联合门静脉/肠系膜上静脉切除重建的胰十二指肠切除术治疗胰腺癌的临床探讨

目的探讨联合门静脉/肠系膜上静脉切除重建的胰十二指肠切除术治疗胰腺癌患者的效果。方法选取2017年3月至2018年9月于本院接受治疗的胰腺癌患者102例并通过随机数字表法分成两组,每组51例。参考组患者采用常规手术切除,观察组患者采用联合门静脉/肠系膜上静脉切除重建的胰十二指肠切除术,比较两组临床指标、术后并发症以及术后生存情况。结果观察组患者术中出血量多于参考组,手术时间、术后住院时间均长于参考组(P<0.05);观察组术后并发症总发生率低于参考组(P<0.05)。术后6个月及术后1年,观察组生存率均高于参考组(P<0.05)。结论胰腺癌患者应用联合门静脉/肠系膜上静脉切除重建胰十二指肠切除术,能提高手术疗效和患者生存率。

Pancreaticoduodenectomy combined with portal vein/superior mesenteric vein resection and reconstruction for the treatment of pancreatic cancer

Objective To investigate the effect of combined portal vein/superior mesenteric vein resection and reconstruction of pancreaticoduodenectomy for patients with pancreatic cancer.Methods A total of 102 patients with pancreatic cancer who were treated in our hospital from March2017 to September 2018 were selected and divided into 2 groups by random number table method,with 51 cases in each group.Patients in the reference group were surgically removed,and patients in the observation group were combined with portal vein/superior mesenteric vein.Resection and reconstruction of pancreaticoduodenectomy,the clinical indicators,postoperative complications and postoperative survival of the two groups were compared.Results The intraoperative blood loss of the observation group was more than that of the reference group,and the operation time and postoperative hospital stay were longer than those of the reference group(P<0.05).The total incidence of postoperative complications in the observation group was lower than that in the reference group(P<0.05).At 6 months after operation and 1 year after operation,the survival rate of the observation group was higher than that of the reference group(P<0.05).Conclusion The combined portal vein/superior mesenteric vein resection and reconstruction of pancreaticoduodenectomy for patients with pancreatic cancer can improve the surgical efficacy and the survival rate of patients.
