自控镇痛用于前列腺切除术后膀胱痉挛的效果观察

目的探讨自控镇痛用于前列腺切除术后膀胱痉挛的效果。方法将117例良性前列腺增生切除术后的患者根据其意愿分为两组,实验组61例采用硬膜外自控镇痛泵镇痛,对照组56例采用传统药物间断镇痛法止痛。通过两组患者视觉模拟评分,对患者的镇痛效果进行评价,同时对两组患者膀胱痉挛发生情况、留置尿管时间、膀胱冲洗时间、冲洗液转清时间等指标进行观察,并对研究结果进行统计学处理。结果实验组镇痛效果优于对照组,实验组膀胱痉挛发生率明显低于对照组,留置尿管时间、膀胱冲洗时间、冲洗液转清时间都较对照组有明显缩短,差异具有统计学意义(P<0.01)。结论良性前列腺增生切除术后应用自控镇痛泵防止膀胱痉挛效果明显,有利于术后康复。同时,应重视应用自控镇痛泵的护理,以及减轻和缓减膀胱痉挛的其他护理措施。

The effect of patient controlled analgesia on bladder spasm after prostatectomy

Objective To study the effect of patient controlled analgesia(PCA) on bladder spasm after prostatectomy.Methods 117 benign prostate hyperplasia(BPH) patients after prostatectomy were divided into two groups according to their desire.61 patients in experiment group took PCA to ease pain,and 56 patients in control group took traditional pharmaceuticals discontinuity analgesia.Then,the effect of pain management was evaluated using Visual analogue scale(VAS).At the same time,the index of bladder spasm,time of indwelling ureter,time of bladder rinsing and the time of rinsing liquid tuned into clear were observed and analyzed.Results The effect of pain management was significantly better in the experiment group than that of control group.The rate of bladder spasm was significantly lower than that of control group and the time of indwelling ureter,time of bladder rinsing,and the time of rinsing liquid tuned into clear were significantly shorter than that of control group(P<0.01).Conclusions The use of PCA in BPH patients after prostatectomy was helpful to prevent bladder spasm and for the recovery post-operation.At the same time,we should attach importance to the nursing for patients with PCA and other nursing measures to alleviate bladder spasm.
