羟基喜树碱与氟尿嘧啶、顺铂联合治疗中晚期食管癌58例

目的　观察HCPT + 5FU +DDP方案治疗食管癌的疗效。方法　中晚期食管癌 5 8例。以HCPT周期剂量不同分为A、B两组 ,两组DDP、5 -FU用法及用量均相同。A组HCPT 5 0mg。B组HCPT为 3 0mg。结果　原发灶肿瘤总有效率为 6 5 5 % (3 8/5 8) ,转移癌总有效率为 3 7 5 % (3 /8) ,明显低于前者 (P <0 0 5 ) ,初治者有效率高于复治者。A组有效率高于B组有效率。有效者中位缓解期为 4 2个月 ,中位生存期为 8 8个月 ,1年存活率及 2年存活率分别为 2 4% (14 /5 8) ,17% (10 /5 8) ,主要毒性反应为消化道反应及骨髓抑制。结论　HCPT、DDP、5FU对食管癌有较好疗效且毒性不大 ,可作为食管癌的治疗方案

Hydroxycamptothecin, Cisplatin and 5 - Fluorouracil Combined Therapy in the Treatment of Advanced Esophageal Cancer: A Report of 58 Cases

Objective To evaluate the result of combination chemotherapy of Hydroxycamptothecin(HCPT),Cisplatin and 5-Fluorouracil in the treatmeant of advanced esophageal cancer.Methods From 1996 through 1998,58 patients with advanced esophageal cancer were entrolled in this study.The median age of the patients was 61.Squamous-cell cancer was the most common type of malignancy.All but 19 patients received no previous chemotherapy.Results Complete response was observed in 11 patients and partial response in 27 patients,with an overall response rate of 65.5%. The response appeared to correlate with dose intensity,as 74.4% response rate was achieved with a dose of HCPT 50 mg versus 60% response rate with HCPT 30 mg.The median response duration was 4.2 months and the mediansurvival period was 8.8 months.Nausea,vomiting and neutropenia were the main adverse reactions in all treated cases.Conclusions A high response rate was obtained in advanced esophageal cancer treated by HCPT,Cisplatin and 5-Fluorouracil with tolerable toxicity.
