利培酮与氯氮平治疗精神分裂症比较

目的 :比较利培酮和氯氮平治疗精神分裂症的疗效和安全性。方法 :利培酮组 30例 (男性 12例 ,女性 18例 ,年龄 34a±s 10a ,BPRS评分 5 4分± 5分 )用利培酮 1～ 8mg/d ,po ,bid ;氯氮平组 30例 (男性 14例 ,女性 16例 ,年龄 33a± 11a ,BPRS评分 5 5分± 5分 )用氯氮平 5 0～ 40 0mg/d ,po ,bid ;均以BPRS ,TESS评定观察 8wk。结果 :利培酮组有效率为 83% ,氯氮平组为 80 % (P >0 .0 5 )。利培酮组对阴性症状起效较早 ,对兴奋躁动控制较差。利培酮组较多见副作用为锥体外系症状 ( 2 7% ) ,与氯氮平组 ( 3% )比较差异有显著意义 (P <0 .0 5 ) ,其他副作用较少而轻。结论 :利培酮与氯氮平疗效相似 ,适宜剂量 (≤ 4mg/d)的利培酮是一种安全有效的抗精神病药。

Risperidone vs clozapine in treatment of schizophrenia

AIM: To compare therapeutic effects and safeties between risperidone and clozapine in the treatment of schizophrenia. METHODS: Thirty Patients with schizophrenia (M 12, F 18; age 34 a±s 10 a; BPRS score 54±5) received risperidone 1_8 mg/d, po, bid, were compared with another thirty patients with schizophrenia (M 14, F 16; age33 a±11 a; BPRS score 55±5) received clozapine 50～400 mg/d, po, bid. Therapeutic effects and safeties were evaluated with BPRS, TESS for 8 wk. RESULTS: The effective rate of risperidone group was 83%, while clozapine group was 80 % (P>0.05). Risperidone takes effect earlier for negative symptoms, but it was relatively poor to control excitment and impetuousness. The common adverse reactions of risperidone group were extrapyramidal symptoms (27%), while clozapine group was 3% (P<0.05). The others were relatively less and light. CONCLUSION: Risperidone and clozapine has similar therapeutic effect, risperidone in appropriate dosage (≤4 mg/d) is an effective and safe antipsychotic agent.
