新生儿重症感染性肺炎降阶梯疗法的治疗效果

目的观察降阶梯疗法治疗新生儿重症感染性肺炎的临床疗效。方法选取我院新生儿重症感染性肺炎患儿84例(2017年3月-2018年8月),随机分为抗生素降阶梯疗法治疗的观察组(42例)与经验性抗生素疗法治疗的对照组(42例),观察患儿治疗效果、临床症状改善情况、血清相关指标情况及不良反应发生率。结果观察组治疗总有效率(92.86%)优于对照组(764.19%),P <0.05;观察组体温恢复、呼吸平稳和肺啰音消失时间均短于对照组,P <0.05;观察组血清中WBC(血液中白细胞)、PCT(降钙素原)、CRP(C-反应蛋白)水平均低于对照组,P <0.05;不良反应发生率观察组(9.52%)与对照组(11.90%)异无统计学意义,P> 0.05。结论给予新生儿重症感染性肺炎降阶梯疗法治疗,能提高患儿治疗效果,有效改善临床症状及血清相关指标,不良反应少,安全性高,值得借鉴。

Therapeutic Effect of Ladder Therapy for Severe Infectious Pneumonia in Neonates

Objective To observe the clinical effect of step-down therapy on neonatal severe infectious pneumonia. Methods From March 2017 to August 2018, 84 cases of children with neonatal intensive infectious pneumonia in our hospital, were randomly divided into antibiotics drop ladder therapy treatment group(42 cases) and empirical antibiotic therapy in control group(42 cases) and to observe the therapeutic effect, clinical symptoms improve the situation, the related parameters of serum and incidence of adverse reactions. Results Comparison of total effective rate of treatment [observation group(92.86%) > control group(764.19%)], P < 0.05;the time of body temperature recovery, breathing stability and disappearance of lung rales in the observation group were significantly shorter than those in the control group, P<0.05;The levels of WBC(white blood cells), PCT(procalcitonin), and CRP(C-reactive protein) in the serum of the observation group were significantly lower than those of the control group, P <0.05;the incidence of adverse reactions in the observation group(9.52%) and the control group(11.90%) was no significant difference, P > 0.05. Conclusion The treatment of neonatal severe infectious pneumonia with step-down therapy can improve the therapeutic effect of children, effectively improve clinical symptoms and serum related indicators, with less adverse reactions and high safety.
