人工髋关节置换术治疗高龄股骨颈骨折78例疗效分析

目的探讨人工髋关节置换术治疗高龄股骨颈骨折的临床疗效，从而为高龄股骨颈骨折患者寻找安全有效的治疗方法。方法对78例高龄（年龄在70岁以上）股骨颈骨折患者采用人工髋关节置换术治疗的临床资料进行回顾性分析，其中采用骨水泥型全髋关节18例，生物型全髋关节52例，人工半髋置换8例。随访观察治疗效果并采用Harris评分进行疗效评价。结果随访12～60个月，平均38个月。髋关节按Harris评分：优54例，良17例，可4例，差3例，总优良率91％。结论人工关节置换术治疗老年人股骨颈骨折，患者可早期下床功能锻练，缩短了卧床时间，可有效预防长期卧床所致并发症的发生，术后髋关节功能恢复良好，能明显提高患者术后生活质量。

Artificial total hip replacement in treatment of transcervical fractures in seventy-eight aged patients

Objective To discuss the clinical effect of treating transcervical fracture with artificial hip replacement in aged patients. Methods Clinical data of 78 patients who were more than 70 years old and treated with the hip replacement were retrospectively analyzed. Eighteen cases were treated with cemented total hip replacement, 52 cases cementless total hip replacement and 8 cases artificial hemiarthroplasty. Results The patients were followed up for 12 to 60 months and 38 months on average. According to Harris hip joint score： 54 cases were excellent, 17 better, 4 good, 3 poor, and the gross satisfactory rate was 91%. Conclusions The old patients whose transcervical fractures were treated with artificial total hip replacement can ambulate early, decrease the bed-time, and prevent the complication of long-term bed. After operation, the recovery of the hip is good and the life quality of patients is improved.
