湿热型胃脘痛与胆汁反流性胃炎的相关性

目的探讨湿热型胃脘痛与胆汁反流性胃炎的相关性。方法采取随机对照方法,选取湿热型胃脘痛(观察组)患者及非湿热型胃脘痛(对照组)患者各250例,通过临床表现,并结合胃镜所见的胃黏膜病变、胃内胆汁量和胆汁颜色,胃黏膜活检的病理检查情况,对幽门螺杆菌感染率的检测以及B超或CT对肝胆系统的检查等对比观察,探讨其与胆汁反流性胃炎的关系。结果观察组符合胆汁反流性胃炎者194例,对照组19例,差异有显著性意义(P<0.01)。观察组中伴有口苦症状者均患有胆汁反流性胃炎,非胆汁反流性胃炎者则无口苦症状;伴有胆道系统疾病者明显多于对照组,且均发生在胆汁反流性胃炎病例中。病理活检示观察组病变程度比对照组严重。结论湿热型胃脘痛尤其伴有口苦症状者与胆汁反流性胃炎有密切相关性。

Correlativity of Stomachache of Damp-Heat Pattern with Bile Reflux Gastritis

Objective To study the correlativity of stomachache of damp-heat pattern with bile reflux gastritis.Methods Five hundred patients with stomachache were selected randomly and divided into observation group (n=250,with damp-heat pattern) and control group (n=250,not damp-heat pattern).The correlativity of stomachache of damp-heat pattern with bile reflux gastritis was studied by clinical manifestations,gastric mucosal lesion,volume and colour of bile in the stomach,infection rate of Helicobacter pylori (Hp),and liver-gallbladder examination by B ultrasound or CT.Results There were 194 cases of bile reflux gastritis in observation group,while 19 cases in control group,resulting in sigificant difference (P<0.01).In observation group,bitter taste of mouth was a typical symptom in every patient with bile reflux gastritis;The cases of biliary system disease which all occurred in bile reflux gastritis patients were more than those in control group.Pathological biopsy showed that pathogenetic condition was more severe in observation group than that in control group.Conclusion There was close relationship between bile reflux gastritis and stomachache of damp-heat pattern espically when accompanied by bitter taste of mouth.
