胰腺实性假乳头状瘤CT表现及病理基础

目的探讨胰腺实性假乳头状瘤的CT表现及病理学基础以提高对该肿瘤的认识及影像学诊断水平。方法回顾2010年1月～2012年10月经手术及病理证实的11例肿瘤患者的临床资料、CT表现、病理结果等。结果患者以中青年为主；肿瘤可发生在胰头、体及尾部，但胰尾部较多；肿瘤以囊实性为主，部分为实性或囊性成分为主；肿瘤包膜大多完整；CT平扫表现为低密度灶，增强扫描实性成分轻中度强化，强化程度低于正常胰腺组织。囊性成分不强化。病理表现为肿瘤假乳头和血窦样肿瘤血管的形成。结论该肿瘤的CT增强扫描有着特征性影像学表现并与病理密切相关，充分认识其影像学表现及病理基础更容易对该疾病做出明确诊断。

CT diagnosis and pathologic feather of solid pseudopapillary tumors of pancreas

Objective To explore spiral CT performance SPTP of the pancreas and pathologic basice in order to im- prove the diagnosis of SPTP. Methods Data from 11 pathologically confirmed cases of SPTP from 2010/1 to 2012/10 were retrospectively analysed. The CT scan and pathologic result and Clinical data were conducted in all cases. Results Most of the cases are young. Most of tumors were located in the tail of the pancreatic.Few in pancreatic head or body. Most of eases presented cystic-solid tumor, few was cystic or solid tumor. The capsul was complete in 9 cases and in- complete in 2 cases. The plain CT scan showed isodense or relatively low-density areas. In contrast-enhanced phase images,the solid parts showed mild to moderate enhancement in arterial phase,and continued to strengthen at portal vein and delayed phase. But they were all below the normal. The cystic areas does not showed enhancement.Pathologic appearance：The pseudopapillary structure and Fibrous capsule and sinusoid-like blood vessels were remarkable. Conclusion There is the feather in spiral CT contrast-enhanced scan of SPTP, this feather has its pathologic basicses. Realizing the imaging appearance and the Pathologic basics of SPTP deeply was helpful to diagnose svrP.
