保留子宫动脉上行支的改良子宫全切除术价值探讨

目的 :探讨经腹保留子宫动脉上行支的改良筋膜内全子宫切除术的临床价值。方法 :对 30例子宫良性病变需子宫切除者采用新术式为研究组 ,选同期传统子宫全切术 30例为对照组 ,比较两组术中出血 ,手术时间 ,术后病率 ,术后 1年随访慢性下腹痛发生率 ,性生活质量 ,手术后的性激素水平 ,卵巢情况。结果 :研究组手术时间短 ,术后病率低 ,慢性下腹痛发生率少 ,性生活无变化 ;手术前后血清性激素水平无改变 ,卵巢无囊性增大 ,与对照组相比有显著差异。结论 :保留子宫动脉上行支的改良子宫全切除术是手术治疗子宫良性病变较好的手术方法

The value of reserving ascending uterine artery for reforming whole uterectomy

Objective: To discuss the clinical value of reserving ascending uterine artery for reforming whole uterectomy within the sinewy membrane. Methods: 30 cases as a research group who need urterectomy by a new fashioned operation due to benign uterine pathological change, and select 30 corresponding cases as a comparison who have received traditional uterectomy, and comparing the two groups by way of haemorrhage in operation, operative time and post operative disease rate. A year later after operation, a random investigation was made on the frequency chronic lower, abdominal pain, the quality of sex life, the level of post operative sex hormone, and the condition of ovary. Results: The less frequency of chronic lower abdominal pain, no variance of sex life, no change of serum sex hormone level before and after the operation, no increase in an ovarian cyst. Conclusion: Reserving ascending uterine artery for reforming whole uterectomy is comparatively good operative way of treating benign uterine pathological change.
