ICU呼吸机相关肺炎的监测与预防

目的:探讨目标监测对控制重症监护病房患者医院感染肺炎的效果。方法:持续追踪观察所有入住ICU患者的医院感染发生状况及危险因素,监督和指导感染预防控制措施的执行,持续质量改进,定期培训与反馈监测结果。结果:监测对象中共发生医院感染46例次,其中下呼吸道感染23例次,构成比为50%;在呼吸机的使用率上升的情况下,呼吸机相关肺炎千日感染率由原来的29.41‰下降为9.69‰。结论:实施目标监测可以提高医务人员对医院感染的防控意识,规范医疗行为,及时掌握医院感染变化情况并采取有效的预防控制措施,通过加强环节质量管理,从而有效地降低医院感染发生率。

Monitoring and prevention of ventilator-associated pneumonia in ICU

Objective To investigate the effect of targeted surveillance to control hospital-acquired pneumonia in patients in intensive care unit(ICU).Method The continuous observation were given to all admitted to the ICU patients,and supervise and guide implementation of measures to infection prevention and control,continuous quality improvement,and regular training and feedback monitoring results to ICU medical staff′s.Results 23 cases(50%) of lower respiratory infection were found in 46 cases of nosocomial infection.In the case of increase in the utilization rate of the ventilator,for a thousand days′ infection rate of ventilator-associated pneumonia(VAP) dropped from 29.41‰ of the original to 9.69‰.Conclusion Targeted surveillance can improve the medical staff′s awareness of hospital infection prevention and control,regulate medical practices,grasp the hospital infection changes and develop effective measures,and reduce the occurrence of VAP.
