小细胞肺癌(NSCLC)患者的18F-脱氧葡萄糖(18F-FDG)PET/CT联合弥散加权磁共振成像(DWMRI)影像特点与其瘤细胞功能状态及总生存期的关系。方法:选取医院收治的100例NSCLC患者资料,应用PET/CT进行检查,依据患者的组织病理学结果,分析不同病理状态患者的最大标准化摄取值(SUVmax)、总生存期(OS)以及无进展生存期(PFS)之间的差异,同时针对以上差异性指标展开多因素和相关性分析。结果:NSCLC患者不同肿瘤最大径≤3 cm及>3 cm之间的OS、PFS和SUVmax差异显著,具有统计学意义(t=10.526,t=7.865,t=20.433;P<0.05);淋巴结转移和TNM分期情况之间的OS、PFS和SUVmax差异有统计学意义(t淋巴结转移=9.186,t=11.256,t=3.416;tTNM分期=10.163,t=11.101,t=9.045;P<0.05);肿瘤最大径、淋巴结转移以及TNM分期均为SUVmax、OS以及PFS的独立危险因素;肿瘤最大径、淋巴结转移以及TNM分期分别与SUVmax、OS以及PFS呈现负相关。结论:NSCLC患者的不同进展状态下,患者的局部病灶代谢水平显著不同,可根据此对患者的治疗效果以及预后进行量化评估。

Study on the relationship between the imaging characteristics of 18F-FDG PET/CT with DWMR and functional situation of tumor cell and overall survival of patients with NSCLC
Abstract:Objective: To investigate the relationship between the imaging characteristics of 18F-FDG PET/CT that combined with diffusion weighted magnetic resonance(DWMR), and functional situation of tumor cell and overall survival(OS) of patients with non-squamous non-small cell lung cancer(NSCLC). Methods: The data of 100 patients with NSCLC who admitted to our hospital were selected as the research objects, and these patients were examined by PET/CT. According to the results of histopathology of patients, the differences of SUVmax, OS and progression-free survival(PFS) among patients with different pathological conditions were analyzed. At the same time, the analysis of multi-factors and correlation which aimed at these diversity indicators was carried out. Results: The differences of OS, PFS and SUVmax of patient with NSCLC were significant when the maximum diameter of different tumors was less or equal 3 cm and larger 3 cm(t=10.526, t=7.865, t=20.433, P<0.05). The differences of OS, PFS and SUVmax of lymph node metastasis and among different TNM staging were significant(tlymph node metastasis=9.186, t=11.256, t=3.416, tTNM staging=10.163, t=11.101, t=9.045, P<0.05). And the maximum diameter of tumor, lymph node metastasis and TNM staging were independent risk factors of SUVmax, OS and PFS. Besides, these risk factors were negatively correlated with SUVmax, OS and PFS, respectively. Conclusion: Under different development situations of patients with NSCLC, the metabolic levels of local lesions of patients with NSCLC are significantly different. And based on this, therapeutic effect and prognosis of patients can be quantitatively evaluated.
