摘    要:目的探讨磁敏感加权成像(SWI)技术在脑梗死出血性转化的价值及其临床相关性。方法选择45例临床和影像明确的脑梗死出血性转化的患者作为研究对象,进行CT、常规MRI(T1WI、T2WI、DWI序列)和SWI序列扫描的自身对比研究,CT和MR检查的间隔不超过12h,MRI序列和SWI序列同时进行。结果45例脑梗死出血性转化的患者,SWI诊断脑出血45例,常规MRI序列诊断29例,CT诊断35例,阳性预测值分别为100%、64.4%、77.8%。结论SWI诊断脑梗死出血性转化比常规MR及CT具有更好的临床应用价值,可作为急性脑梗死及溶栓治疗后常规复查方法。

Abstract:Objective To investigate the value of susceptibility weighted imaging (SWI) technique in cerebral infarction and hemorrhagic transformation and its clinical significance. Methods 45 cases of cerebral infarction with hemorrhagic transformation confirmed by clinic and imaging datah were selected as research objects and all underwent CT, conventional MRI (T1 WI, T2 WI, DWI sequence) and SWI sequence scanning, and then, the comparative study was performed in this work. The interval for CT and MR examination was less than 12 h and MRI sequences and SWI sequences were carried out at the same time. Results Of these 45 cases, 45 cases of cerebral hemorrhage were diagnosed with SWI, 29 cases were di- agnosed with routine MRI sequence, and 35 cases were diagnosed with CT. The positive predictive rates were 100%, 64.4%, and 77.8%, respectively. Conclusion The diagnosis of cerebral hemorrhage transformation with SWI is better than that with conventional MR or CT, which can be used as a routine method for reexamining the post-treatment effects of cerebral infarction and thrombolytic therapy.
