不同胎龄早产儿肺透明膜病的X线征象特点

目的 提高对不同胎龄早产儿肺透明膜病的X线征象特征性的认识,增强早期诊断的能力.方法 回顾性分析40例早产儿肺透明膜病的患儿首次X线分级和患儿胎龄、发病时间、出生时体重与患儿临床预后关系.结果 患儿首次胸片Ⅳ级5例（死亡3例）,Ⅲ级15例（死亡1例）,Ⅱ级10例（死亡1例）,Ⅰ级10例.胎龄、出生体重均与首次胸片等级有差异（P =0.0119,P=0.0324）.X线分级与胎龄、体重、发病时间呈负相关,而与临床预后呈正相关.结论 X线摄影是临床诊断早产儿肺透明膜病的最重要的影像方法,应结合临床不断提高对HMD的早期诊断的能力.

The X-ray diagnosis different age preterm infants with hyaline membrane disease

Objective To discuss X-ray diagnosis of newborn hyaline membrane disease （HMD）. Methods The newborns with hyaline membrane disease by X-ray were sCected and comparably analyzed. Results Accoding to X-ray manifestations,all cases were divided into four stages IV stage （n=5）, Ⅲ stage （n= 15）, Ⅱ stage （n= 10）, Ⅰ stage （n= 10）. Typical X-ray signs included the pulmonary lucency decreasd, wide-ranging net and grain shadowes of high density, and in companing with a lot of air brunchus sing. There was statistically significant difference between the fetal age or birth weight and the first time X-findings （P =0. 0119, P =0 . 0324）. The younger the fetal age and the lower the birth weight, the more likely that the first X-ray findings would be below stage Ⅱ. Conclusion X-ray is the most important imaging method in diagnosis of HMD, and shall be improved the ability of diagnosis.
