非脱垂子宫阴式切除术的临床应用

摘    要:目的对非脱垂子宫经阴道(阴式)切除开展临床研究。方法对术前已排除恶性病变的非脱垂子宫切除的患者,分为阴式和腹式两组进行临床观察。结果阴式组较腹式组平均手术时间短(P<0.001),平均出血量稍多(P>0.05),术后疼痛、发热率低(P<0.001),肛门排气时间早(P<0.001),术后平均住院时间短(P<0.001)。结论阴式子宫切除较腹式子宫切除手术时间短,损伤少,腹腔干扰轻,术后胃肠功能恢复早,疼痛轻,发热率低,术后一周内出院的多,腹壁不留切口疤痕,深受患者欢迎。对于非脱垂子宫≤80天妊娠子宫大小的患者,有切除子宫的手术指征,阴式子宫切除是一种较好的方法。

Abstract:Objective:To study the transvaginal hysterectomy for a moderate enlarged normal positioned uterus.Methods:A clinical observation and comparison were made between transvaginal hysterectomy and abdominal hysterectomy for moderate enlarged normal positioned uteruses.Results:Transvaginal hysterectomy can be accomplished with shorter operating duration( P <0.001),less postoperative pain and fever( P <0.001),earlier anus exhaustion( P <0.001),shorter length of postoperative hospital stay( P <0.001),but a little more bleeding.Conclusion:Compared with abdominal hysterectomy,transvaginal hysterectomy in associated with less operating duration,less damage,lighter interference with the abdomen,earlier recovery of indigestion aftr operation,lighter pain ,fewer complications,a week hospital stay after operation for most and no scar .So transvaginal hysterectomy is better for those who need to undergo hysterectomy when their normal positioned uterused are 80 pregnant day or fewer big.
