新生儿缺氧缺血性脑病血清与脑脊液胆红素含量的变化

①目的探讨缺氧缺血性脑病(HIE)病儿血清及脑脊液胆红素含量的变化及临床意义.②方法HIE组(38例)和对照组(12例)新生儿均于生后48h内抽取股静脉血,同时行腰穿抽取脑脊液,检测胆红素含量并进行对比.③结果HIE组病儿血清、脑脊液胆红素含量与对照组比较明显增高(F=3.8276,5.6283,a=2.9132～4.8561,P＜0.05).轻度HIE组脑脊液中胆红素含量比对照组明显增高(F=5.6283,a=3.6921,P＜0.05).轻、中、重度HIE组病儿之间血清、脑脊液胆红素含量差异均有显著性(F=3.8276,5.6283,q=3.∞25～4.8561,P＜0.05).④结论HIE病儿脑内胆红素水平明显高于正常,因此对HIE病儿应积极治疗高胆红素血症,以减少后遗症发生.

CHANGES OF BILIRUBIN IN SERUM AND CSF IN NEONATES WITH HYPOXIA-ISCHEMIC ENCEPHALOPATHY

Objective To explore the changes and their significance of bilirubin in serum and CFS in neonates with hypoxia-ischemic encephalopathy(HIE). \ Methods The venous blood and CSF were collected from 38 neonates with HIE (cases group) and 12 healthy neonates (control group) within 48 hours after birth.The serum bilirubin and CSF bilirubin were measured and compared.\ Results \ The serum bilirubin and CSF bilirubin levels in HIE group were significantly higher than those of the control group (F=3.827 6, 5.628 3, q=2.913 2-4.856 1, P<0.05). The CSF bilirubin levels in mild HIE were significantly higher than those of the control group(F= 5.628 3 , q=3.692 1, P<0.05). The difference of CSF bilirubin levels between mild, moderate and severe HIE were significant(F= 3.827 6,5.628 3 , q=3.092 5-4.856 1, P<0.05).\ Conclusion The CSF bilirubin level in neonates with HIE was significantly higher than that of healthy neonates. Hyperbilirubinemia should be treated actively for neonates with HIE so as to reduce the sequelae.
