尿微蛋白量与糖尿病人白内障术后视力的关系

目的：了解糖尿病人和一般老年人白内障术后视力恢复程度及其与尿微蛋白系列的相互关系。方法：对近2年住院行白内障复明术的病人，顺序选糖尿病和无糖尿病的老年性白内障各50例，测定尿微蛋白系列和各项实验室检测作比较。结果：糖尿病组的尿微蛋白系列显著高于对照组。其中尿β2M高于正常上限者达50％；ALb82％；IgG52％。对照组分别是22％；14％；4％。P值＞0．01～0．001。是早期诊断糖尿病肾病的一个指标。结论：糖尿病视网膜病交往往合并糖尿病肾损害，早期DN的诊断不但能抓紧在DN的关键时期及时治疗，可望逆转DN进展．也有助于对这部分糖尿病白内障复明术的病员，加强随访观察和治疗，并可作为评定预后的指标。尿微蛋白系列强阳性者视力预后差，并发症多。

Evaluation of urine microprotein series test for predicating postoperatine visual of diabetic cataract

This report studies the result of urine microprotein series(UMPS) and other laboratory tests of 100 orderly selected admission patients, divided into two groups evenly: diabetic and senior cataract. Comparing the result, we fine that UMPS test of diabetic group is appeareutly higher than that of the contral group. In the diabetic group, the rate of the value Of urine β2M test higher above normal limitation reaches 50%, ALbb82% and IgG 52 %, while in the control group, the rate of urine β2M, ALb and IgG is 22 %, 14% and 4% respecticely (P<0.01~0. 001). The value of UMPS test is one of the diagnostic indicators of the early stage of diabetic nephritis. And diabetic retinopathy can always be complicated with diabetic nephritic lesion. Diagnosis of the early stage with diabetic nephritis, therefore, can not only help to treat it in time at its important stage to reverse its development, but also help to enhance observation and treatment after cataract surgery of diabeticpatients and be an indicator of assessment of postopeatine visual. The patient with strong positive result of UMPS test have a dismal prognosis because have serious complication.
