| 摘 要： | 目的探讨血清B型脑钠肽(BNP)水平与连续性肾脏替代治疗(CRRT)急性肾损伤患者预后情况的相关性。方法选择2012年1月至2014年6月住院治疗的156例急性肾功能损伤(AKI)患者,利用受试者工作特征曲线(ROC)、约登指数、多元Logistic回归等综合分析BNP水平与AKI患者临床预后。结果生存组患者BNP水平明显低于病死组患者(1 155.3±212.1)pg/mL vs.(1 877.3±421.7)pg/mL](P0.05)。CRRT治疗参数方面,两组差异无统计学意义(P0.05)。BNP的AUC为0.873(95%CI,0.781~0.926),最佳切点为1 327.3pg/mL,灵敏性和特异性分别为84.2%和86.8%。Logistic回归分析发现BNP是预测AKI患者预后的独立危险因素,回归系数为0.031,决定系数为0.051。结论 AKI患者血清BNP水平与疾病预后密切相关,是AKI预后的独立危险因子。
