经胸前径路腔镜甲状腺手术的难点及其处理

目的探讨经胸前径路腔镜甲状腺手术的手术难点及其解决方法。方法总结该院2003年10月至2004年10月所行的20例经胸前径路腔镜甲状腺手术的临床资料。结果20例手术均顺利完成，无一例中转开放手术，无遗留永久并发症。结论经胸前径路腔镜甲状腺手术的难点是创造人工手术空间，显露腺体，预防及处理出血，估计腺体残留量及缝合颈前肌，通过掌握相应的手术技巧可以顺利地完成手术。

The difficulties and its managements in endoscopic thyroidectomy by anterior breast approach

Objective To investigate the operative difficulties and its managements in endoscopic thyroidectomy by anterior breast approach.Methods The clinical data of 20 cases of endoscopic thyroidectomy by anterior breast approach in our hospital from Oct. 2003 to Oct. 2004 were analyzed retrospectively.Results All of the 20 operations have been finished successfully,nobody has been transferred to open surgery and nobody left over permanent complication.Conclusion The difficulties of endoscopic thyroidectomy by anterior breast approach are the creation of operative space,exposure of the thyroid gland,prevention and management of the bleeding,estimation of the residual quantity and suture of the anterior cervical muscles.With correct operative techniques,this operation could be finished successfully.
