2型糖尿病患者TI水平相关因素分析

摘    要:目的分析2型糖尿病患者空腹血清真胰岛素(TI)水平的变化趋势及临床意义。方法采用微粒子化学发光免疫分析法测定本院收治的68例糖尿病患者(观察组)空腹血清TI水平,与同期入院体检的68例健康志愿者(对照组)比较;并测定各组患者空腹血糖(FBG)、尿酸(UA)、C肽水平及胰岛素抵抗指数(HOMA-IR),分析TI水平的改变趋势及临床意义。结果与对照组相比,观察组患者TI水平显著升高(P〈0.05);且观察组患者血清FBG、UA、C肽及HOMA-IR水平均显著升高(P〈0.05);经Pearson直线相关性分析,TI与血清FBG、UA、C肽及HOMA-IR水平均有直线正相关性(P〈0.05)。结论糖尿病患者TI水平异常增高,TI升高水平可反映血糖、尿酸代谢障碍,肾功能损伤程度及胰岛素抵抗水平,对糖尿病的临床诊断、治疗及预后评估有一定的指导意义。

Abstract:Objective To analyze the changing trends and clinical significance of fasting serum true insulin (true insulin, TI) level for type 2 diabetes. Methods The TI level of 68 cases of patients with diabetes (observation group) were determined using the method of micro particle chemiluminescence immunoassay, which were compared with the other 68 cases of healthy volunteers (control group) physical examined in our hospital. And the fasting plasma glucose (fasting blood glucose, FBG), uric acid (UA, uric acid), C peptide level and insulin resistance index (HOMA-IR) were also compared and analyzed between two groups. Results Compared with the control group, the TI level of patients in the observation group were significantly higher (P 0.05); And the other four indexes of serum PBG, UA, C peptide and HOMA-IR level were also significantly higher (P 0.05). Pearson linear correlation analysis showed that the level of TI had positive linear correlation with the level of PBG, UA, C peptide and HOMA-IR ( P 0.05). Conclusion The level of TI in type 2 diabetic patients is abnormally higher than the healthy people, and the TI elevated level can be related with high blood sugar level, uric acid metabolic disorders, kidney damage degree and the level of insulin resistance, which has certain guiding significance for clinical diagnosis, treatment efficacy and prognosis assessment of type 2 diabetes
