摘    要:目的对广东省妇幼保健院微生物实验室2010年分离的5种临床最常见病原菌的分布及耐药性进行分析,为临床感染性疾病送检标本和抗菌药物的合理选择及调整提供实验室依据。方法分析临床送检标本资料,所有标本按《全国临床检验操作规程》培养、分离,经全自动细菌鉴定分析系统(VITEK-2compact)进行鉴定及药敏试验,应用WHONET5.4软件分析药敏结果。结果细菌培养以呼吸道标本为主,常见菌的检出率在儿科系列病房均超过30%;在导管和支气管冲洗液等检出率平均达27.66%以上。常见菌对多种抗生素耐药,其中铜绿假单胞菌对亚胺培南的耐药达14.9%,但对氨基糖苷类、喹诺酮类抗生素均有较高的敏感率。结论病原菌检出率高,儿科系列病房应重视病原菌检测;革兰阴性条件致病菌常见。及时掌握病原菌及耐药性,有利于指导临床合理选择抗菌药物,预防医院感染的发生。

Abstract:Objective To investigate the drug resistance of common clinicalpathogens isolated in clinical microbiology laboratory in our hospital in 2010,and provide basis for clinical infectious diseases diagnosis and therapy.Methods Retrospective analysis of the isolated strains was performed in 2010.The bacteria were cultured and isolated according to National Clinical Laboratory Operation Manual,and then identified and tested by Compact VITEK-2 with CLSI 2007 criterion.The data were analysed by WHONET5.4.Results Five common pathogenic bacteria were identified in respiratory tract specimens in the department of pediatrics ward,with a positive rate 〉30%;the positive rates of the five common bacteria of the samples of the catheter and the bronchial washing,was 〉27.66%.Resistance rates for all agents and all organisms were high,among which Pseudomonas aeruginosa had a resistant rate to imipenem was 14.9%,but was more sensitive to aminoglycosides and fluoroquinolone antibiotics.Conclusion The detection rates of common pathogenic bacteria were highest in the department of pediatrics ward.Gram-negative pathogenic bacteria were common.It is importance to enhance the monitoring of pathogenic bacteria and determine their resistance to guide the reasonable clinical medication.
