不同引流方法在甲状腺次全切术中应用的比较

目的研究与探讨橡胶管引流同套管针引流在甲状腺次全切术中应用的临床情况比较。方法选用2010年5月～2013年5月入住本院治疗甲状腺疾病、并且实施甲状腺次全切术的患者244例,将其随机均分为两组,对照组122例采用橡胶管引流方法进行治疗,治疗组122例采用套管针引流方法进行治疗。比较两组患者术后2d换药次数,患者满意度,不良反应以及引流、拆线和组织复位时间的具体情况。结果对照组术后2d换药（3.62±0.52）次,引流时间为（3.22±0.62）d,拆线时间为（7.75±0.22）d,组织复位时间为（4.72±0.32）d,81例患者满意、41例患者不满意;治疗组术后2d换药（1.32±0.32）次,引流时间为（1.69±0.35）d,拆线时间为（4.95±0.11）d,组织复位时间为（2.99±0.24）d,118例患者满意、4例患者不满意。治疗组患者各项指标均显著优于对照组（P〈0.05）。结论针对甲状腺次全切除术,采用套管针引流技术的效果显著优于传统的橡胶管引流,不但安全系数高,患者的满意度亦显著高于橡胶管引流法,值得临床推广。

Comparision on the application of different drainage method in subtotal thyroidectomy

Objective To study and investigate the different clinical situations between rubber drainage and trocar drainage used in subtotal thyroidectomy.Methods 244 cases of patients with thyroid disease,who accepted subtotal thyroidectomy in our hospital from May 2010 to May 2013,were randomly evenly divided into two groups.Patients in control group treated with rubber drainage,patients in treatment group treated with trocar drainage.The comparisons between two groups were conducted on frequency of dressing change two days after operation,patients ’ satisfaction,adverse reactions,and the time of drainage,taking out stitches and tissue reduction.Results In control group,dressing change for（3.62±0.52） times in two days after treatment,the drainage time was（3.22 ±0.62） days,the time of taking out stitches was（7.75±0.22） days,tissue reduction time was（4.72±0.32） days,81 patients were satisfied,and 41 patients were not satisfied.In treatment group,dressing change for（1.32 ±0.32） times in two days after treatment,the drainage time was（1.69±0.35） days,the time of taking out stitches was（4.95±0.11） days,tissue reduction time was（2.99±0.24） days,118 patients were satisfied,and 4 patients were not satisfied.Every indicator of treatment group patients was significantly better than that of control group（P＆lt;0.05）.Conclusion In subtotal thyroidectomy,the effect of trocar drainage technology is significantly better than that of traditional rubber drainage.Trocar drainage not only has a high safety coefficient,but also has a higher patients’ satisfaction than rubber drainage method.It is worth popularizing in the corresponding clinical.
