单侧椎板间开窗髓核摘除术治疗后外侧型腰椎间盘突出症

目的探讨单侧椎板间开窗髓核摘除术治疗后外侧型腰椎间盘突出症的适应证及疗效。方法对自2003年1月至2004年12月收治的34例术前经SCT或MR1确诊的后外侧型腰椎间盘突出症患者行单侧椎板间开窗髓核摘除术。结果全部病例得到3～24个月的随访(平均11个月)。疗效评定按照Nakai评级，其中优24例，良7例，可2例，差1例，优良率为91．2％。疗效差的1例考虑为术中过度牵拉神经根所致，术后卧床1周后缓解。本组病例未出现椎间隙感染等其他并发症。结论单侧椎板间开窗髓核摘除术是治疗外侧型腰椎间盘突出症的有效方法，临床效果满意。

Surgical treatment of lateral lumbar disc herniation by applying uni-vertebrai plate windowing lumbar discectomy

Objective To evaluate the indication and clinical results of treatment of lateral lumbar disc herniation by applying uni-vertebral plate windowing discectomy.Methods 34 cases were evaluated from January 2003 to December 2004.Diagnosis was made according to MRI and SCT findings.Uni-vertebral plate windowing discectomy were applied in all cases.Results All cases were followed up 3 to 24 month(mean 11 month).Clinical assessment was based on Nakai standard.Excellent was 24 cases,good was 7 cases,fair was two cases and poor was one which was concerned that nerve root over dragged in the operation might be the reason and symptom relieved after staying in bed for a week.No infection and other complications was found in the group.Conclusion Uni-vertebral plate windowing lumbar discectomy is an ideal method for treatment of lateral lumbar disc herniation and clinical outcome is satisfied.
