不同胰岛素对口腔颌面部多间隙感染合并糖尿病的治疗体会

目的:探索口腔颌面部多间隙感染合并糖尿病患者的最佳胰岛素治疗方案。方法:将2006年1月~2012年12月在我科住院的57例口腔颌面部多间隙感染合并糖尿病患者随机分为3组。A组:"三短一长"强化注射组;B组:预混胰岛素组;C组:中性胰岛素组。同时给予切开引流、抗感染、全身营养支持治疗,比较治疗后实验室检测血糖达标时间、伤口平均愈合时间、低血糖发生次数。结果:采用"三短一长"强化注射组其血糖达标时间及平均愈合时间明显少于其它两组(P0.05),而低血糖发生次数三组间无显著差异(P0.05)。结论:对口腔颌面部多间隙感染合并糖尿病患者采用"三短一长"强化注射胰岛素法控制血糖,能明显缩短治疗疗程及住院时间,降低死亡率,提高治疗效果。

Treatment of oral and maxillofacial region multiple space infection of patients with diabetes mellitus by different insulin

Objective To explore the optimal treatment plan of insulin on infection of oral and maxillofacial region in patients with diabetes mellitus. Methods From January 2006 to December 2012,57 cases of oral and maxillofacial multiple space infection with diabetes mellitus were randomly divided into three groups, which included Group A of three short and one long, group B of premixed insulin group, group C of neutral insulin group, meanwhiel they were given the incision drainage, anti- infection and nutrition support treatment to compare the time of blood sugar decreasing to normal range, average healing time of wound and the frequency of hypoglycaemia. Results The time of blood sugar decreasing to normal range and average healing time of wound in A group were significantly less than the other two groups （P〈0.05）,but the frequency of hypoglycaemia were no significant difference in the three groups （P〉0.05）. Conclusion Application of three short and one long strengthening method to control blood sugar can obviously shorten the treatment course and the length of hospital stay,reduce mortality and improve therapeutic effect, when patient with diabetes mellitus complicates oral and axillofacial multiple space infection.
