AECOPD住院期间cTnT预后价值初探

摘    要:目的评价cTnT在慢性阻塞性肺疾病急性加重期(AECOPD)住院期间的预后价值。方法采用前瞻性队列研究方法,测定82例AECOPD住院患者血清cTnT、Mb、CK-MBmass,收集患者临床资料,随访临床结局(死亡或存活)。采用单因素、Logistic回归分析进行预后分析。结果logistic多元回归分析结果显示,PaO2、cTnT增高、P(A-a)O2、年龄为AECOPD住院期间的主要预后因素(P〈0.05)。结论cTnT增高是AECOPD住院期间的主要预后指标。

Abstract:Objective To assess the prognostic value of cTnT elevation during hospital stay for acute exacerbation of chronic obstructive pulmonary disease. Methods Eighty-two consecutive patients with confirmed acute exacerbation of chronic obstructive pulmonary disease were enrolled in this prospective study. Serum cTnT,myoglobin(Mb),creatine kinase-MB mass(CK-MBmass)were measured and a variety of clinical and laboratory variables were collected,then followed up clinic outcome(death and survival). Univariate analysis and Logistic regression analysis was used to prognosis analysis. Results Age, PaO2, P(A-a)O2 and cTnT elevation was identified as principal predictors of in-hospital death in a logistic regression model. Conclusions Elevated cTnT is a principal prognostic factor of in-hospital death in patients admitted for acutely exacerbated chronic obstructive pulmonary disease.
