低频脉冲治疗广泛性子宫切除术后尿潴留的临床研究

目的研究膀胱功能治疗仪低频脉冲治疗广泛性子宫切除术后尿潴留的疗效。方法广泛性子宫切除术后尿潴留的住院患者60例随机分为膀胱功能治疗仪低频脉冲治疗组、针灸治疗组和对照组，每组各20例，对3组治疗5天拔除尿管后有效率、最大尿流率、膀胱残余尿量和膀胱逼尿肌压力进行比较分析。结果治疗仪组、针灸组和对照组治疗有效率分别为90％、75％和50％，治疗仪组和针灸组治疗有效率高于对照组（X^2=6．936，P=0．008）。治疗后治疗仪组、针灸组和对照组最大尿流率量分别为（16．6±2．35）、（13．3±1．77）和（10．1±1．59）mL／s；残余尿量分别为（20．5±5．68）、（25．5±7．72）和（60．5±10．26）mL；膀胱逼尿肌压力分别为（78．5±7．88）、（75．5±7．88）和（56．5±7．88）mmH2O.治疗后针灸组和治疗仪组的最大尿流率、膀胱逼尿肌压力明显〉对照组（P〈0．05），残余尿量明显〈对照组（P〈0．05）；且治疗仪组效果优于针灸组（P〈0．05）。结论膀胱功能治疗仪能有效治疗广泛性子宫切除术后尿潴而且具有安全、简便和患易于接受等优点。

Treatment of urinary retention after radical hysterectomy by low frequency pulse device

Objective To evaluate the therapeutic effect of urinary retention after radical hysterectomy by bladder treatment apparatus. Methods 60 patients of urinary retention after radical hysterectomy were divided into three groups ： the treatment group, the acupuncture group and the control group, 20 patients in each group. Five days after treatment, the efficacy rate, maximum urine flow rate, bladder residual urine volume, and detrusor pressure of bladder were compared. Results The efficacy rate of the treatment group, the acupuncture group and the control group was 90% ,75% and 50%. The efficacy rate of the treatment group and the acupuncture group was higher than the control group（x^2 = 6. 936,P = 0. 008）. After treatment, the maximum urine flow rate of the treatment group, the acupuncture group and the control group was （ 16.6 ± 2.35 ）, （ 13.3 ± 1.77 ） and （ 10.1 ± 1.59 ） mL/s ; the bladder residual urine volume was （ 20.5± 5.68 ）, （ 25.5 ± 7.72 ） and （ 60.5± 10.26 ） mL; the detrusor pressure was （ 78.5 ± 7.88 ）, （ 75.5 ± 7.88 ） and （56.5± 7.88 ） mmH： O. After treatment, the maximum urine flow rate and detrusor pressure of the treatment group and the acupuncture group was higher than the control group（ P 〈 0.05 ） ; bladder residual urine volume was lower（ P 〈 0.05 ）. The therapeutic effect of the treatment group was superior to that of the acupuncture group （P 〈 0.05）. Conclusions Urinary retention after radical hysterectomy can be effectively treated by bladder treatment apparatus, which is safe, convient and easy to be accepted by patients.
