无创正压机械通气治疗慢性阻塞性肺疾病急性加重期合并呼吸衰竭42例疗效观察

目的探讨无创正压机械通气(NIPPV)治疗慢性阻塞性肺疾病(COPD)急性加重期合并呼吸衰竭的临床疗效。方法选择COPD急性加重期合并呼吸衰竭患者42例,患者均给予双水平气道正压通气(BiPAP)治疗,同时监测心率、呼吸及动脉血气分析氧分压(PaO2)、二氧化碳分压(PaO2)、pH]等指标改善情况。结果治疗总有效率为83.3%。治疗后心率、呼吸及血气分析等指标均明显改善,差异均有统计学意义(P<0.05)。结论 NIPPV治疗COPD急性加重期合并呼吸衰竭患者疗效显著,可明显改善COPD患者的血气指标、肺功能及临床症状,值得临床推广应用。

Observation of noninvasive positive pressure ventilation on 42 cases of chronic obstructive pulmonary disease of acute exacerbations with respiratory failure

Objective To explore the clinical efficacy of noninvasive positive pressure ventilation（NIPPV） in chronic obstructive pulmonary disease （COPD） of acute exacerbation with respiratory failure. Methods Choose 42 patients with COPD of acute exacerbation with respiratory failure. All patients were given bi-level positive airway pressure （BiPAP）. After treat- ment, detected breathing, heart rate and arterial blood gas analysis and other indicators. Results The total effective rate was 83.3%. After treatment, breathing, heart rate and blood gas analysis were significantly improved compared with before treat- ment,the differences were statistically significant（ P 〈 0.05 ）. Conclusion Effieacy of NIPPV in COPD of acute exacerbation with respiratory failure is significant. NIPPV can significantly improve the blood of patients with COPD, lung function and clini- cal symptoms, it is worth in clinical application.
