血小板参数血糖血脂与2型糖尿病微血管病变关系的观察

摘    要:目的探讨2型糖尿病(T2DM)患者血小板参数及血糖、血脂的变化,为提前干预2型糖尿病患者微循环障碍提供依据。方法选取235例住院及门诊T2DM微血管病变患者为T2DM微血管病变组,另选取健康体检者251例为对照,分别检测其血小板计数(PLT)、平均血小板体积(MPV)、血小板分布宽度(PDW)与空腹血糖(FPG)、血尿酸(UA)、三酰甘油(TG)、胆固醇(CH)、高密度脂蛋白胆固醇(HDL)、低密度脂蛋白胆固醇(LDL)等指标。结果①T2DM微血管病变组与对照组相比,FPG、TG、CH、MPV、PDW均显著增高(P〈0.05);PLT、HDL偏低,LDL、UA较高,但差异无统计学意义(P〉0.05)。②双变量相关分析显示:MPV与FPG呈正相关(r=0.13,P〈0.05),与PDW呈正相关(r=0.27,P〈0.01);MPV、PLT、UA、TG、CH、HDL、LDL间均无明显相关性。③用Logistic多元逐步回归方法,显示FPG(OR=7.654)、TG(OR=6.923)、TCH(OR=6.863)、PDW)OR=4.328)是2型糖尿病微血管病变的危险因素。结论糖尿病患者存在血小板功能异常及代谢紊乱,且此异常是T2DM微血管病变的危险因素,故控制糖尿病患者的各代谢指标,监测血小板参数,有助于防治糖尿病微血管病变。

Observing the relationship of thrombocyte parameter, blood glucose and blood fat with type 2 diabetes microangiopathy

Abstract:Objective To observe the change of Thrombocyte Parameter and the correlation analysis of Thrombocyte Parameter,blood glucose and blood fat in patients with type 2 Diabetes,to establish(provide)the foundation of pre-treating the microcirculation disturbance in patients with type 2 Diabetes.Methods To select 235 patients with type 2 diabetes microangiopathy(male 122,female 113) and 251 healthy people(male 134,female 117)to study Blood platelets count(PLT),mean platelet volume(MPV),platelet distribution width(PDW),blood-fasting sugar(FPG),blood uric acid(UA),triglyceride(TG),cholestero(lCH),high density lipoprotein cholestero(lHDL),low density lipoprotein cholestero(lLDL).Results ①The level of FPG,TG,CH,MPV,PDW in DM group were significantly higher than those in NC group(P<0.05),The level of PLT,HDL in DM group were lower than those in NC group,the level of LDL and UA were higher than those in NC group,but there was no significant difference in two groups(P>0.05).②there was a positive correlation between MPV and FPG(r = 0.13,P<0.05),and there was a positive correlation between MPV and PDW(r=0.27,P<0.01),there was no correlation between MPV,PLT,UA,TG,CH,HDL and LDL.③FPG(OR=7.654),TG(OR=6.923),TCH(OR=6.863),PDW(OR=4.328)were significant risk factors for type 2 Diabetes microangiopathy.Conclusion Disfunction of thrombocyte,blood fat disorderly and higher FPG were significant risk factors for type 2 Diabetes microangiopathy,therefore,to control the metabolism index could protect thrombocyte function and prevent microcirculation disturbance in the patients with type 2 diabetes.
